Cardiovascular Calcification Heterogeneity in Chronic Kidney Disease.

Patients with chronic kidney disease (CKD) exhibit tremendously elevated risk for cardiovascular disease, particularly ischemic heart disease, due to premature vascular and cardiac aging and accelerated ectopic calcification. The presence of cardiovascular calcification associates with increased risk in patients with CKD. Disturbed mineral homeostasis and diverse comorbidities in these patients drive increased systemic cardiovascular calcification in different manifestations with diverse clinical consequences, like plaque instability, vessel stiffening, and aortic stenosis. This review outline…

Risk factors for atherosclerotic and medial arterial calcification of the intracranial internal carotid artery.

BACKGROUND AND AIMS: Calcifications of the intracranial internal carotid artery (iICA) are an important risk factor for stroke. The calcifications can occur both in the intimal and medial layer of the vascular wall. The aim of this study is to assess whether medial calcification in the iICA is differently related to risk factors for cardiovascular disease, compared to intimal calcification. METHODS: Unenhanced thin slice computed tomography (CT) scans from 1132 patients from the Dutch acute stroke study cohort were assessed for dominant localization of calcification (medial or intimal) by one

Bone mineral density and mortality in end-stage renal disease patients.

Osteoporosis characterized by low bone mineral density (BMD) as assessed by dual-energy X-ray absorptiometry (DXA) is common among end-stage renal disease (ESRD) patients and associates with high fracture incidence and high all-cause mortality. This is because chronic kidney disease-mineral bone disorders (CKD-MBDs) promote not only bone disease (osteoporosis and renal dystrophy) but also vascular calcification and cardiovascular disease. The disturbed bone metabolism in ESRD leads to 'loss of cortical bone' with increased cortical porosity and thinning of cortical bone rather than to loss of …

Coronary artery calcification and cardiovascular disease in children with chronic kidney disease.

PURPOSE OF REVIEW: Cardiovascular disease is the leading cause of death in children and young adults with end-stage renal disease (ESRD). As … e development of coronary artery calcification in ESRD. Recent studies have shown that induction of vascular calcification begins in early normophosphatemic CKD by the reduction of vascular Klotho and increased FGF23 secretion. Pediatric studies confirmed the presence of abnormal FGF23 and Klotho metabolism and the association of increased circulating FGF23 with coronar …

18F-FDG uptake and calcifications in the thoracic aorta on positron emission tomography/computed tomography examinations: frequency and stability on serial scans.

PURPOSE: Vascular 18F-2-fluoro-2-deoxy-D-glucose (18F-FDG) uptake has been suggested as a means of identifying metabolically active atheroma, whereas vascular calcification is accepted as an indicator of atherosclerosis. This investigation describes the frequency and stability of 18F-FDG uptake and vascular calcification in the thoracic aorta on serial 18F-FDG positron emission tomography/computed tomography (PET/CT) studies and correlates the findings with clinical data and risk factors for cardiovascular disease (CVD). MATERIALS AND METHODS: Serial 18F-FDG-PET/CT scans of 100 cancer patient…

Bone-Vascular Axis in Chronic Kidney Disease.

Patients with chronic kidney disease (CKD) are at increased risk of osteoporosis and vascular calcification. Bone demineralization and vascular mineralization go often hand in hand in CKD, similar to as in the general population. This contradictory association is independent of aging and is commonly referred to as the "calcification paradox" or the bone-vascular axis. Various common risk factors and mechanisms have been identified. Alternatively, calcifying vessels may release circulating factors that affect bone metabolism, while bone disease may infer conditions that favor vascular calcifica…

Dysregulated mineral metabolism in children with chronic kidney disease.

PURPOSE OF REVIEW: Cardiovascular disease (CVD) begins early in the course of chronic kidney disease (CKD) and is the most common cause of death even in children with CKD. Mechanisms of CVD development are poorly understood. This review focuses on the role of dysregulated homeostasis of calcium (Ca), phosphorus (P), parathyroid hormone (PTH) and vitamin D in ectopic vascular calcification. RECENT FINDINGS: Converging evidence from clinical, epidemiological and translational research studies suggest that uraemic vasculopathy and ectopic vascular calcification begin in predialysis CKD, progress…

The relationship between serum osteopontin and FGF 23 levels with valvular calcification in hemodialysis patients
.

BACKGROUNDS AND AIMS: Cardiovascular calcification is an important cause of morbidity and mortality in hemodialysis (HD) patients. Vascular and valvular calcification are indicators of increased tissue calcification. The relationship of osteopontin (OPN) - which is known as a vascular calcification inhibitor - and fibroblast growth factor-23 (FGF-23) - which its related to vascular calcification, as recently shown - to valvular calcification is unknown. In this cross-sectional study, we examined the relationship between heart valve calcification, serum OPN, and FGF-23 levels. MATERIALS AND ME…

Toll-like receptor 9 promotes aortic chondrogenesis and calcification in apolipoprotein E-deficient mice.

Vascular calcification represents a significant clinical challenge, leading to cardiovascular disease, though its underlying mechanisms remain incompletely understood. Recent studies indicate that Toll-like receptor 9 (TLR9), a key element of innate immunity, plays a pathogenic role in vascular inflammation and atherogenesis. Therefore, we hypothesized that TLR9 signaling promotes vascular chondrogenesis and calcification. We compared apolipoprotein E-deficient (ApoE-/-) mice and Tlr9-/- ApoE-/- mice after 24 -weeks high-cholesterol diet feeding. There were no differences between the groups in…

Valvular heart disease and calcification in CKD: more common than appreciated.

Ischaemic heart disease, sudden cardiac death and arrhythmias, heart failure, stroke and peripheral arterial disease make up >50% of the causes of death in advanced chronic kidney disease (CKD). Calcification of the vascular tree and heart valves is partially related to these complications and has received growing attention in the literature. However, the main focus of research has been on the pathophysiology and consequences of vascular calcification, with less attention being paid to valvular calcification (VC) and its impact on the survival of CKD patients. Although VC has long been seen as…

Smooth muscle cell phenotypic transition associated with calcification: upregulation of Cbfa1 and downregulation of smooth muscle lineage markers.

Bovine aortic smooth muscle cell (BASMC) cultures undergo mineralization on addition of the organic phosphate donor, beta-glycerophosphate (betaGP). Mineralization is characterized by apatite deposition on collagen fibrils and the presence of matrix vesicles, as has been described in calcified vascular lesions in vivo as well as in bone and teeth. In the present study, we used this model to investigate the molecular mechanisms driving vascular calcification. We found that BASMCs lost their lineage markers, SM22alpha and smooth muscle alpha-actin, within 10 days of being placed under calcifying…

Etidronate prevents dystrophic cardiac calcification by inhibiting macrophage aggregation.

Cardiovascular calcification is associated with high risk of vascular disease. This involves macrophage infiltration of injured vascular tissue and osteoclast-related processes. Splenic monocytes from mice, that are predisposed (C3H) or resistant (B6) to calcification, were isolated and differentiated in vitro with M-CSF to generate macrophages, which aggregate to form multinucleated (MN) cells in the presence of RANKL. MN cell formation was significantly decreased in monocytes from resistant compared with calcifying mice. Conditioned media from C3H macrophages strongly induced calcification i…

Targeting TREM2 signaling shows limited impact on cerebrovascular calcification.

Brain calcification, the ectopic mineral deposits of calcium phosphate, is a frequent radiological finding and a diagnostic criterion for pr … e pathologies, we investigated whether administration of a TREM2-activating antibody could mitigate vascular calcification in Pdgfb ret/ret mice. Single-nucleus RNA-sequencing analysis showed that calcification-associated microglia share transcriptional similarities to disease-associated microglia and exhibited activated TREM2 and TGFβ signaling. Administration of a TRE …

Network determinants of cardiovascular calcification and repositioned drug treatments.

Ectopic cardiovascular calcification is a highly prevalent pathology for which there are no effective novel or repurposed pharmacotherapeutics to prevent disease progression. We created a human calcification endophenotype module (ie, the "calcificasome") by mapping vascular calcification genes (proteins) to the human vascular smooth muscle-specific protein-protein interactome (218 nodes and 632 edges, P < 10-5 ). Network proximity analysis was used to demonstrate that the calcificasome overlapped significantly with endophenotype modules governing inflammation, thrombosis, and fibrosis in the h…

Noninvasive Assessment of Carotid Plaques Calcification by 18F-Sodium Fluoride Accumulation: Correlation with Pathology.

BACKGROUD: Vascular calcification is currently recognized as an important pathobiological process in atherosclerosis, but the mechanism remains elusive. Given the similarities in vascular calcification and bone formation, 18F-sodium fluoride (18F-NaF) is now considered a novel marker of vascular calcification. This study aimed to correlate 18F-NaF accumulation with the histological characterization of vascular calcification in carotid plaques. METHODS: A total of 8 patients who were undergoing carotid endarterectomy (CEA) for carotid artery stenosis were recruited. Before CEA, 18F-NaF positro…
