A rare association of twenty-nail dystrophy with segmental vitiligo is described in two patients. Vitiligo preceded the nail dystrophy. In both cases, all twenty nails were uniformly affected with the nail plates showing longitudinal striations and loss of luster. Longitudinal nail biopsy revealed a histological picture suggestive of eczematous changes and lichen planus respectively. Intramatrix injections of triamcinolone acetonide into the proximal and lateral nail folds were administered with considerable improvement.

We report a case of vitiligo notable for a degree and pattern of lymphocytic inflammation. Hypopigmentation and subsequent depigmentation developed in a 65-year-old man. Initial skin biopsy specimen from the border of a patch of hypopigmentation revealed a moderately dense perivascular and interstitial infiltrate of lymphocytes with exocytosis into epidermis and follicular epithelium. The pattern of the infiltrate suggested the diagnosis of cutaneous T-cell lymphoma or connective-tissue disease. Fifty native Omanis with vitiligo were studied to compare the incidence of HLA ABC and DR antigens with a control population. HLA Bw6 was found in 82% of patients compared with 49% controls (Pc = 0.0009 RR = 4.56) and HLA DR7 occurred in 40% of patients and 9% in controls (Pc = 0.00075 RR = 6.17).
