Paclitaxel-coated balloon adjunct for benign ureteral and ureteroenteric strictures: a narrative review and translational proposal for enhanced endourological patency.

BACKGROUND AND OBJECTIVE: Conventional endourological techniques, including balloon dilation and endoureterotomy, offer mechanical luminal relief but do not modulate the fibroproliferative cascade that drives restenosis. Paclitaxel-coated balloons, widely validated in vascular and urethral interventions, deliver localized antiproliferative therapy with minimal systemic exposure. Their mechanism-selective drug uptake into the urothelium, submucosa, and muscularis-may theoretically enhance long-term patency after endoscopic management. This review aims to summarize the translational rationale, m

Regional citrate anticoagulation vs systemic heparin anticoagulation for double-filtration plasmapheresis.

INTRODUCTION: Double-filtration plasmapheresis (DFPP) has been utilized for immunomodulation in kidney transplantation. Anticoagulation is important to maintain circuit patency during DFPP. We aimed to compare the efficacy and safety of regional citrate anticoagulation (RCA) with systemic heparin anticoagulation during DFPP in kidney transplant recipients. METHODS: A retrospective cohort study was conducted to compare the efficacy and safety of RCA (RCA-DFPP) to systemic heparin anticoagulation (Hep-DFPP) for DFPP among kidney transplant recipients in a single tertiary center. RESULTS: A tot

Early cannulation prosthetic graft (Acuseal) for arteriovenous access: a useful option to provide a personal vascular access solution.

PURPOSE: Early cannulation arteriovenous grafts (ecAVGs), such as the GORE Acuseal, have "low bleed" properties permitting cannulation within 24 hours of insertion. They may provide an alternative to tunneled central venous catheters (and associated line complications) in patients requiring urgent vascular access. METHODS: We present our early experience of 37 patients treated with the GORE Acuseal ecAVG. RESULTS: A total of 11 upper limb, 24 lower limb and 2 complex graft procedures were performed. Indications for ecAVG were as follows: bridge to transplantation (21.6%); bridge to arteriove

Endothelial nitric oxide synthase gene intron 4 (VNTR) polymorphism and vascular access graft thrombosis.

Vascular access thrombosis is a leading cause of vascular access failure in hemodialysis patients. Thrombosis is a multifactorial condition and genetic makeup can affect thrombosis risk. We conducted a study to investigate for possible associations between ecNOS gene intron 4 variable-number tandem repeat (VNTR) polymorphism and thrombosis of polytetrafluoroethylene hemodialysis arteriovenous access grafts (AVG) in Turkish patients. Fifty-five patients with end-stage renal disease who had AVGs implanted between 2000 and 2002 and 167 healthy individuals representing our healthy population were

Distal arteriovenous fistula as an adjunct to maintaining arterial and graft patency for limb salvage.

Sixty-one distal arteriovenous fistulas (dAVFs) were constructed as adjuncts to tibial and peroneal vascular reconstructive procedures in 58 patients threatened with imminent limb loss. Specific indications for dAVF construction were absent or deficient pedal arches, usually associated with small, calcareous crural vessels. Conventional bypasses had been previously performed in 35 patients. Cumulative graft patency rates were 56%, 39%, and 18% at 6, 12, and 24 months, respectively. For the same intervals, the figures were 56%, 52%, and 52% (P less than 0.001 at 24 months) for nonAVF reconstruc

Mimicking techniques in small diameter vascular grafts: enhancing design through multi-layered electrospinning techniques.

The increasing prevalence of occlusive arterial diseases, such as peripheral arterial disease and coronary heart disease, underscores the critical need for effective vascular interventions. Traditional methods, including autologous vessel grafting, often fall short in the case of small-diameter vascular grafts (SDVGs), which present challenges such as thrombosis and limited long-term patency. This study reviews recent advancements in electrospinning techniques aimed at generating biomimetic SDVGs that closely replicate the structure and mechanical properties of natural blood vessels. By employ

Ultrasound and computed tomographic evaluation in hepatic transplantation.

Ultrasound and CT are valuable in the non-invasive evaluation of complications of hepatic transplantation. Ultrasound is particularly helpful in detecting biliary obstruction and in documenting vascular patency. As with renal transplantation, the value of US in the assessment of rejection will likely increase with time. Computed tomography is most valuable in identifying large parenchymal abnormalities and abdominal fluid. Both US and CT can be used to guide diagnostic and therapeutic percutaneous procedures in hepatic transplant recipients.

Diabetes does not worsen outcomes following infrageniculate bypass or endovascular intervention for patients with critical limb ischemia.

OBJECTIVE: Both open surgery and endovascular peripheral interventions have been shown to effectively improve outcomes in patients with peripheral arterial disease, but minimal data exist comparing outcomes performed at and below the knee among patients with diabetes (DM) specifically. The purpose of this study is to compare outcomes following open bypass (lower extremity bypass [LEB]) and peripheral vascular intervention (PVI) at and below the knee in patients with DM vs patients without DM (non-DM) with critical limb ischemia. METHODS: Data from the 2008-2014 Vascular Quality Initiative (VQ

Infrapopliteal run-off and the outcome of femoropopliteal percutaneous transluminal angioplasty.

BACKGROUND: The outcome of percutaneous transluminal angioplasty (PTA) of peripheral arterial lesions is influenced by several factors, including the haemodynamic conditions. Our study tested: (a) whether infrapopliteal run-off after completed PTA influenced the time course of restenosis/reocclusion of the femoropopliteal arterial segment, and (b) whether worsening of infrapopliteal run-off influenced the long-term femoropopliteal patency after PTA. PATIENTS AND METHODS: Among 245 patients treated by femoropopliteal PTA we enrolled 176 patients who consented to regular follow-up. Concomitant

Central vein stenosis- prevalence, clinical characteristics, outcome and its impact on dialysis access.

Central vein stenosis (CVS) is a complication primarily affecting chronic hemodialysis patients, frequently resulting in vascular access dysfunction and decreased dialysis efficacy. While CVS has a variable mortality rate, its impact on dialysis access is profound, including arm edema, facial swelling, and venous hypertension. We describe the prevalence, clinical presentation, and treatment outcomes of CVS for vascular access in hemodialysis patients. We conducted a prospective observational study from July 2023 to June 2024, on patients undergoing maintenance hemodialysis (MHD) at a tertiary

[Local thrombolytic treatment of peripheral arterial thrombosis and embolism].

Local intra-arterial low-dose thrombolysis has become a therapeutic alternative for acute and subacute occlusion of vascular grafts and native vessels in the lower limbs. The series comprises 31 patients treated with Streptokinase as thrombolytic agent. Complete primary thrombolysis was achieved in 20 patients, whereas in 11 patients the outcome was only partially successful or a failure. Vascular stenoses were considered to precipitate thrombosis in 18 cases, and prompted percutaneous transluminal angioplasty after thrombolysis. Two-year patency was 48% (30-66%) in the total series and 74% (5
