Port-site recurrence after complete resection of stage I thymoma by video-assisted thoracoscopic surgery: report of a case.

A 62-year-old female with myasthenia gravis and a mediastinal tumor underwent extended thymothymectomy by video-assisted thoracoscopic surgery (VATS). The tumor was diagnosed as a stage I type B2 thymoma. Eight years after the initial surgery, an abnormal shadow was detected on the annual follow-up chest radiograph, and computed tomography showed a homogeneous 35 × 15 mm hemispherical mass under a port-site scar in the right anterior chest wall. The mass was between the second and third ribs and was found to be adherent to the lung during surgery, and was excised via lung and chest wall resect

Video-assisted thoracoscopic surgery with posterior spinal reconstruction for the resection of upper lobe lung tumors involving the spine.

BACKGROUND CONTEXT: Video-assisted thoracoscopic surgery (VATS) is associated with less morbidity and recovery time compared with traditional open thoracotomy (OT) for the resection of early stage non-small cell lung cancer (NSCLC). Local invasion of NSCLC into adjacent vertebrae confers a TNM T status of T4. Anatomical lobectomy by VATS with simultaneous posterior spinal reconstruction (PSR), as a single procedure, offers advantages to selected patients judged as suitable candidates for resection. PURPOSE: To report the preliminary results of a novel, multidisciplinary surgical technique for

Video-assisted Thoracoscopic Surgery (VATS) drainage of salmonella enteritidis empyema and needle-localization for retrieval of a dropped gallstone.

INTRODUCTION: Spillage of gallstones into the peritoneal cavity (dropped gallstones) is more common in laparoscopic compared with open cholecystectomy. Subsequent infectious complications are often delayed, occurring in 0.1% to 2.9% of cases. We present the first reported case of Salmonella enterica subphrenic abscess and empyema complicating dropped gallstones, treated with video-assisted thoracoscopic surgery (VATS), and surgical retrieval. CASE REPORT: A 67-year-old male with symptomatic cholelithiasis underwent an uncomplicated laparoscopic cholecystectomy. Four years later, he presented

Delayed onset of contralateral pulmonary edema following reexpansion pulmonary edema of a collapsed lung after video-assisted thoracoscopic surgery.

This case report describes a 61-year-old man who developed reexpansion pulmonary edema (RPE) of the collapsed left lung after video-assisted thoracoscopic surgery because of left thoracic empyema, complicated with secondary contralateral pulmonary edema later. The left lung was gently reexpanded after surgery under one-lung ventilation anesthesia for 2.5 hours. The patient developed RPE of the left lung immediately after surgery, and required mechanical ventilation with positive end-expiratory pressure support. RPE was resolved within 24 hours. Nevertheless, delayed onset of contralateral pulm

Video and robotic-assisted minimally invasive mitral valve surgery: a comparison of the Port-Access and transthoracic clamp techniques.

BACKGROUND: In order to assess different surgical techniques for video-assisted minimally invasive mitral valve surgery, a retrospective study was undertaken comparing the Port-Access system (Cardiovations, Ethicon Inc, Somerville, NJ) and the transthoracic clamp technique. METHODS: In 120 patients mitral valve surgery was performed through a small right minithoracotomy using either the Port-Access endovascular cardiopulmonary bypass system (Port-Access, n = 60) or the transthoracic clamp technique (MICRO, n = 60). Mean patient age was 61.5 +/- 10.5 years (81 patients with isolated mitral val

[Anesthetic management of a patient with dilated cardiomyopathy for left ventricular pacing lead insertion under video-assisted thoracic surgery].

A 54-year-old man with dilated cardiomyopathy treated with diuretics, alpha-beta blockers, antiarrhythmics for the previous 5 years, had the indication for biventricular pacing and was scheduled for placing of pacing leads in his left ventricular wall under video-assisted thoracic surgery. Preoperative tests revealed first degree A-V block with left bundle branch block and left ventricular dilation with an ejection fraction of 0.11 on echocardiography. Anesthesia was induced with ketamine and midazolam. Endotracheal intubation was facilitated by administration of vecuronium. Anesthesia was mai

Development and validation of a predictive model for intraoperative hypothermia in patients undergoing video-assisted thoracoscopic surgery: a retrospective study.

BACKGROUND: Intraoperative hypothermia is common during video-assisted thoracoscopic surgery (VATS) because of prolonged exposure of the pleural cavity and the thermoregulatory effects of anesthesia. Despite its clinical relevance, predictive tools tailored to VATS remain limited. This study aimed to develop and validate a practical model to identify patients at increased risk of intraoperative hypothermia. METHODS: This retrospective study included 651 patients who underwent VATS at a tertiary hospital in Beijing (January 2023–August 2025). Thirty-three perioperative variables were assessed.

The value of thoracoscopic ultrasound for the localization of ground-glass opacities with incomplete lung collapse in video-assisted thoracoscopic surgery.

BACKGROUND: Video-assisted thoracoscopic surgery (VATS) has been widely used for the resection of lung lesions. However, it is difficult to palpate or see small lesions, especially ground-glass opacities (GGOs) during VATS. Thoracoscopic ultrasound has definite value in locating pulmonary parenchymal nodules. However, due to the air in the lung parenchyma, its wide application is limited. This study investigated the value of thoracoscopic ultrasound for the localization of GGOs with incomplete lung collapse in VATS. METHODS: A retrospective analysis was conducted on patients diagnosed with gr

Video-Assisted Thoracic Surgery vs. Thoracotomy for the Treatment in Patients With Esophageal Leiomyoma: A Systematic Review and Meta-Analysis.

Background: Surgical treatment is usually suitable for patients with esophageal leiomyoma. Video-assisted thoracic surgery (VATS) offers a minimally invasive approach to thoracotomy. However, there is no clear conclusion on whether VATS can achieve an equal or even better surgical effect when compared with the traditional open approach in the treatment of esophageal leiomyoma. We performed this meta-analysis to explore and compare the outcomes of VATS vs. thoracotomy for patients with esophageal leiomyoma. Methods: PubMed, Cochrane Library, EMBASE, China National Knowledge Infrastructure (CNKI

A case of schwannoma at the origin of the right recurrent laryngeal nerve resected under uniportal video-assisted thoracic surgery.

Mediastinal neurogenic tumours are mostly derived from sympathetic nerves and intercostal nerves, and vagus nerve-derived schwannomas are rare. We encountered a tumour originating from the origin of the recurrent laryngeal nerve that was accompanied by the azygos lobe, which made it difficult to approach; it was ultimately able to be removed via uniportal video-assisted thoracic surgery. This case involved a 63-year-old female patient. There were no particular symptoms, but an abnormal chest shadow was noted on an imaging examination. Chest imaging revealed a smooth-surfaced mass in the upper

Analgesic Efficacy of Intravenous Dexamethasone as an Adjunct to Ultrasound-Guided Paravertebral Block with Bupivacaine in Video-Assisted Thoracoscopic Surgery.

PURPOSE: Thoracic paravertebral block (TPVB) is a recommended regional analgesia during video-assisted thoracoscopic surgery (VATS). However, single-injection TPVB does not last long enough to provide sufficient acute postoperative pain relief. Continuous TPVB through a catheter is technically challenging and often unreliable. Intravenous dexamethasone extends the analgesic duration with some peripheral nerve blocks. However, data on the effect of intravenous dexamethasone on pain relief with TPVB are limited. This study aimed to assess the analgesic efficacy of intravenous dexamethasone in pa

Uniportal video-assisted thoracic surgery lobectomy in a patient with carcinoid tumor located at the orifice of the right lower lobe.

We present here a 47-year old male patient who had a typical carcinoid tumor located at the orifice of right lower lobe bronchus underwent uniportal thoracoscopic lower lobectomy following bronchoscopic removal of the endobronchial tumor. Tumor was seen by bronchoscopy to protrude out from the lower lobe into the intermediary bronchus obstructing the lower lobe completely and the middle lob partially. However, the origin of the endobronchial tumor was in the distal part of the lower lobe bronchus. In this case, open surgery could be an alternative to save the middle lobe by incising the lower

A Retrospective Analysis on Level of Suction in Digital Drainage Devices After Video-assisted Lobectomy in a Thoracic Surgery Centre.

INTRODUCTION: The management of chest tubes after pulmonary resection remains non- standardized, and suction levels are often determined by the surgeon's preference. This retrospective study aimed to compare the clinical outcomes of low suction -2cmH2O-2cmH2O versus the conventional suction level used in our institution -15cmH2O-15cmH2O using digital drainage devices after videoassisted thoracic surgery (VATS) lobectomy for suspected or confirmed lung cancer in a thoracic surgery centre. METHODS: We analysed 120 patients who underwent pleural drainage after VATS lobectomy between January 20
