A modified BPaL regimen for tuberculosis treatment replaces linezolid with inhaled spectinamides.

The Nix-TB clinical trial evaluated a new 6 month regimen containing three oral drugs; bedaquiline (B), pretomanid (Pa), and linezolid (L) (BPaL regimen) for the treatment of tuberculosis (TB). This regimen achieved remarkable results as almost 90% of the multidrug-resistant or extensively drug-resistant TB participants were cured but many patients also developed severe adverse events (AEs). The AEs were associated with the long-term administration of the protein synthesis inhibitor linezolid. Spectinamide 1599 is also a protein synthesis inhibitor of Mycobacterium tuberculosis with an excelle

Neuro-Behçet's Disease Onset in the Context of Tuberculous Meningoencephalitis: A Case Report.

Behçet's disease (BD) is a systemic vasculitis that frequently presents with a relapsing-remitting pattern. CNS involvement (Neuro-Behçet) is rare, affecting approximately 10% of patients. Its etiological mechanisms are not yet fully understood. The most commonly accepted hypothesis is that of a systemic inflammatory reaction triggered by an infectious agent or by an autoantigen, such as heat shock protein, in genetically predisposed individuals. Mycobacterium tuberculosis is known to be closely interconnected with BD, both affecting cell-mediated immunity to a certain extent and probably shar

Pharmacokinetics of oral and intravenous rifampicin during chronic administration.

We investigated the pharmacokinetics of rifampicin and its major metabolites, 25-desacetylrifampicin and 3-formylrifampicin, in two groups of six patients with active pulmonary tuberculosis, who received either multiple oral or intravenous rifampicin therapy in combination with intravenous isoniazid and ethambutol. Serum concentrations of rifampicin were each determined after a single oral and intravenous test dose of 600 mg rifampicin at the beginning and after 1 and 3 weeks of tuberculostatic treatment. Analysis of rifampicin and its metabolites was performed by high-pressure liquid chromato

[Infections of the oral mucosa II. Bacterial, mycotic and viral infections].

Non-specific infections of the oral mucosa are rare; however, they may present during HIV infection in the form of gingivo-periodontal lesions. In some of these Candida albicans may play a role in the pathogenesis. Sexually transmitted bacterial infections such as gonorrhoea and syphilis are frequently associated with HIV infection. Since penicillin resistance is frequent in gonorrhoea, the cephalosporines are mainly used for treatment. Syphilis increases the risk for transmission of HIV. Lues maligna with oral manifestations has been described. For this, penicillin G is the therapy of choice.

PDE4 inhibition in pediatric psoriasis: analysis of the SPROUT study on apremilast.

INTRODUCTION: Moderate-to-severe plaque psoriasis in the pediatric population represents a complex therapeutic challenge. Although the advent of biologic drugs (anti-TNF, anti-IL17) has revolutionized efficacy standards, there remains an unmet need for oral, safe therapeutic options that do not require regular monitoring. Apremilast, an oral phosphodiesterase 4 (PDE4) inhibitor, has recently been evaluated in this population. AREAS COVERED: This review analyzes the 16-week and 52-week results of the phase 3 SPROUT trials which evaluated the efficacy and safety of apremilast in children and ad

Inherited IL-12Rβ1 Deficiency in a Child With BCG Adenitis and Oral Candidiasis: A Case Report.

Tuberculosis is a major worldwide problem, and protection from it is achieved mainly by live attenuated bacille Calmette-Guérin vaccine, which is capable of causing disease in immunocompromised host. Oral thrush is abnormal in healthy children, which suggests an underlying immunodeficiency. Mendelian susceptibility to mycobacterial disease is a rare primary immunodeficiency characterized by a selective predisposition to weakly virulent Mycobacteria and Salmonella and also predisposition to chronic mucocutaneous candidiasis. Interleukin 12 receptor β1 (IL-12Rβ1) deficiency is the most common di

Delpazolid in combination with bedaquiline, delamanid, and moxifloxacin for pulmonary tuberculosis (PanACEA-DECODE-01): a prospective, randomised, open-label, phase 2b, dose-finding trial.

BACKGROUND: Linezolid plays a crucial role in the first-line treatment of drug-resistant tuberculosis globally. Its prolonged use can lead to neurological and haematological toxicity, highlighting the need for safer oxazolidinones. Delpazolid, a novel oxazolidinone, might be safer. We aimed to evaluate the safety and efficacy of delpazolid and identify an optimal dose. METHODS: PanACEA-DECODE-01 was a prospective, randomised, open-label, phase 2b, multicentre, dose-finding trial done in five tuberculosis trial sites in Tanzania and South Africa. Adults aged 18-65 years, who weighed 40-90 kg,

Antiglomerular Basement Membrane (Anti-GBM) Glomerulonephritis in the Context of Genitourinary Tuberculosis: A Case Report.

Antiglomerular basement membrane (anti-GBM) glomerulonephritis (GN) has been reported to be associated with tuberculosis (TB), but anti-GBM GN in the context of genitourinary TB is exceptionally rare. We report a case of anti-GBM GN with concurrent genitourinary TB. The patient underwent plasma exchange and was treated with high-dose prednisolone following a 3-day course of intravenous methylprednisolone. Oral cyclophosphamide (125 mg once daily) was also prescribed. After 14 cycles of plasma exchange, the anti-GBM antibody titer decreased. However, the patient developed pulmonary hemorrhage r

Meningeal sporotrichosis in an immunocompetent host: A case report.

A 78-year-old woman presented to hospital with altered mental status. Hyponatremia was diagnosed and treated. She maintained mental confusion despite normal sodium. No headache, fever or focal signs were present. CSF analysis showed chronic meningitis; MRI demonstrated basilar enhancement. She was empirically treated for tuberculosis, with no improvement. PCR for Sporothrix in the CSF was positive. After treatment with Amphotericin followed by oral itraconazole, she completely recovered her cognitive abilities. Follow-up CSF was normal. This report illustrates the need to consider sporotrichos

[New and rediscovered treatments for tuberculosis].

Tuberculosis (TB) is still a major challenge for global health, but in recent years there have been several important developments in treatment options. Shorter treatment regimens for latent TB, based on conventional drugs, offer better adherence and fewer side effects, while new and repurposed antibiotics have opened the door to more effective, shorter and less side effect-heavy treatments for drug susceptible TB and multidrug-resistant/extensively drug resistant TB as well as atypical mycobacterioses. This review investigates how these shorter and predominantly oral treatments, in the future

[Injection therapy of the prostate in prostatic tuberculosis].

Oral therapy of prostatic tuberculosis so far has only been able to achieve conversion in one-fifth of the cases. Since 1970 we have practiced the transcutaneous injection of the prostate and have achieved conversion in all cases within a short time. Injection of 1 g Streptomycin and 0.1 g INH twice a week over a period of four to five weeks was carried out in 90 patients. Complications contraindicating the treatment was not seen. The transcutaneous injection treatment of the prostate is indicated in all patients where cavernous changes in the prostate have been demonstrated by urethrography.

Risk factors for pulmonary tuberculosis in patients with chronic obstructive airway disease in Taiwan: a nationwide cohort study.

BACKGROUND: An association between chronic obstructive pulmonary disease (COPD) and tuberculosis (TB) has been described, mainly due to smoking and corticosteroid use. Whether inhaled corticosteroid (ICS) therapy is associated with an increased risk of TB remains unclear. METHODS: We selected COPD cases by using six diagnostic scenarios and control subjects from a nationwide health insurance database, and applied time-dependent Cox regression analysis to identify the risk factors for TB. RESULTS: Among 1,000,000 beneficiaries, 23,594 COPD cases and 47,188 non-COPD control subjects were selec

Molecular Mechanisms of Resistance and Treatment Efficacy of Delamanid Against Mycobacterium tuberculosis: A Systematic Review.

BACKGROUND AND AIMS: Tuberculosis (TB) remains a major global health problem, and treatment progress is increasingly threatened by rising multidrug-resistant tuberculosis (MDR-TB). Delamanid (DLM), a nitroimidazole drug, has shown good efficacy and safety against both drug-susceptible and drug-resistant Mycobacterium tuberculosis (Mtb) strains. However, data on its resistance mechanisms, drug susceptibility testing (DST), clinical effectiveness, safety, and pharmacokinetics remain limited. This review aims to summarize the most recent molecular, structural, and clinical evidence related to DLM
