Brucella Pneumonia Mimicking Pulmonary Tuberculosis: A Case Report.

Brucellosis is a zoonotic infection prevalent in certain regions. It typically presents with nonspecific symptoms but can also cause focal organ involvement, including the musculoskeletal, neurological, or pulmonary systems. Herein, we present a case of a 31-year-old male patient who presented with a 5-day history of fever and cough. Chest X-ray revealed bilateral patchy consolidations, initially suggesting an infectious etiology such as tuberculosis. Given the patient's background, further investigation confirmed brucellosis-related pneumonia, an uncommon pulmonary manifestation of brucellosi

Benchtop NMR urine metabolomics for diagnosing pulmonary tuberculosis.

Tuberculosis (TB) remains a major global health issue, especially in resource-limited settings, where rapid and accurate diagnosis is critical yet often lacking. Nuclear magnetic resonance (NMR)-based metabolomics has emerged as a promising tool for identifying disease-specific biomarkers. This study assessed the diagnostic potential of urinary metabolomics using high-resolution (HR-)NMR and benchtop NMR (bNMR) in Nigerian adults with presumed pulmonary TB, including individuals with and without HIV. Urine samples were analysed by HR-NMR and bNMR. Multivariate analysis was used to develop clas

Risk-stratified treatment for drug-susceptible pulmonary tuberculosis.

The Phase 3 randomized controlled trial, TBTC Study 31/ACTG A5349 (NCT02410772) demonstrated that a 4-month rifapentine-moxifloxacin regimen for drug-susceptible pulmonary tuberculosis was safe and effective. The primary efficacy outcome was 12-month tuberculosis disease free survival, while the primary safety outcome was the proportion of grade 3 or higher adverse events during the treatment period. We conducted an analysis of demographic, clinical, microbiologic, radiographic, and pharmacokinetic data and identified risk factors for unfavorable outcomes and adverse events. Among participants

Early bactericidal activity of sitafloxacin against pulmonary tuberculosis.

Sitafloxacin is a quinolone broad-spectrum antimicrobial agent, and its pharmacologic properties and in vitro data demonstrate that sitafloxacin has a potent killing effect against Mycobacterium tuberculosis, including drug-resistant strains, which is superior to that of other available quinolones. However, its efficacy in patients with primary-sensitive tuberculosis is unclear. This study aims to evaluate the early bactericidal activity (EBA) of sitafloxacin in patients with primary drug-susceptible tuberculosis. In this early bactericidal activity study, 30 patients with primary smear-positi

COVID-19 pneumonia with pulmonary tuberculosis: Double trouble.

A 75-year-old male presented with acute febrile respiratory illness with hypoxia and anorexia of longer duration; computed tomography (CT) of the thorax was suggestive of cavitary lung disease, with sputum smear positive for acid-fast bacilli and also having classical COVID-19 pneumonia patterns in the CT thorax; and COVID-19 rapid antigen test was positive. He was treated for COVID-19 pneumonia and antituberculosis treatment was initiated at the discharge. He was recovered of both conditions, and we have documented the crucial role of chest CT in managing this case in this pandemic period.

Mycobacterial lineages causing pulmonary and extrapulmonary tuberculosis, Ethiopia.

Molecular typing of 964 specimens from patients in Ethiopia with lymph node or pulmonary tuberculosis showed a similar distribution of Mycobacterium tuberculosis strains between the 2 disease manifestations and a minimal role for M. bovis. We report a novel phylogenetic lineage of M. tuberculosis strongly associated with the Horn of Africa.

Pulmonary tuberculosis associated retinal vasculitis presenting as xanthopsia.

PURPOSE: To report a patient presenting with unilateral xanthopsia who was diagnosed as having retinal vasculitis with pulmonary tuberculosis. DESIGN: A case report. METHODS: A 20-year-old man presented with xanthopsia in the right eye. Fundus examination revealed multiple retinal hemorrhages, vascular sheathing, and a yellowish retina. RESULTS: He was diagnosed as having retinal phlebitis with pulmonary tuberculosis by chest x-ray and computerized tomography scans and treated with anti-tuberculosis medications. After treatment, he described clearance of xanthopsia, and fundus examination a

T-SPOT.TB responses during treatment of pulmonary tuberculosis.

BACKGROUND: Immune responses to Mycobacterium tuberculosis antigens could serve as surrogate markers of treatment response. METHODS: Using the T-SPOT.TB assay and frozen peripheral blood mononuclear cells, we enumerated ESAT-6- and CFP-10-specific IFN-gamma-producing T cells over time in pulmonary TB patients receiving directly observed treatment. T cell responses (measured as "spot forming cells" or "SFCs") were assessed prior to treatment and at 16 and 24 weeks of treatment. RESULTS: 58 patients were evaluated, of whom 57 were HIV seronegative. Mean (SD) ESAT-6, CFP-10, and summed RD1 spec

Pulmonary necrosis resulting from DNA vaccination against tuberculosis.

The use of DNA constructs encoding mycobacterial proteins is a promising new approach to vaccination against tuberculosis. A DNA vaccine encoding the hsp60 molecule of Mycobacterium leprae has previously been shown to protect against intravenous infection of mice with Mycobacterium tuberculosis in both the prophylactic and immunotherapeutic modes. It is shown here, however, that this vaccine was not effective in a more realistic aerosol infection model or in a model of latent tuberculosis in the lungs. Moreover, when given in an immunotherapeutic model the immunized mice developed classical Ko

Increased cortisol: cortisone ratio in acute pulmonary tuberculosis.

To evaluate a possible role for altered cortisol metabolism in mediating the immunoparesis associated with progressive tuberculosis (TB), we have studied the hypothalamic-pituitary-adrenal axis, and the activities of the 11beta-hydroxysteroid dehydrogenases (11-HSDs) that interconvert active cortisol and inactive cortisone. In active pulmonary tuberculosis (PTB), the ratio of cortisol/cortisone metabolites in 24-h urine showed a shift towards active cortisol (ratio, 1.19 +/- 0.1, n = 16 versus 0. 89 +/- 0.05 in cured pulmonary tuberculosis (CTB), n = 13, p < 0. 01; and 0.78 +/- 0.04 healthy vo

Cavitating pulmonary tuberculosis below age of 2 years.

A prospective study was conducted on 75 consecutive cases of primary cavitary pulmonary tuberculosis in hospitalized children below 2 years to determine the spectrum of the disease. Diagnosis was based on clinical, radiological, tuberculin test and histopathological findings and not on isolation of tubercle bacilli. Peak age was 7-12 months. Ten mothers suffered from pulmonary fibrocavitary disease and 73% of cases were severely malnourished. Presenting symptoms were fever and cough, at times dyspnea and often followed measles. Unlike adults, hemoptysis was seldom encountered. Cavitary lesions

The adult respiratory distress syndrome bronchogenic pulmonary tuberculosis.

In three cases of pulmonary tuberculosis associated with the adult respiratory distress syndrome the clinical features, which were similar to those of patients with miliary tuberculosis and adult respiratory distress syndrome, included a history of cough, fever, and dyspnoea on effort, and the physical signs of fever, tachypnoea, pulmonary adventitious sounds, tachycardia, and hepatomegaly. In these cases the radiological features, though suggestive of diffuse pulmonary oedema, were more prominent on the side in which the cavitatory lesion appeared. The diagnosis of tuberculosis was made easil
