Spontaneous bilateral internal carotid and vertebral artery dissections with dominant-hemisphere circulation maintained by external carotid artery-ophthalmic artery anastomoses.

Spontaneous cervical artery dissection (sCAD) is a major cause of stroke in young adults. Multiple sCAD is a rarer, more poorly understood presentation of sCAD that has been increasingly attributed to cervical trauma such as spinal manipulation or genetic polymorphisms in extracellular matrix components. The authors present the case of a 49-year-old, otherwise healthy woman, who over the course of 2 weeks developed progressive, hemodynamically significant, bilateral internal carotid artery and vertebral artery dissections. Collateral response involved extensive external carotid artery-internal…

Bilateral internal carotid artery and vertebral artery dissections with retinal artery occlusion after a roller coaster ride - case report and a review.

We present the first case of a woman with no significant medical history who developed dissections of bilateral carotid and bilateral verteb … eurological exam was otherwise normal. The cerebral angiogram showed bilateral internal carotid and vertebral artery dissections. The patient remained stable with conservative therapy without further worsening of vision or any new neurological deficits. Outcomes for cervicocephalic arterial dissection are usually favorable, but early diagnosis is critical for initiation of a …

A near-fatal consequence of chiropractor massage: massive stroke from carotid arterial dissection and bilateral vertebral arterial oedema.

A 35-year-old Chinese man with no risk factors for stroke presented with a 2-day history of expressive dysphasia and a 1-day history of right-sided weakness. The presentation was preceded by multiple sessions of neck, shoulder girdle and upper back massage for pain relief in the prior 2 weeks. CT of the brain demonstrated an acute left middle cerebral artery infarct and left internal carotid artery dissection. MRI cerebral angiogram confirmed left carotid arterial dissection and intimal oedema of bilateral vertebral arteries. In the absence of other vascular comorbidities and risk factors, mas

Late morphological progression of a dissecting basilar artery aneurysm after staged bilateral vertebral artery occlusion: case report.

BACKGROUND: The authors present a patient who experienced late (5-year follow-up) morphological progression of a dissecting aneurysm of the distal basilar artery after treatment with a combined microsurgical and neuroendovascular Hunterian strategy. In addition to postulating about the possible reasons underlying the evolution of this lesion, the role of stenting is discussed. CASE DESCRIPTION: The patient was 37 years old when she suffered a subarachnoid hemorrhage from spontaneous basilar artery dissection. At the time of the hemorrhage, minimal aneurysmal enlargement was noted angiographic

Concurrent Reversible Cerebral Vasoconstriction Syndrome and Bilateral Vertebral Artery Dissection in the Postpartum Period: A Case Report.

Reports of postpartum women with concurrent reversible cerebral vasoconstriction syndrome (RCVS) and bilateral vertebral artery dissection (VAD) are rare. Guidelines on the management of future pregnancies in these patients are also limited. We present a case of a 33-year-old female who developed a severe, sudden-onset headache 2 h after a vaginal birth in a maternity hospital. The severe headaches recurred and were associated with neck pain, nausea, vomiting, and photophobia. Preliminary neuroimaging only included a noncontrast CT brain. This was normal and she was treated with triptans for p…

Structural Restoration of an Unruptured Vertebral Artery Dissection Following Autologous Mesenchymal Stem Cell Therapy: A Case Report.

Vertebral artery dissection (VAD) is a recognized cause of ischemic stroke, and current management relies on antithrombotic therapy that is not specifically directed at structural repair of the arterial wall. We report a man in his mid-50s who presented with a sudden, severe right occipital headache after a golf swing and had a history of a contralateral left VAD 16 years earlier. High-resolution vessel wall magnetic resonance imaging and angiography showed an acute, unruptured dissection of the right V4 segment with an intramural hematoma, and serial imaging documented progression of the true…

Posterior circulation stroke secondary to atlantoaxial instability-induced vertebral artery dissection and pseudoaneurysm in a paediatric patient.

A boy in his early teens presented with transient visual disturbances and headache. Brain imaging revealed bilateral occipital infarctions, while vascular studies demonstrated bilateral V3 vertebral artery wall abnormalities with pseudoaneurysm formation associated with C1-C2 rotatory subluxation. Initial conservative management, including resting and a rigid cervical brace, failed to prevent vascular progression. The patient underwent posterior C1-C2 fixation to address the underlying mechanical cause. Follow-up CT angiography 1 year after surgery demonstrated complete resolution of the pseud

Poor neurological outcome after treatment with photobiomodulation in a patient recovering from sequelae of vertebral artery dissection.

BACKGROUND: PhotBioModulation Therapy (PBMT) is regarded as a safe therapy for various disorders. We report a patient with a vertebral artery dissection treated with PBMT resulting in a poor neurological outcome. CASE PRESENTATION: A 69-year-old patient with a PICA infarction after a vertebral artery dissection was treated with PhotoBioModulation Therapy using a helmet. This therapy induces strong vasodilatation in the area of application. Shortly after this treatment, she fell asleep and did not wake up the next morning. Clinically she was comatose and an MRI showed bithalamic and mesencepha…

High-resolution magnetic resonance imaging in the diagnosis and management of vertebral artery dissection: A case report.

BACKGROUND: Vertebral artery dissection (VAD) is a rare but life-threatening condition characterized by tearing of the intimal layer of the vertebral artery, leading to stenosis, occlusion or rupture. The clinical presentation of VAD can be heterogeneous, with common symptoms including headache, dizziness and balance problems. Timely diagnosis and treatment are crucial for favorable outcomes; however, VAD is often missed due to its variable clinical presentation and lack of robust diagnostic guidelines. High-resolution magnetic resonance imaging (HRMRI) has emerged as a reliable diagnostic too…

Profiles of Vertebral Artery Dissection with Congenital Craniovertebral Junction Malformation: Four New Cases and a Literature Review.

OBJECTIVE: Vertebral artery dissection (VAD) combined with congenital craniovertebral junction malformation (CVJM) is rare. This study aimed to analyze the etiology, clinical and imaging features, treatment, and prognosis of VAD with CVJM. METHODS: Four new cases of VAD with congenital CVJM and 28 similar cases found in the literature were included. Detailed clinical data from all cases were retrospectively analyzed. RESULTS: A total of 32 patients (28 men, four women; mean age 19.01±12.53 years) were included. Seventeen of 32 cases (53.1%) had had multiple ischemic episodes. The most common…

Bilateral Internal Carotid and Left Vertebral Artery Dissection after Blunt Trauma: A Case Report and Literature Review.

Multi-vessel cervical arterial injury after blunt trauma is rare, and its pathophysiology is unclear. Although blunt cerebrovascular injury … he developed bleariness. Computed tomography angiography showed bilateral internal carotid and left vertebral artery dissection. Aspirin therapy was started immediately, and then clopidogrel was added to the regimen. Two weeks later, magnetic resonance angiography (MRA) showed improved blood flow of the vessels. Only aspirin therapy was continued. About 3 months after discha …

Subarachnoid haemorrhage due to intracranial vertebral artery dissection presenting with atypical cauda equina syndrome features: case report.

BACKGROUND: Failing to recognise the signs and symptoms of subarachnoid haemorrhage (SAH) causes diagnostic delay and may result in poorer outcomes. We report a rare case of SAH secondary to a vertebral artery dissection (VAD) that initially presented with cauda equina-like features, followed by symptoms more typical of SAH. CASE PRESENTATION: A 55-year-old man developed severe lower back pain after sudden movement. Over the next 5 days he developed paraesthesiaes in the feet, progressing to the torso gradually, and reported constipation and reduced sensation when passing urine. On day six he…

Headache and transient visual loss as the only presenting symptoms of vertebral artery dissection: a case report.

BACKGROUND: Vertebral artery dissection is an important cause of stroke in the young and diagnosis is often challenging as symptoms are varied and subtle. CASE PRESENTATION: A 33-year-old, previously healthy, white male office worker was stretching his neck when he developed sudden left-sided visual loss lasting 5 minutes associated with headache. He had no other neurological symptoms or signs. He was investigated with a computed tomography angiogram, which revealed a left vertebral artery dissection with a right posterior cerebral artery vascular occlusion. CONCLUSIONS: We describe an atypi…

Spontaneous vertebral artery dissection with multiple supratentorial and infratentorial acute infarcts in the posterior circulation Case report.

The article represents a case of a young patient with atypical clinical and paraclinical presentation of vertebral artery dissection by multiple cerebral infarcts, localized at the supratentorial and infratentorial levels in the posterior circulation. A case of a 21-year-old man, without a history of trauma in the cervical area or at the cranial level, without recent chiropractic maneuvers or practicing a sport, which required rapid, extreme, rotational movements of the neck, was examined. He presented to the emergency room with nausea, numbness of the left limbs, dysarthria, and incoordinatio…
