Brief report: childhood disintegrative disorder as a likely manifestation of vitamin B12 deficiency.

Childhood disintegrative disorder is a rare disorder, characterized by regression of acquired skills after a period of normal development. The case of childhood disintegrative disorder presented here was found to have vitamin B12 deficiency and hyperhomocysteinemia on extensive evaluation to find a probable cause for regression. This case illustrates the need for a thorough evaluation of all cases of childhood disintegrative disorder so that treatable causes of regression, like vitamin B12 deficiency, are not missed.

Cobalamin deficiency.

Cobalamin (Cbl, vitamin B12) consists of a corrinoid structure with cobalt in the centre of the molecule. Neither humans nor animals are able to synthesize this vitamin. Foods of animal source are the only natural source of cobalamin in human diet. There are only two enzymatic reactions in mammalian cells that require cobalamin as cofactor. Methylcobolamin is a cofactor for methionine synthase. The enzyme methylmalonyl-CoA-mutase requires adenosylcobalamin as a cofactor. Therefore, serum concentrations of homocysteine (tHcy) and methylmalonic acid (MMA) will increase in cobalamin deficiency. T

The prevalence of hypocobalaminaemia in cats with spontaneous hyperthyroidism.

OBJECTIVES: To determine the prevalence of hypocobalaminaemia in cats with moderate to severe hyperthyroidism and to investigate the relationship between cobalamin status and selected haematologic parameters. METHODS: Serum cobalamin concentrations were measured in 76 spontaneously hyperthyroid cats [serum thyroxine (T(4) ) concentration ≥100 nmol/L] and 100 geriatric euthyroid cats. Erythrocyte and neutrophil counts in hyperthyroid cats with hypocobalaminaemia were compared with those in hyperthyroid cats with adequate serum cobalamin concentrations (≥290 ng/L). RESULTS: The median cobalami

Diagnostic performance of serum cobalamin tests: a systematic review and meta-analysis.

AIMS: Serum cobalamin (cbl, vitamin B(12)) tests are routinely ordered for investigating conditions potentially amenable to cbl supplementation. This study aimed to systematically assess the evidence of diagnostic accuracy for serum cbl tests across patient subgroups. METHODS: Seven medical databases were searched (1990 to November 2009). Studies were included that compared serum cbl to a reference standard (all reference standards employed). Study quality was assessed using QUADAS. Summary estimates of test performance were determined using the bivariate model and hierarchical summary receiv

Occurrence of pseudovitamin B12 and its possible function as the cofactor of cobalamin-dependent methionine synthase in a cyanobacterium Synechocystis sp. PCC6803.

To clarify the physiological function of pseudovitamin B(12) in cyanobacteria, we determined pseudovitamin B(12) contents and cobalamin-dependent methionine synthase activity in Synechocystis sp. PCC6803 grown under CoSO(4)-sufficient and -limited conditions. Pseudovitamin B(12) and cobalamin-dependent methionine synthase activity (0.8 nmol/min/mg protein) were found in a homogenate of the Synechocystis cells grown for 10 d in the CoSO(4)-sufficient medium. The cellular pseudovitamin B(12 )contents increased significantly at the early logarithmic growth phase and thereafter decreased rapidly a

Influence of cobalamin deficiency compared with that of cobalamin absorption on serum holo-transcobalamin II.

BACKGROUND: Cobalamin attached to transcobalamin II (TC II), known as holo-TC II, is the active cobalamin fraction taken up by tissues. Holo-TC II is also the form in which absorbed cobalamin enters the circulation from the ileum. Therefore, holo-TC II has been proposed variously as a marker of cobalamin adequacy, cobalamin absorption, or both, including even its advocacy as a surrogate Schilling test. Such claims carry conflicting diagnostic implications because metabolic adequacy and absorption are not identical. OBJECTIVE: The objective was to examine metabolic and absorptive influences on

Update on cobalamin, folate, and homocysteine.

Three topics affecting cobalamin, folate, and homocysteine that have generated interest, activity, and advances in recent years are discussed. These are: (I). the application of an expanded variety of tools to the diagnosis of cobalamin deficiency, and how these affect and are affected by our current understanding of deficiency; (II). the nature of the interaction between homocysteine and vascular disease, and how the relationship is affected by vitamins; and (III). the improved understanding of relevant genetic disorders and common genetic polymorphisms, and how these interact with environmen

Vitamin B12 deficiency: report of a childhood case.

An 11-year-old boy who presented with anemia, premature grey hair, hyperpigmented skin, paresthesia, recurrent aphthous ulcers and epistaxis was eventually proved to be a case of vitamin B12 deficiency. Due to the paucity of this deficiency, the diagnosis may easily be delayed and overlooked resulting in unfavorable consequences. Therapeutic response to vitamin B12 was dramatic in this reported case.

Determination of 'total' and 'true' cobalamin with the SimulTRAC Assay.

The determination of 'total' and 'true' cobalamin (the SimulTRAC Assay, Becton Dickinson) has been compared with other laboratory tests and with clinical symptoms typical of cobalamin deficiency. Patient specimens (n = 5709) were analysed for total cobalamin which was measured by its binding to a crude hog intrinsic factor preparation in the radioisotope assay. Specimens with borderline values (150-225 pmol/l) were reanalysed for true cobalamin in order to find out whether there were patients with cobalamin deficiency and a low true cobalamin content in serum. Of 803 patient sera with total co

An Animal-Source Food Supplement Increases Micronutrient Intakes and Iron Status among Reproductive-Age Women in Rural Vietnam.

Background: Few studies have examined the impact of local animal-source foods (ASFs) on the nutritional status of reproductive-age women in developing countries. Objective: We hypothesized that a midmorning snack of local ASF for 6 mo would reduce dietary micronutrient deficiencies [usual intake less than the estimated average requirement (EAR)] and improve blood biomarkers of iron, zinc, and vitamins A and B-12 status among nonpregnant, reproductive-age women in rural Vietnam. Methods: One hundred seventeen women, 18-30 y old, were randomly assigned to receive either an ASF (mean: 144 kcal, 8.9

Association of MTHFR and RFC1 gene polymorphism with hyperhomocysteinemia and its modulation by vitamin B12 and folic acid in an Indian population.

BACKGROUND/OBJECTIVES: Unlike most Western populations, MTHFR 677T is a rare allele and a risk factor for a variety of disorders in India. What kind of nutritional (environmental) and/or genetic factors could contribute to the genetic risk is not known. To assess the incidence of hyperhomocysteinemia and its correlation with the polymorphism in homocysteine (Hcy)-pathway genes and associated cofactors in the native populations of eastern India. SUBJECTS/METHODS: Healthy population from four eastern states of India. Genotyping of SNPs, HPLC and chemiluminescence-based assay for homocysteine, v

OBJECTIVES: To explore the association between vitamin B12 deficiency and dementia in patients at high risk for vitamin B12 deficiency. DESIGN: Chart review. SETTING: Emergency, critical care/ trauma, neurology, medicine, and rehabilitation units of two hospitals in Southwestern Ontario, Canada. PARTICIPANTS: Adult patients (n = 666) admitted from 2010 to 2012. Data collection included: reason for admission, gender, age, clinical signs and symptoms of B12 deficiency, serum B12 concentration, and B12 supplementation. Patients with dementia were identified based on their medication profile an
