[Light protection and increase of UV tolerance].

Topical sun-ray filter creams protect the skin from sunburn and from long-term damages caused by UV-radiation. The choice of the sun protecting factor depends on the radiation dose as well as on the individual sensitivity to UV-radiation. For the prevention of photodermatoses, the industry offers broad spectrum absorbents. For internal prophylaxis, oral ingestion of a new preparation containing 25 mg beta-carotene and 35 mg canthaxanthin per dragee, has proved to restore the tolerance of sunlight with polymorphic light eruption, light urticaria, protoporphyria, and other photodermatoses to a c

Ultraviolet A phototherapy in the prophylaxis of solar urticaria.

Five patients with solar urticaria were treated with ultraviolet A (UVA) radiation. Three experienced marked increases in sunlight tolerance, but two patients failed to improve. Subjective clinical improvement was documented objectively by marked increases in the minimal urtication doses obtained by provocative light testing. These results indicate that some patients with solar urticaria may respond to treatment with UVA radiation alone.

Methotrexate and ultraviolet radiation.

Reactivation of a sunburn has been reported after the administration of methotrexate for cancer chemotherapy. A similar reaction is described in a patient with psoriasis who was receiving both coal tar with ultraviolet radiation for cutaneous lesions and methotrexate for arthritis. Knowledge of this poorly understood side effect of methotrexate is particularly important for physicians administering phototherapy and, perhaps, photochemotherapy.

PUVA-treatment for solar urticaria and persistent light reaction.

A technique is described to successfully treat patients with extreme sensitivity to UV electromagnetic wave lengths with photochemotherapy. Representative data from a patient with solar urticaria and two patients with persistent light reaction are given. Prior to treatment the threshold doses for UV-C, UV-B, and UV-A were determined. Photochemotherapy was performed with standard 8-methoxypsoralen-UV-A (PUVA) schedules. Initial treatments were very difficult because of the extreme UV-sensitivity in all patients. Once pigmentation was induced, PUVA-treatments were easy to perform. All patients b

A rational approach to the treatment of vitiligo and other hypomelanoses.

A rational approach to hypopigmented disorders needs a clear and widely accepted classification and meta-analysis of the literature to position the medical, physical, surgical, and combined treatments available in terms of safety and effectiveness profiles. Best available scientific evidence must be rationally adapted to the patient's expectations and to the clinical presentation of the hypopigmentary disease, to reach the most valid final results.

Novel Combination of UVA-1 Phototherapy With Topical Tofacitinib for Treating Eosinophilic Fasciitis With Morphea: Case Report.

Eosinophilic fasciitis (EF) and morphea are rare sclerosing connective tissue disorders, and there remains debate about whether these entities are pathologically distinct or related. This case report describes the successful treatment of an adolescent patient who developed progressive morphea plaques while being treated with systemic corticosteroids, intravenous immunoglobulin, and methotrexate for EF. A novel combination of topical tofacitinib, a Janus kinase (JAK) inhibitor, and UVA-1 phototherapy induced clinical remission and was well tolerated, warranting further investigation into such c

Narrowband UVB Phototherapy in Dermatology: GEF-CILAD 2026 Update.

This document updates the scientific evidence and clinical practice regarding narrowband UVB phototherapy (NB-UVB) in dermatology. The review addresses indications, therapeutic regimens, safety aspects, combinations with systemic and biologic agents, and adaptation to special populations. NB-UVB remains a first-line therapy for psoriasis, vitiligo, atopic dermatitis, early mycosis fungoides, and photodermatoses due to its efficacy, safety, and cost-effectiveness. Emerging evidence supports its integration with next-generation systemic therapies. This update provides recommendations for its app

Comparison between 308-nm excimer light alone versus 308-nm excimer light and platelet-rich plasma in the treatment for localized vitiligo.

BACKGROUND: Monochromatic excimer light (MEL) is a safe and effective treatment for localized stable vitiligo. Previous reports of the combination of platelet-rich plasma (PRP) with narrowband ultraviolet B (NB-UVB) or excimer laser yielded conflicting results. AIMS: This prospective, self-controlled, randomized, comparative study aimed to assess whether the addition of PRP to MEL therapy would be of an additive benefit in the treatment for localized stable vitiligo. Patients/Methods The current study included 36 patients with at least 2 more or less symmetrical patches of localized stable vi

An economic evaluation of the randomized controlled trial of topical corticosteroid and home-based narrowband ultraviolet B for active and limited vitiligo (the HI-Light Vitiligo Trial).

BACKGROUND: Economic evidence for vitiligo treatments is absent. OBJECTIVES: To determine the cost-effectiveness of (i) handheld narrowband ultraviolet B (NB-UVB) and (ii) a combination of topical corticosteroid (TCS) and NB-UVB compared with TCS alone for localized vitiligo. METHODS: Cost-effectiveness analysis alongside a pragmatic, three-arm, placebo-controlled randomized controlled trial with 9 months' treatment. In total 517 adults and children (aged ≥ 5 years) with active vitiligo affecting < 10% of skin were recruited from secondary care and the community and were randomized 1: 1: 1 t

Effectiveness of a 308-nm excimer laser in treatment of vitiligo: a review.

Vitiligo is a relatively common acquired disorder, characterized by progressive loss of melanocytes from the epidermis and the epidermal appendages. The disease is associated with considerable morbidity because of a major impact on the quality of life. The treatment for vitiligo is generally unsatisfactory and challenging. There are a variety of therapeutic possibilities including topical corticosteroids, topical calcineurin inhibitors, as well as phototherapy with Psoralen plus UVA (PUVA), narrow-band UVB, and a 308-nm excimer laser and/or lamps. Furthermore, surgical methods encompass grafti

Phototherapy in pediatric patients: choosing the appropriate treatment option.

Phototherapeutic modalities, including narrowband-UVB, broadband-UVB, PUVA photochemotherapy, and excimer laser therapy are valuable tools that can be used for photoresponsive dermatoses in children. As a systematically safer alternative compared with internal agents, including the prebiologic and biological therapies, phototherapy should be considered a possible treatment option for children with diseases including psoriasis, atopic dermatitis, pityriasis lichenoides chronica, and vitiligo.

Influence of PUVA and UVB radiation on expression of ICAM-1 and VCAM-1 molecules in psoriasis vulgaris.

The expression of adhesion molecules Intercellular adhesion molecule-1 (ICAM-1) and Vascular cell adhesion molecule-1 (VCAM-1) is increased in lesional and in non-lesional skin of psoriatic patients, and play role in pathogenesis of the disease. PUVA and UVB therapy are important treatments of psoriasis vulgaris. It has been demonstrated that UVA and UVB therapies reduce expression of these molecules. In this investigation, phototherapy was used to treat psoriatic patients. The expression of these molecules was examined by immunohistochemical method in lesional and non-lesional skin of 10 pati
