It is well known that paroxysmal positional vertigo is induced by change of head position and subsides immediately when the head is returned to the original position. There are two types of paroxysmal positional vertigo. One is what DIX and HALLPIKE (1952) call the benign type which is thought to be caused by otolith lesions. Another is a malignant type, in contrast to the former and is related to a lesion in the central nervous system. Otologists are usually familiar with the former, but the latter is not known among neurologists and neurosurgeons. We experienced 37 cases of the malignant typ…

The two main problems in defining and classifying the syndrome of benign paroxysmal vertigo (BPV) in childhood are the vestibular function pattern and the relationship between BPV and migraine. 13 children suffering from this syndrome were submitted to complete otoneurological examination, including caloric and rotational labyrinthine stimulation with ENG recording, and to headache provocation tests with nitroglycerin, histamine and fenfluramine. Vestibular responses were normal in all except 2 cases which presented signs of central vestibular impairment at the level of the vestibulocerebellar…

Sixty-two patients with vertigo were examined with ABR. Eight of the 30 patients with vestibular neuronitis had abnormal ABR indicating brain stem dysfunction. Most of the patients with abnormal ABR showed continued ABR abnormality after recovery from the disease. Only one patient with epidemic vertigo had abnormal ABR, the remaining 15 patients had no signs of brain stem involvement. All 16 patients with benign positional vertigo had normal ABR.

In order to validate the hypothesis that height vertigo is based on visual destabilization of free stance when the distance between eye and object becomes critically large, several of its consequences were demonstrated in posturographic experiments: (1) Visual signals conflicting with simultaneous vestibular and somatosensory inputs provided by sinusoidally tilting rooms may destabilize postural sway in the fore-aft as well as in the lateral direction. (2) In natural surrounding sway amplitudes increase with increasing eye-object distance up to 5 meters. Thus, teleologically, subjective height…

The clinical and electronystagmographic findings of 255 patients with paroxysmal positional vertigo are presented. We believe the disorder should be diagnosed on the basis of the history and clinical findings since we did not find any characteristic electronystagmographic findings other than the recording of an actual attack. Our observations lead us to believe that there is only one type of attack. We conclude that the nystagmus which occurs during the attack is generated by stimulation of the posterior semicircular canal of the lowermost ear.
