Combat health care providers and resiliency: adaptive coping mechanisms during and after deployment.

The purpose of this investigation was to understand the varied health care provider responses to traumas by identifying perceptions of control and self-efficacy, appraisal styles, and postevent coping strategies in active duty military nurses and physicians deployed to combat/terrorist regions. Twenty purposively sampled military health care providers completed a descriptive questionnaire, the Posttraumatic Stress Disorder Checklist, the General Self-Efficacy Scale, and a recorded semistructured interview that was later transcribed and content analyzed. Cognitive-behavioral determinants of hea

Evaluation of second victim experiences and support needs among physicians and nurses.

BACKGROUND: Patient safety aims to prevent medical errors and improve care quality; however, adverse events may also negatively affect the healthcare professionals involved. This phenomenon, known as second victimhood, is associated with psychological and professional consequences such as guilt, burnout, and trauma symptoms. Evidence from Türkiye remains limited. This study aimed to assess second victimhood experiences among physicians and nurses, examine associated impacts, and identify support needs. METHODS: This cross-sectional study was conducted in October 2023 among physicians and nurs

Epidemiological and antimicrobial resistance profile of catheter-associated and non-catheter urinary tract infections in trauma patients (2017-2024).

BACKGROUND: Catheter-associated urinary tract infections (CAUTIs) are a major healthcare-associated infection (HAI) in trauma care settings, contributing to morbidity, mortality and antimicrobial resistance. In this study we characterize the epidemiology, microbiological profile, antimicrobial resistance patterns, and clinical outcomes of CAUTIs and non-CAUTI urinary tract infections (UTIs) at a Level 1 Trauma Centre in India from 2017 to 2024, using a modified CDC-NHSN definition and digital surveillance. METHODS: A retrospective analysis of 723 UTI events was conducted using Centers for Dis

Reported effects of the COVID-19 pandemic on the psychological status of emergency healthcare workers: A scoping review.

BACKGROUND: While literature on psychological consequences among frontline healthcare workers (HCWs) flourishes, understanding the psychological burden on this group is particularly crucial, as their exposure to COVID-19 makes them especially at high risk. We explored what is known about psychological effects of the COVID-19 pandemic on emergency HCWs. METHODS: We used a scoping review following the Preferred Reporting Items for Systematic Reviews and Meta-Analyses extension for Scoping Reviews. RESULTS: The search identified 5432 articles, from which a total of 21 were included in the final

Analysis of Mild Traumatic Brain Injury Outpatient Follow-Up.

BACKGROUND: Mild traumatic brain injury (mTBI) remains a prevalent public health concern. Implementation of an mTBI guideline encouraged screening all patients at risk for mTBI, followed by outpatient follow-up in a "concussion clinic." This resulted in an increase in inpatient concussion evaluations, followed by high-volume referral to the concussion clinic. This prompted the routine use of an outpatient mTBI symptom screening tool. The purpose of this quality improvement study was to analyze the characteristics of an mTBI population at outpatient follow-up and describe the clinicians' care r

Using object relations theory to understand childhood deprivation.

In clinical practice, it has been noted that many patients referred for eating disorders, self-abuse, and other self-destructive behavior patterns had a common history of deprivation in childhood. Little research or conceptual literature has been devoted to the study or clarification of the phenomenon of emotional deprivation. Current literature focuses on the more obvious, measurable factors of abuse and neglect. This article defines deprivation from an object relations perspective, demonstrates its overlap with abuse, and suggests that deprivation, alone or in conjunction with other trauma a

Reducing Blood Culture Contamination in the Emergency Department Using PIVO: A Quality Improvement Project.

BACKGROUND: Blood culture contamination contributes to diagnostic delays, unnecessary antimicrobial therapy, increased length of stay, and excess health care costs. The emergency department in which this quality improvement project was implemented maintained a baseline contamination rate of 10.5%, exceeding the recommended benchmark of ≤3%. High-acuity emergency department workflows, characterized by rapid diagnostics, frequent intravenous manipulation, and workflow variability, further increase contamination risk. The PIVO device draws blood through a peripheral intravenous catheter by advanc

Globe Rupture - A Case Report and Review of Emergency Department Diagnosis and Management.

Globe ruptures, while uncommon, must be promptly recognized to optimize the possibility of preserving vision. Differentiating open globe injuries from corneal abrasions, hyphema, and other ocular injuries is critical. When a globe rupture is identified, prompt ophthalmology consultation, placement of a rigid eye shield, administration of prophylactic systemic antibiotics (and antifungals with an appropriate history), and a tetanus vaccine update (if needed) are key to a successful outcome, so long as the visual prognosis is not dismal. We describe a case of ocular trauma presenting as a blood

Case Report: A multidisciplinary collaborative case of complex rectovaginal fistula resulting from childhood sexual trauma.

Rectovaginal fistula (RVF) is a severe gynecological complication that can arise from obstetric trauma, surgical injury, infection, or physical trauma. RVF resulting from childhood sexual assault accompanied by extensive perineal injury is uncommon in clinical practice, and its management is particularly complex, as it requires not only meticulous anatomical reconstruction but also sustained attention to long-term psychological sequelae. This report presents the case of a patient with RVF secondary to childhood sexual assault. The patient underwent two unsuccessful repair attempts and subseque

Evaluation and management of pain in patients in the emergency trauma room: a best practice implementation project.

INTRODUCTION: Pain is a frequent complaint in emergency departments (ED), particularly among polytrauma patients, who often experience high levels of pain. The Advanced Trauma Life Support protocol is widely used to assess such patients, prioritizing life-threatening conditions through the ABCDE approach. However, pain documentation is often insufficient, and analgesia administration is frequently delayed, even in conscious patients able to report their pain. OBJECTIVES: This project aimed to promote evidence-based practices for pain assessment and management in an emergency trauma room. MET

Adolescents' Perceptions of Screening, Brief Intervention, and Referral to Treatment Service at Pediatric Trauma Centers.

OBJECTIVE: Screening, brief intervention, and referral to treatment (SBIRT) for adolescent alcohol and drug (AOD) use is recommended to occur with adolescents admitted to pediatric trauma centers. Most metrics on SBIRT service delivery only reference medical record documentation. In this analysis we examined changes in adolescents' perception of SBIRT services and concordance of adolescent-report and medical record data, among a sample of adolescents admitted before and after institutional SBIRT implementation. METHODS: We implemented SBIRT for adolescent AOD use using the Science to Service

BACKGROUND: Orthogeriatric comanagement of older patients with hip fractures has been proven to provide significant benefits concerning functional status, readmissions, nursing home placement, in-hospital complications and mortality. Medication management in older individuals is a cornerstone in orthogeriatric comanagement. The aim of the study was to analyze the extent of overprescription and undertreatment in older trauma patients. METHODS: Personal and medical data of consecutively admitted older trauma patients were analyzed. Evaluation of medication was conducted according to the Fit fOR

Post-Traumatic Intracranial Pseudoaneurysm Presenting as Epistaxis.

BACKGROUND: Epistaxis is a frequent presenting complaint in the Emergency Department (ED). Roughly 60% of the population will suffer from epistaxis in their lifetime. The most common causes of epistaxis include nose picking, facial trauma, foreign bodies, and coagulopathies. There are other causes that are much less common, such as intracranial pseudoaneurysms. There are multiple causes that precipitate intracranial pseudoaneurysm formation, with head trauma accounting for less than 1% of inciting events. CASE REPORT: A 24-year-old female with history of traumatic brain injury with associated

The arts of war and medicine: a study in symbiosis.

Practitioners of warfare and medicine have both considered their respective disciplines part art and part science. Each has seen dramatic advances over the past several centuries, and the histories of progress in warfare and medical practice are inextricably intertwined. For example, the congregation of large armies inspired the development of epidemiologic analysis and preventive medicine, which forestalled the disastrous loss of life that typified all wars through the U.S. Civil War. Warfare also helped to clarify the critical distinction between medicine and health in several ways. Battles

I can't believe you don't have a box for this: 'other' events and posttraumatic stress symptoms among women.

Background: Traumatic exposures are often assessed in clinical and research settings by a list of events chosen because they meet the definition of trauma required for the ICD-11 or DSM-5 diagnosis of posttraumatic stress disorder (PTSD). Some trauma measures include an 'other' option, with a text box enabling participants to describe an event not listed. Objective: There were three aims in this study: (1) to identify commonly endorsed 'other' events not listed on a standard measure of trauma exposure, (2) to identify thematic commonalities among these other events, and (3) to assess the preval

[Primary management of polytrauma. Comparison of a German and American air rescue unit].

Hospital-based helicopter services from German and American university-affiliated trauma centers were reviewed. All multitrauma patients transported via helicopter from the scene of the accident to the trauma center during a 1-year period were included. The patients were comparable regarding mechanism of injury, age, flight times, mean ISS, ISS distribution, and number of severe injuries per body region (patients with AIS > 3 for head, thorax and abdomen). Overall mortality for the German system was 21/221 (9.5%) and 21/186 (11.3%) for the American system (not significant). Survivor-based TRIS

The Effect of 2019 Coronavirus Stay-at-Home Order on Geriatric Trauma Patients in Southern California.

BACKGROUND: California issued stay-at-home (SAH) orders to mitigate COVID-19 spread. Previous studies demonstrated a shift in mechanisms of injuries (MOIs) and decreased length of stay (LOS) for the general trauma population after SAH orders. This study aimed to evaluate the effects of SAH orders on geriatric trauma patients (GTPs), hypothesizing decreased motor vehicle collisions (MVCs) and LOS. METHODS: A post-hoc analysis of GTPs (≥65 years old) from 11 level-I/II trauma centers was performed, stratifying patients into 3 groups: before SAH (1/1/2020-3/18/2020) (PRE), after SAH (3/19/2020-6
