Delayed endolymphatic hydrops: a case study.

Delayed endolymphatic hydrops (DEH) is an unusual variation of Ménière's disease characterized by episodic vertigo that develops some time after the onset of a profound, typically unilateral sensorineural hearing loss. This case study describes a 48-year-old male who presented with complaints of episodic vertigo and disequilibrium 15 years following the onset of unilateral sensorineural hearing loss. The patient's history, audiologic findings, and vestibular evaluation led to the diagnosis of DEH. The case highlights the diagnostic and treatment challenges associated with this condition and fo

Periodontal health, quality of life, and smiling patterns--an exploration.

BACKGROUND: This study explored whether periodontal health/disease affects psychosocial outcomes in smiling patterns of particular subjects and their smile-related quality of life. METHODS: We collected data from 21 regularly scheduled patients in a periodontal graduate student clinic (four males and 17 females; average age: 50.38 years; age range: 24 to 82 years). The subjects were videotaped while watching a funny television (TV) program. Two independent raters rated each videotape at 31 predetermined time points to assess four aspects of the objective smiling patterns of the subjects. In a

Detecting emotional expression in face-to-face and online breast cancer support groups.

Accurately detecting emotional expression in women with primary breast cancer participating in support groups may be important for therapists and researchers. In 2 small studies (N = 20 and N = 16), the authors examined whether video coding, human text coding, and automated text analysis provided consistent estimates of the level of emotional expression. In Study 1, the authors compared coding from videotapes and text transcripts of face-to-face groups. In Study 2, the authors examined transcripts of online synchronous groups. The authors found that human text coding significantly overestimate

Phenomenology of abnormal movements in stuttering.

BACKGROUND: Stuttering is often accompanied by involuntary movements, abnormal gestures or changes in facial expression. OBJECTIVE: To describe the incidence and phenomenology of abnormal movements (AMs) in stuttering patients. MATERIALS AND METHODS: Eighty-five consecutive patients with stuttering and 119 normal controls videotaped and subsequently reviewed, in which AMs were classified as voluntary or involuntary, and as concurrent or unrelated to speech. Movement phenomenology was correlated with disease severity. RESULTS: Of 85 stuttering patients studied, 51.7% had AMs and 22 more than

Tic suppression in the treatment of Tourette's syndrome with exposure therapy: the rebound phenomenon reconsidered.

Exposure and response prevention (ER), a behavioral treatment program consisting of exposure to premonitory sensory experiences during prolonged tic suppression, was shown to be a promising new treatment for tics in Tourette's syndrome (TS). In this study, the commonly reported paradoxical increase in tic frequency following voluntary tic suppression, i.e., rebound phenomenon, was examined. Tic frequency was rated in 20 TS patients during 15-minute videotaped conversations taken both before and following 10 ER sessions. In addition, tic frequency was obtained at home by family members of the p

Cognitive mechanisms of social anxiety reduction: an examination of specificity and temporality.

Cognitive theories posit that exposure-based treatments exert their effect on social anxiety by modifying judgmental biases. The present study provides a conservative test of the relative roles of changes in judgmental biases in governing social anxiety reduction and addresses several limitations of previous research. Longitudinal, within-subjects analysis of data from 53 adults with a Diagnostic and Statistical Manual of Mental Disorders (4th ed.; American Psychiatric Association, 1994) social phobia diagnosis revealed that reductions in probability and cost biases accounted for significant v

Risk factors for psychogenic nonepileptic seizures in children and adolescents with epilepsy.

There is evidence that psychogenic nonepileptic seizures (PNES) remain underdiagnosed, especially in children and adolescents. Diagnosis of such events is even more difficult in patients that do have epilepsy, leading to delayed diagnosis and treatment and, consequently, iatrogenic complications. This study aimed to evaluate possible risk factors in children with epilepsy who had PNES. Seizures and epileptic syndromes were classified according to International League Against Epilepsy guidelines. Patients were evaluated with a structured psychiatric anamnesis and classified according to the Dia

A PET provocation study of generalized social phobia.

In an investigation of the neural circuits that may mediate the subjective experience of social phobia (SP), six male patients with a primary DSM-IV diagnosis of generalized social phobia watched, in the presence of a group of "communication experts," a videotape of themselves giving an impromptu talk (Exposure condition). In the control Baseline condition, they viewed a videotape of a socially competent stranger giving a talk. Regional cerebral blood flow was measured thrice under each condition. The study revealed significant deactivations from Baseline during Exposure in the right lingual g

Lateral neck radiography versus direct video rhinoscopy in assessing adenoid size.

OBJECTIVE: To evaluate the usefulness of adynamic lateral neck radiographs and dynamic video rhinoscopy in assessing adenoid size and the relationship of these methods to associated symptoms and thus the severity of the disease. METHODS: Children with suspected adenoid hypertrophy underwent standard lateral neck soft tissue radiographs: the percentage of airway occlusion, adenoid to nasopharynx (AN) ratio, airway to soft palate ratio, and adenoid thickness were assessed by a radiologist. The percentage of airway closure was assessed by direct fibre-optic rhinoscopy in an ear, nose, and throat

Validation of a pain evaluation scale for patients with severe cerebral palsy.

Polyhandicapped children are extremely limited by severe mental retardation, motor deficiency, perception disability and expression troubles. So pain is very difficult to assess in these patients. We have developed a specific scale for rapid evaluation of pain based on the observation of their behaviour. A first questionnaire listing 22 items was elaborated by physicians and nurses on the basis of their clinical experience. The first part of our study was designed to select the most pertinent items in order to define a final scale for simple and reliable everyday use. The second part of our st

Two-dimensional analysis of vocal fold vibration in unilaterally atrophied larynges.

OBJECTIVE: Considerable inconsistencies regarding the vibratory pattern of the vocal fold among patients with unilateral vocal fold paralysis (UVFP) have been reported. These differences are derived from differences in the position, stiffness, and atrophy of the paralyzed vocal fold and other factors among patients. The purpose of this study was to assess the effects of unilateral atrophy of the vocal fold on vocal fold vibration. METHODS: Seven excised canine larynges were studied. The unilateral recurrent laryngeal nerve was severed to cause vocal fold atrophy in four of the seven. The late

Typical absence status in adults: diagnostic and syndromic considerations.

PURPOSE: To study the electroclinical features of typical absence status (TAS) in adults with syndromes of idiopathic generalized epilepsies (IGEs). METHODS: Twenty-one patients with one or more spells of TAS were identified among 136 consecutive adult patients with IGEs. All patients with TAS had comprehensive electroclinical investigations and EEG or video-EEG recorded absences. RESULTS: TAS occurred in 24.4% of 86 patients who had IGEs with typical absences alone or in combination with other seizures presisting in adult life. The prevalence of TAS appeared to be syndrome related, ranging

Observer agreement about laryngoscopic assessment of papilloma.

BACKGROUND: Observer bias may confound the assessment of therapies utilized for recurrent respiratory papilloma, a recidivistic fluctuating disease. Any convincing change in a patient's disease must exceed the imprecision of the measuring system. MATERIALS AND METHODS: Videotapes of ten children, who had airway endoscopy for care of laryngeal papilloma, were edited. The videotapes were independently reviewed by six otolaryngologists, of whom five repeated their assessments 5 to 20 weeks later. The 'reference standard' was the operating surgeon's categorization of his patient's videotape. Agre

Parents and families of children with communication disorders.

The role of parents in the development of the communicative competence of their children has been of growing interest to researchers and practitioners in the field of speech and language therapy. Researchers have attempted to define and test specific hypotheses regarding the nature and degree of the influence of parental interaction styles upon the developing child's pragmatic skills. The literature includes studies regarding normal children and those with speech and language impairments. Practitioners have endeavoured to move towards a firmer framework of partnership with parents which will u

Glottic configuration after arytenoid adduction.

It has been recently noted that laryngeal paralysis results in a complex alteration of the glottis. The membranous segment of the paralyzed vocal fold is shortened, and, during phonation, patients use hyperfunction to shorten the normal vocal fold to about the same length. Additionally, if the paralyzed vocal fold is not near the midline, the angle between the membranous and cartilaginous segments of the vocal fold is decreased, resulting in a "posterior" gap which cannot be closed by hyperadduction of the normal side. To determine whether arytenoid adduction addresses these problems, videolar
