We describe clonal shifts in vanA Enterococcus faecium isolates from clinical samples obtained from patients in Denmark from 2015 to the first quarter (Q1) of 2019. During Q1 2019, the vancomycin-variable enterococci (VVE) ST1421-CT1134 vanA E. faecium became the most dominant vanA E. faecium clone and has spread to all five regions in Denmark. Among 174 E. faecium isolates with vanA, vanB or vanA/vanB genes in Q1 2019, 44% belonged to this type.

BACKGROUND: Data regarding the effectiveness of vancomycin-resistant Enterococci (VRE)-active prophylaxis for preventing early post-liver transplantation (LT) VRE infections in VRE-colonized patients are scarce.
METHODS: 131 pre-LT VRE-colonized patients who underwent LT were enrolled in a retrospective, observational, multicenter study. The incidence of early-onset VRE infections was compared between patients who received active prophylaxis for VRE and those who did not.
RESULTS: Sixty-nine (52.7%) and 62 (47.3%) patients were enrolled in the VRE-active and non-VRE-active prophylaxis group.

UNLABELLED: Vancomycin-resistant Enterococcus faecium (VREfm) is a major cause of invasive healthcare-associated infections. Daptomycin is an important treatment option, but its efficacy may be compromised for isolates with minimum inhibitory concentrations (MICs) of 4-8 mg/L, even at high doses. EUCAST has therefore assigned an "insufficient evidence" category for Enterococcus spp., making accurate MIC determination essential. We compared the performance of UMIC Daptomycin, several automated broth microdilution (BMD) systems, and E-test against the reference BMD method. Eighty-nine VREfm isol

Multidrug-resistant (MDR) nosocomial pathogens represent a critical global health challenge, necessitating the development of novel multitarget antibacterial agents. In this study, a new series of aryl-urea derivatives incorporating uracil and perfluorophenyl moieties were synthesized and structurally characterized using NMR, IR, and elemental analyses. Among the synthesized compounds, perfluorophenyl-based derivatives (compounds 4 and 5) exhibited potent antibacterial activity against vancomycin-resistant Enterococcus faecalis and carbapenem-resistant Acinetobacter baumannii, with low minimum

Clostridium innocuum is increasingly detected in patients with Clostridioides difficile infection (CDI), particularly following vancomycin t … the BAX/BCL-2 ratio, driving apoptosis through PARP1 activation. Notably, the organism's intrinsic vancomycin resistance enables its survival and expansion during CDI treatment, fostering its emergence as a pathobiont. Our results demonstrate that toxigenic C. difficile facilitates C. innocuum epithelial invasion and exacerbates host cell damage through synergistic in …

The rapid emergence of multi-drug resistant (MDR) pathogenic bacteria as well as the continued burden of malignant diseases requires the safe development of novel and multifunctional therapeutic agents. The WHO designated vancomycin-resistant Staphylococcus aureus (VRSA) as "high priority" AMR pathogen. Thus, this study aimed to ecofriendly synthesis of silver-selenium nanocomposite (Ag-Se NC) and evaluate its in vitro anticancer effect and inhibitory activity against VRSA clinical isolates. Biogenic Ag-Se NC was successfully synthesized using the aqueous peel extract of Cucumis melo (C. melo)

Colonization of the gastrointestinal (GI) tract by vancomycin-resistant Enterococcus faecium (VREfm) often precedes bloodstream infection and serves as a reservoir for onward patient transmission in healthcare settings. Routine clonal isolate-based sequencing often underestimates within-patient diversity and can miss transmission involving low-abundance and co-colonizing strains. Here, we applied culture-enriched metagenomic sequencing to matched GI tract and blood VREfm populations collected ≤14 days apart from 35 patients with positive VREfm blood cultures obtained between 2020 and 2025 at a

Enterococcus faecium is a major global health threat, particularly in high-risk patient groups, and the emergence of vancomycin-resistant strains (VREfm) has severely limited treatment options. Here, we conducted a comprehensive genomic analysis of two clinical vanA-type VREfm isolates. Two VREfm strains were isolated 20 days apart from surgical site infections (SSIs) from two different hospitalised patients in obstetrics and gynaecology departments because of post-operative wound infections. Antibiotic susceptibility profile, whole-genome sequencing (WGS), and bioinformatic analyses were cond

BACKGROUND: Vancomycin-resistant Enterococcus (VRE) are present across the One Health continuum and pose a considerable risk for transmission along the food chain. This systematic review and meta-analysis estimates the prevalence of VRE colonization in livestock, food of animal origin, and in human populations.
METHODS: Embase, MEDLINE and CAB Abstracts were searched for eligible literature. A total of 54 manuscripts passed inclusion criteria by providing prevalence estimates of VRE in a human population and at least one of either livestock or food. Random effects meta-analysis was conducted

OBJECTIVES: Faecal microbiota transplantation (FMT) has shown promise as a treatment for recurrent or refractory Clostridioides difficile infections. This study aimed to evaluate the decolonization effects of FMT on vancomycin-resistant Enterococcus (VRE).
METHODS: This feasibility trial prospectively recruited patients with more than three recurrent VRE infections. FMT was performed by infusing faecal microbiota solutions from healthy, unrelated donors into the participants' guts via colonoscopy. Faecal microbiota profiles before and after FMT were analysed.
RESULTS: Three of the six patien

The isolation of affected patients and intensified infection control measures are used to prevent nosocomial transmission of vancomycin-resistant enterococci (VRE), but early detection of VRE carriers is needed. However, there are still no standard screening criteria for VRE, which poses a significant threat to patient safety. Our study aimed to develop and evaluate an artificial intelligence (AI)-based approach for identifying and predicting of at-risk patients who could assist infection prevention and control staff through a human-in-the-loop approach. We used data from 8,372 patients, combi

BACKGROUND: Vancomycin-resistant Enterococcus faecium (VREfm) are significant nosocomial pathogens. Irish VREfm comprise diverse vanA-encoding ST80-complex type (CT) lineages. Recent studies indicate that within-patient VREfm diversity could confound surveillance. This study investigated the intra-host VREfm genetic diversity among colonized Irish hospital patients.
METHODS: Rectal VREfm (n = 150) from 10 patients (15 isolates each) were investigated by WGS, core-genome MLST and split k-mer (SKA)-SNP analysis. Plasmids and vanA-transposons from 39 VREfm representative of CTs identified were r

INTRODUCTION: Methicillin-resistant Staphylococcus aureus (MRSA), a notorious multidrug-resistant (MDR) pathogen, frequently resides and proliferates within macrophages, contributing to refractory and recurrent infections. Conventional antibiotics exhibit limited efficacy against intracellular MRSA due to poor cellular penetration.
METHODS: Vancomycin (VAN) was encapsulated into membrane vesicles (ΔagrAMVs) derived from the attenuated S. aureus strain RN4220ΔagrA, generating VAN-loaded nanoparticles (ΔagrAMV-VAN). In vitro and in vivo experiments were performed to test the efficacy of ΔagrAMV

Introduction: Methicillin-resistant Staphylococcus aureus pneumonia (PNA) can be ruled out via methicillin-resistant Staphylococcus aureus (MRSA) culture and polymerase chain reaction (PCR) nasal screening, facilitating the de-escalation of empiric anti-MRSA agents like intravenous vancomycin. This study evaluated the impact of transitioning from culture to PCR-based MRSA nasal screening in patients with PNA. Methods: This Institutional Review Board (IRB)-approved retrospective quasi-experimental study was conducted at a 5-hospital system and included adult, nonpregnant hospitalized patients f

BACKGROUND: Vancomycin-resistant enterococcal (VRE) infections pose significant challenges in healthcare. Transmission dynamics of VRE are complex, often involving patient colonization and subsequent transmission through various healthcare-associated vectors. We utilized a whole genome sequencing (WGS) surveillance program at our institution to better understand the contribution of clinical and colonizing isolates to VRE transmission.
METHODS: We performed whole genome sequencing on 352 VRE clinical isolates collected over 34 months and 891 rectal screening isolates collected over a 9-month n
