Retinal detachment in highly myopic eyes due to macular holes and epiretinal traction.

We operated on four highly myopic eyes with recurrent retinal detachment due to macular holes. A thin layer of epiretinal tissue was present over the posterior retina in each eye, probably causing tangential traction on the retina and accounting for the recurrent detachment. Biopsy of the tissue from one case showed mainly acellular material resembling cortical vitreous. Recognizing this clinical condition may permit successful treatment of certain cases with recurrent retinal detachment in highly myopic eyes with macular holes.

Blunt multiple trauma (ISS 36), femur traction, and the pulmonary failure-septic state.

Fifty-six blunt multiple trauma patients (HTI-ISS 22-57) were studied for the effects of immediate versus delayed internal fixation of a femur or acetabular fracture on the pulmonary failure septic state. The pulmonary failure septic state may be defined as an alveolar arterial oxygen tension difference greater than 100, plus fever and leukocytosis. These patients were divided into four groups. Group I (N = 20) had immediate internal fixation, postoperative ventilatory support, and was sitting up at 30 hours. Group II (N = 20) had 10 days of femur traction and postoperative ventilatory support…

Transsphenoidal chiasmapexy for correction of posthypophysectomy traction syndrome of optic chiasm. Case report.

Postoperative improvement occurred as a result of transsphenoidal chiasmapexy in a patient with posthypophysectomy visual loss. Traction injury of the optic chiasm may have been caused by a deficient diaphragma sellae and inadequate packing and repair of the sella floor. A cartilaginous seal is recommended.

MRI safe lower limb traction techniques: a systematic review and novel economic technique.

OBJECTIVES: To systematically review existing MRI-safe lower limb traction methodology and describe a simple, accessible technique that provides an economically viable alternative to specialized equipment. METHODS: Embase, Emcare, HMIC, Medline, and Ovid Journal databases were queried without time or language limitations (registered protocol INPLASY2025100045). Studies with lower limb traction-enhanced MRI were included, studies without traction methodology were excluded. The novel MRI-safe-traction setup consisted of a pillow "traction-hill" supporting the leg, a rolled-up blanket fulcrum an…

Erdafitinib-associated Subretinal Fluid in a 75-Year-Old Man With Vitreomacular Traction.

Fibroblast growth factor receptor (FGFR) inhibitors are increasingly used in cancer therapy but cause ocular side effects, including subretinal fluid (SRF). Baseline vitreomacular traction (VMT) may predispose patients to drug-induced SRF. A 75-year-old man with bladder cancer and VMT developed SRF after initiating erdafitinib therapy. Baseline and follow-up findings were analyzed, including ophthalmic examinations, optical coherence tomography (OCT), and fluorescein angiography (FA). One month after starting erdafitinib, the patient's vision decreased, and OCT revealed new bilateral SRF. Erda…

Butterfly Traction: A New Standardized Approach for Gallbladder Bed Detachment in Laparoscopic Cholecystectomy.

INTRODUCTION: Gallbladder bed detachment (GBB-D) in laparoscopic cholecystectomy (LC) lacks a standardized technique. We developed butterfly traction (BT) to perform optimal GBB-D. This study evaluates its effectiveness and potential for standardization. METHODS: BT aligns the dissection axis with the surgeon's dominant hand by positioning the gallbladder upright and using a left-sided lateral approach. This improves right-side mobility, enabling horizontal traction and avoiding axis interference. BT has been adopted in all LCs at our institution since July 2023. Among 118 LCs performed from …

Curative Resection of T1a Inlet Patch Adenocarcinoma With Traction-Assisted Endoscopic Submucosal Dissection.

Esophageal inlet patch (EIP) adenocarcinoma is extremely rare. We present a case of a 58-year-old man who underwent a diagnostic esophagogastroduodenoscopy for dysphagia and found to have a 2 cm polypoid mass arising from an EIP. Biopsies and staging were consistent with T1aN0M0 EIP adenocarcinoma. While surgical resection was the main method of treatment of these lesions, very few case reports have shown that endoscopic resection can successfully remove these lesions. After multidisciplinary discussion, the patient underwent curative traction-assisted endoscopic submucosal dissection-which is…

Clinical Case Report: Neonatal Mesenteric Traction Syndrome During Surgery for Congenital Duodenal Stenosis.

Mesenteric traction syndrome (MTS) is a common complication of major abdominal surgery, characterized by flushing, hypotension, and tachycardia. However, its occurrence in neonates has not yet been documented. This report details a neonatal case of MTS that emerged during surgery for congenital duodenal stenosis. The patient was a two-day-old girl, born at 39 weeks and three days of gestation via vaginal delivery, weighing 2708 g. She underwent general anesthesia for the surgery, and continuous IV administration of remifentanil at 0.2 μg/kg/min was commenced minutes before the surgery began. I…

Endoscopic submucosal dissection assisted by adaptive traction: results of the first 54 procedures.

BACKGROUND : Good submucosal exposure is key to successful endoscopic submucosal dissection (ESD) and can be achieved with various traction devices. Nevertheless, these devices have a fixed traction force that tends to decrease as the dissection progresses. In contrast, the ATRACT adaptive traction device increases traction during the procedure. METHODS : In this retrospective analysis of prospectively collected data (from a French database), we analyzed ESD procedures performed with the ATRACT device between April 2022 and October 2022. The device was used consecutively whenever possible. We …

Visualization of preretinal membranes using trypan blue in patients with traction retinal detachments.

OBJECTIVE: Surgical repair of complex tractional retinal detachments (TRDs) can be challenging due to the difficulty in delineating the fibroglial membranes from the underlying retinal surface. We describe the visual and retinal reattachment rate with the intraoperative use of trypan blue (TB) to visualize proliferative fibrous membranes in patients with TRDs. DESIGN: Retrospective study. PARTICIPANTS: Seventeen patients who underwent a pars plana vitrectomy for surgical TRD repair with TB use between January 1, 2005, and December 31, 2020. METHODS: Patient charts were retrospectively evalu…

[Traction of impacted teeth using bone anchorage and cantilever springs: a case report].

Egression and orthodontic traction can lead to parasitic movements on the anchoring elements. It is often necessary to use a device including several dental elements sometimes associated with traction auxiliaries. The author, through clinical cases, will develop his therapeutic approach, to limit these parasitic movements as much as possible. The use of a bone anchorage associated with a cantilever arch improves the management of this frequent orthodontic problem.
