Early prediction of rheumatoid arthritis by magnetic resonance imaging in the absence of anti-cyclic citrullinated peptide antibodies and radiographic erosions in undifferentiated inflammatory arthritis patients: a prospective study.

OBJECTIVE: To assess the practicability of magnetic resonance imaging (MRI) in confirming the diagnosis of clinically suspected rheumatoid arthritis (RA), when anti-cyclic citrullinated peptide antibody and radiographic erosions are absent. METHODS: We prospectively involved 31 treatment-naive patients with early inflammatory arthritis. At the initial visit, X-rays and gadolinium-enhanced MRI of both hands, as well as serological examinations and acute phase reactants were performed. The scores of synovitis, bone edema, bone erosion and tenosynovitis of metacarpophalangeal and wrist joints we…

Severe laceration of flexor tendons after locking palmar plate fixation of distal radius fracture: a case report.

We report a case of flexor pollicis longus and flexor digitorum profundus(II) laceration as a result of the severe tenosynovitis after volar locking plate fixation of distal radius fracture.

Mycobacterium Intracellulare Infection of the Metacarpophalangeal Joint in a Patient With Rheumatoid Arthritis: A Case Report.

Background: Nontuberculous Mycobacterium (NTM) infections affecting musculoskeletal structures are rare, particularly in patients with well-controlled rheumatoid arthritis (RA). This case is reported to highlight the potential risk of focal tenosynovitis due to Mycobacterium intracellulare following intra-articular glucocorticoid injection. Case presentation: A 79-year-old man with well-controlled RA developed tenosynovitis with bone destruction in the right index finger metacarpophalangeal joint following a single intra-articular injection of triamcinolone acetonide. Despite antibiotic treatm…

Longitudinal incision in surgical release of De Quervain disease.

The objective of this paper is to contrast the use of a longitudinal incision in surgical decompression of De Quervain disease with a transverse incision. The advantages are ease in recognition of compartment variations and superficial branches of radial nerve and prevention of palmar tendon subluxation by permitting a more dorsal release of the compartment sheath. Since 2002, we have used a longitudinal skin incision instead of the classic transverse incision to release the first dorsal compartment.

On a form of chronic tendovaginitis by Dr. Fritz de Quervain in la Chaux-de-Fonds. 1895.

This paper represents the first English translation of the original article "On A Form Of Chronic Tendovaginitis" by Fritz de Quervain, published in 1895. This classic paper describes the natural history, symptomatology, pathology, and etiology of stenosing tenovaginitis of the first dorsal compartment (de Quervain's tenosynovitis). Dr. de Quervain also discusses the differential diagnosis for this disorder, his approach to management of the problem, and the results of surgical treatment in this series of collected patients. The paper concludes with a brief discussion of trigger digits. In add…

The athlete's foot: the grey zone behind the ankle.

Posterior ankle and heel pain is common among athletes. The most common injury in this region is that of the Achilles tendon. However, besides the Achilles tendon, pain can originate from a retrocalcaneal bursitis, posterior impingement syndrome, os trigonum syndrome, or flexor hallucis longus tenosynovitis. These pathologies often caused by overuse, but can also occur after trauma. With careful examination, and the help of MRI imaging, the distinction can be made among these pathologies and the more common Achilles tendon problems. Like most overuse injuries, the majority of cases of retrocal…

Dupuytren's disease, carpal tunnel syndrome, trigger finger, and diabetes mellitus.

A comparative prospective study of 120 adult diabetics (60 insulin dependent, 60 non-insulin dependent) and 120 non-diabetic adults as controls showed significantly higher incidence of Dupuytren's disease, limited joint motion, carpal tunnel syndrome, and flexor tenosynovitis in the diabetic population. Of the diabetic patients one third had a mild non-progressive form of Dupuytren's disease, which commonly involved the long and ring rays. Limited joint motion was noted in a third of diabetics, and carpal tunnel syndrome was observed in 15-25%, and flexor tenosynovitis in about a fifth. Limite…

Magnetic resonance imaging of hand involvement in polymyalgia rheumatica: a comparison with seropositive rheumatoid arthritis.

BACKGROUND: Involvement of hands is a frequent finding in polymyalgia rheumatica (PMR) and can lead to misclassification as late-onset rheumatoid arthritis (RA). OBJECTIVE: To compare the inflammatory pattern as shown by contrast-enhanced MRI (ceMRI) of hand involvement in PMR to seropositive RA (RA+). METHODS: MRIs of patients diagnosed with PMR and clinically suspected hand involvement and with RA+, matched for age/sex, were analysed retrospectively. Synovitis, osteitis and erosions of metacarpophalangeal (MCP) joints 2-5 plus tenosynovitis of flexor/extensor compartments were evaluated us…

Improving the feasibility of MRI in clinically suspect arthralgia for prediction of rheumatoid arthritis by omitting scanning of the feet.

OBJECTIVES: The use of MR-imaging is recommended for the early detection of RA. Next to the small joints of the hands, foot-joints are often … P2-5 and wrist) and foot (MTP1-5) joints at baseline. Scans were scored for synovitis, osteitis and tenosynovitis. After ⩾1 year follow-up, the development of clinically apparent inflammatory arthritis (IA) was studied. Cox regression was performed and test characteristics were evaluated. Sensitivity analyses were performed for the outcome RA-development (2010- …

High numbers of people are sti … lyarthritis or chronic polyarthritis resembling rheumatoid arthritis. Leprosy can also present with tenosynovitis, either isolated or associated with arthritis. CASE PRESENTATION: We report a case of 29-year-old man who developed tenosynovitis and acute symmetrical polyarthritis including small joints of hands and feet three weeks after appearance of typical c …

Analysis of "hidden lesions" of the extra-articular biceps after subpectoral biceps tenodesis: the subpectoral portion as the optimal tenodesis site.

BACKGROUND: In biceps tenodesis for intra-articular tears, determining the distal extension of the lesions through the biceps groove is impo … ngth and delamination depth of the biceps tears were examined, and the severity of the accompanying tenosynovitis and degeneration was assessed. RESULTS: Tears invaded zone B in all the cases and extended to zone C in 28 cases (77.8%). Tenosynovitis was observed along the tear in 28 cases (77.8%) and extended to zone C in 26 cases (72.2%). The mean tear length …

The usefulness of patient-reported measures for clinical practice.

BACKGROUND: The authors assessed the diagnostic potential of commonly used patient-reported measures, namely, the Boston Carpal Tunnel Quest … o diagnostic groups: those with Dupuytren disease and those with carpal tunnel, osteoarthritis, and tenosynovitis conditions. Logistic regression analysis was performed, and a receiver operating characteristic curve was used in data analysis.
