The quality of gynecologic cytology has been questioned in the last two years. This author's hospital laboratory has a sizable outpatient gynecologic cytology and biopsy practice from which data have been obtained in several different quality assurance projects. In this article the author analyzes those data with respect to detection of false negative cytology screening errors, specimen sampling errors, precision in cytology and biopsy interpretation, and productivity of quality assurance methods. The perception of visual information in cytoscreening was studied: eye movements were recorded while the cytotechnologist was screening cervical smears by means of a projection screen. Four phases of eye movement could be distinguished: small, aimless movements during the stage movement; a latency period with a duration of about 180 milliseconds; saccadic movement to the position of an object; and fixation on an object. The efficacy of the MiMark endometrial cell sampler in the cytological assessment of endometrium was evaluated in 101 patients with postmenopausal bleeding. The apparatus was successful in daignosing all but one of the endometrial carcinomas in the series. Reports of its accuracy in diagnosing endometrial hyperplasia and other endometrial pathology cannot be substantiated.

Sixteen cytologic endocervical atypias are reported with corroborative tissue analysis. Maternal diethylstilbestrol (DES) history was positive in three patients, one with clear cell adenocarcinoma, one with colposcopy and biopsy proven benign vaginal adenosis, and one with squamous carcinoma in situ of the cervix. A study of 350 newborn girls revealed vaginal (uterine) bleeding in 25,4%, mostly demonstrable only by chemical micro-methods, but also macroscopically visible in 3,3%.
