Vagotomy and pyloroplasty with excision of the ulcer has been performed in a series of 90 patients with benign gastric ulcers. Long term follow-up has shown only 2 patients with recurrent, ulceration . Fifty-eight patients with more than 2 years' follow-up attended for personal interview, of whom 96 per cent had a satisfactory result.

In a retrospective study the clinical and secretory results of 118 patients operated upon with selective proximal vagotomy (SPV) with or without pyloroplasty for duodenal ulcer were examined. The results of surgery in the two groups, with pyloroplasty or without pyloroplasty, were compared. The recurrence rate was higher, although statistically not significant, for patients operated upon without pyloroplasty (19.2%) than for patients operated upon with pyloroplasty (10.6%). Fifty-six patients with benign gastric ulcer of the body of the stomach have been entered into a randomized trial of highly selective vagotomy with excision of the ulcer (HSVE) (26 cases) against standard Billroth I partial gastrectomy (BI) (30 cases). In peripheral and portal venous blood, the immunoreactive insulin (IRI) and glucose levels in response to orally and intravenously administered glucose were measured in 14 patients with selective gastric vagotomy and pyloroplasty (SGV+P) and in 17 control subjects with other abdominal surgery. Forty-five patients with benign gastric ulcers were treated by truncal vagotomy and pyloroplasty or Billroth I gastric resection in a randomized, controlled trial. Postoperatively, one Billroth I patient died. During the follow-up recurrence occurred in three patients treated by vagotomy, but in no Billroth I patient. The overall functional results according to the classification of Visick were slightly, but unsignificantly better after Billroth I than after vagotomy and pyloroplasty.

The result of a prospective trial of 40 patients with duodenal ulcer treated either by proximal gastric vagotomy (antrum innervated) or by selective vagotomy (antrum denervated) shows that they are equally effective in reducing gastric acid and pepsin secretion. Preservation of antral innervation by proximal gastric vagotomy reduces gastric secretion as effectively as denervation of the entire stomach.
