Hemorrhagic cystitis complicating untreated necrotizing vasculitis.

Although the genitourinary system is frequently affected in polyarteritis nodosa, involvement of the wall of the urinary bladder is extremely uncommon; moreover, when reported, it has been merely as an incidental finding late in the disease course. In contrast, hemorrhagic cystitis is a well-recognized complication of cyclophosphamide therapy for the systemic vasculitides, but has not previously been reported to complicate untreated vasculitis. Herein is described a patient who presented with non-treatment-related hemorrhagic cystitis which was found to be due to nongranulomatous necrotizing v…

Digital vasculitis following allergic desensitization treatment.

Systemic necrotizing vasculitis or a polyarteritis nodosa-like clinical presentation, is an unusual complication of immunotherapy (hyposensitization therapy). We describe a patient who developed features of vasculitis several years after beginning hyposensitization treatment for allergic rhinitis. In the 7 months preceding the onset of the vasculitis he experienced 4 episodes of anaphylaxis immediately after receiving desensitization injections. The vasculitis was characterized by the abrupt onset of pain and discoloration of the middle 3 digits of his right hand. Cyanosis and small areas of f…

Cerebral vasculitis associated with cocaine abuse.

BACKGROUND: Earlier reports of cocaine-associated cerebral vasculitis have been based primarily on angiographic findings without pathological verification. CASE DESCRIPTION: We present a case of acute encephalopathy following intravenous and intranasal administration of cocaine. Brain biopsy revealed vascular changes involving primarily small arteries. Findings included lymphocytic infiltration, endothelial thickening, and deposition of proteinaceous amorphous material within and around vessel walls. CONCLUSIONS: These abnormalities are consistent with pathological features of arteritis prev…

Scleroderma and central nervous system vasculitis.

We describe a patient with scleroderma (CREST syndrome) and central nervous system vasculitis. While angiography demonstrated segmental symmetrical arterial narrowing characteristic of vasculitis, results of leptomeningeal biopsy were normal. There was no evidence of systemic vasculitis, renal failure, or malignant hypertension previously thought to be required to explain central nervous system dysfunction in patients with scleroderma. Signs and symptoms attributable to vasculitis were reversible with aggressive immunosuppressive therapy.

Coagulation factors in orbital venous vasculitis.

Orbital venous vasculitis has earlier been shown to be associated with spontaneous ecchymoses. With the reservation that platelet function was not investigated and fibrinolysis only partly, this study of the coagulation factors of eight patients with orbital venous vasculitis showed normal findings, suggesting immunologic etiology rather than coagulation deficiency as the cause of the spontaneous ecchymoses.

Antineutrophil cytoplasm antibody (ANCA) associated vasculitis.

In 1982 we first reported the presence of antineutrophil cytoplasm antibodies (ANCA) in 8 patients with systemic vasculitis and segmental necrotizing glomerulonephritis. The results of long-term follow-up are described. Screening of 7,500 serum samples revealed positive ANCA in 9 additional patients with vasculitis. Eighty-eight other patients with vasculitis were ANCA negative, including 7 with microscopic polyarteritis nodosa (MPAN) and 8 with Wegener's granulomatosis (WG). Conversely, ANCA were never detected in the absence of vasculitis. Fourteen patients presenting with glomerulonephritis…

[Diffuse pulmonary hemorrhage in pulmonary vasculitis].

Diffuse pulmonary hemorrhages may occur in pulmonary vasculites, including Wegener disease, systemic lupus erythematosus, Behçet disease, and other systemic vasculites. The pulmonary bleeding is often the first manifestation of the disease and frequently denotes a severe form. The etiologic diagnosis can be established only by performing a surgical lung biopsy, which is often a difficult decision. Leukoclastic capillaritis of the alveolar septa is the common features shared by all the vasculites responsible for pulmonary hemorrhage.

Testicular vasculitis: implications for systemic disease.

Nine cases of testicular vasculitis were identified from the surgical pathology and autopsy files of the Johns Hopkins Hospital. In three cases this was the initial manifestation of polyarteritis nodosa. Two of these men presented with recurrent testicular pain and fever, with orchiectomy samples showing focal infarcts and necrotizing vasculitis. The third man presented with epididymitis, with his biopsy specimen showing vasculitis. In two cases, men presented with systemic and testicular signs of polyarteritis nodosa, and the diagnosis was made on testicular biopsy and later studied at autops…

Choroidal involvement in systemic necrotizing vasculitis.

Wegener's granulomatosis and lymphomatosis are systemic necrotizing vasculitides of unknown cause. We report similar involvement of the choriocapillaris in these two diseases. A presumed choriocapillaritis caused ischemia and infarction of the choriocapillaris-retinal pigment epithelium-outer neurosensory retina complex. Systemic corticosteroid and immunosuppressive agents were successful in controlling systemic disease and preserving useful vision. At the time of this study, one patient had not had a recurrence for 12 years, and the other patient had been asymptomatic for seven years.

Systemic necrotizing vasculitis presenting as epididymitis.

Epididymitis is an uncommonly recognized complication of systemic necrotizing vasculitis. We report 2 cases in which epididymitis heralded the onset of more severe visceral organ involvement. Early biopsy of atypical epididymitis in patients with hypertension or constitutional symptoms is important if effective therapy is to be instituted before irreversible organ damage occurs.

Cutaneous vasculitis in adult polymyositis/dermatomyositis.

Seven (9.2%) of 76 patients with adult-onset polymyositis/dermatomyositis (PM/DM) seen over an 11-year period had cutaneous vasculitis. This was manifest by dermal and/or subcutaneous nodules in 4, periungual infarcts in 3 and digital ulceration in 2. When these 7 patients were compared to the remaining 69, a significant association was noted between cutaneous vasculitis and DM (p = .025); only 1 of 31 patients with primary PM and none of 18 with overlap syndromes had vasculitis. Furthermore, 2 (28.6%) of those with vasculitis had an associated malignancy compared to only 4 (5.8%) of those wit…

[Giant-cell vasculitis (arteritis and phlebitis)].

Vasculitis was detected in 5 patients although not in the temporal artery but in vessels of other localizations: subcutaneous veins of the lower extremities (an adolescent of 17 and a man of 41), cardiac and renal arteries (a man of 32), gall bladder artery (a woman of 72), veins of the spermatic cord (a man of 28). Giant cells formed in granulomatous reaction, most frequently in the intima of the vessel or close to it in such nosological forms as endarteritis, periarteritis nodosa, obliterating thromboangiitis (Buerger's disease). A hypothesis is suggested concerning the etiological role in t…

Relapsing polychondritis associated with cutaneous vasculitis.

A patient with relapsing polychondritis associated with cutaneous vasculitis is described in detail. Clinically, a systemic chondritis complicated by conjunctivitis, iritis, otitis media, tracheal stenosis, sinusitis, and cutaneous vasculitis were observed in this case. Corticosteroids have been effective in controlling the clinical symptoms as well as the vasculitis.
