Sleep-Wake Disorders After Traumatic Brain Injury: Pathophysiology, Clinical Management, and Future.

After experiencing a traumatic brain injury (TBI), the majority of patients will develop sleep-wake disorders (SWD). These can include insomnia, posttraumatic pleiosomnia (increased sleep need), excessive daytime sleepiness (EDS), obstructive and/or central sleep apnea, circadian SWD, and a variety of parasomnias. Untreated SWD may impede the recovery process and can negatively impact mood, metabolic health, cognitive function, and immune function among other processes. Importantly, these patients tend to misperceive their posttraumatic sleep problems. Consequently, interviews performed in sta

Is there a mental health wake following the COVID-19 pandemic on adolescents in a male juvenile prison?

Lingering effects of the COVID-19 pandemic loom large in the United States but are particularly of concern in prison settings. In the current study, we examine the ongoing mental health consequences of the pandemic among young people incarcerated in a male juvenile prison. Sequential mixed methods data were obtained-78 young people assigned male at birth completed surveys and 19 completed individual interviews. Seven staff were also interviewed about youth experiences during and following the pandemic. Youth survey results indicated racial and ethnic identity exploration, feeling less safe at

Comparative analysis among different alternative fuels for ship propulsion in a well-to-wake perspective.

The shipping sector is required to give a significant contribution to the reduction of Green House Gas (GHG) emissions, according to the ambitious goals fixed by the International Maritime Organization (IMO). To achieve these targets, new technologies and measures are required, related to logistics, digitalization, hydrodynamics, machinery, energy, and aftertreatment. A large potential to reduce GHG emissions is offered by alternative fuels. In this perspective a Well-to-Wake (WtW) approach is due for a comprehensive analysis. The paper is focused on the evaluation of WtW CO2 equivalent emissi

Iron Deficiency and Restless Sleep/Wake Behaviors in Neurodevelopmental Disorders and Mental Health Conditions.

Iron deficiency (ID) and restlessness are associated with sleep/wake-disorders (e.g., restless legs syndrome (RLS)) and neurodevelopmental disorders (attention deficit/hyperactivity and autism spectrum disorders (ADHD; ASD)). However, a standardized approach to assessing ID and restlessness is missing. We reviewed iron status and family sleep/ID history data collected at a sleep/wake behavior clinic under a quality improvement/quality assurance project. Restlessness was explored through patient and parental narratives and a 'suggested clinical immobilization test'. Of 199 patients, 94% had ID,

Validating CircaCP: a generic sleep-wake cycle detection algorithm for unlabelled actigraphy data.

Sleep-wake (SW) cycle detection is a key step for extracting temporal sleep metrics from actigraphy. Various supervised learning algorithms have been developed, yet their generalizability from sensor to sensor or study to study is questionable. In this paper, we detail and validate an unsupervised algorithm-CircaCP-for detecting SW cycles from actigraphy. It first uses a robust cosinor model to estimate circadian rhythm, then searches for a single change point (CP) within each circadian cycle. Using CircaCP, we estimated sleep/wake onset times (S/WOTs) from 2125 individuals' data in the MESA s

Deciphering Complexity: Atypical Hemolytic Uremic Syndrome Unraveled in the Wake of Elective Hip Arthroplasty.

Atypical hemolytic uremic syndrome (aHUS) is a rare and complex disease characterized by microangiopathic hemolytic anemia, thrombocytopenia, and acute renal failure. This case report details the clinical presentation, diagnosis, and management of a 49-year-old female who developed aHUS following elective hip arthroplasty. The patient, with a history of cardiovascular events and no prior renal disease, presented with elevated LDH levels, thrombocytopenia, and acute renal failure on the first postoperative day. A diagnostic workup confirmed aHUS, and the patient was successfully treated with th

Real world evidence supports waking salivary cortisone as a screening test for adrenal insufficiency.

OBJECTIVE: Worldwide, adults and children are at risk of adrenal insufficiency largely due to infectious diseases and adrenal suppression from use of anti-inflammatory glucocorticoids. Home waking salivary cortisone is an accurate screening test for adrenal insufficiency, it has potential to reduce costs, and patients prefer it to the adrenocorticotropin (ACTH) (synacthen) stimulation test. We carried out a service evaluation of home waking salivary cortisone in clinical care to identify implementation barriers. DESIGN, PATIENTS AND MEASUREMENTS: Service evaluation in a centre where 212 patie

Machine Learning Identifies a Rat Model of Parkinson's Disease via Sleep-Wake Electroencephalogram.

Alpha-synuclein induced degeneration of the midbrain substantia nigra pars compact (SNc) dopaminergic neurons causes Parkinson's disease (PD). Rodent studies demonstrate that nigrostriatal dopamine stimulates pallidal neurons which, via the topographical pallidocortical pathway, regulate cortical activity and functions. We hypothesize that nigrostriatal dopamine acting at the basal ganglia regulates cortical activity in sleep and wake state, and its depletion systemically alters electroencephalogram (EEG) across frequencies during sleep-wake state. Compared to control rats, 6-hydroxydopamine i

Narcolepsy: a model interaction between immune system, nervous system, and sleep-wake regulation.

Narcolepsy is a rare chronic neurological disorder characterized by an irresistible excessive daytime sleepiness and cataplexy. The disease is considered to be the result of the selective disruption of neuronal cells in the lateral hypothalamus expressing the neuropeptide hypocretin, which controls the sleep-wake cycle. Diagnosis and management of narcolepsy represent still a substantial medical challenge due to the large heterogeneity in the clinical manifestation of the disease as well as to the lack of understanding of the underlying pathophysiological mechanisms. However, significant advan

Traumatic Brain Injury Characteristics Predictive of Subsequent Sleep-Wake Disturbances in Pediatric Patients.

The objective of this study was to determine the prevalence of sleep-wake disturbances (SWD) following pediatric traumatic brain injury (TBI), and to examine characteristics of TBI and patient demographics that might be predictive of subsequent SWD development. This single-institution retrospective study included patients diagnosed with a TBI during 2008-2019 who also had a subsequent diagnosis of an SWD. Data were collected using ICD-9/10 codes for 207 patients and included the following: age at initial TBI, gender, TBI severity, number of TBIs diagnosed prior to SWD diagnosis, type of SWD, a

First-episode psychotic disorders in the wake of the COVID-19 pandemic: a descriptive review of casereports.

OBJECTIVE: Since the onset of COVID-19 pandemic, many case reports and case series dealt with new-onset psychotic disorders in patients either infected with SARS-CoV-2 or thematically linked to the pandemic, but without an infection. Our aim was to provide a comprehensive collection of these reports to illustrate the nature of these psychoses. METHODS: We conducted a literature search in MEDLINE, Embase, PsycINFO, using search terms regarding first-episode psychotic disorders in the context of corona. RESULTS: 96 case reports or case series covering 146 patients (62 without and 84 with SARS-

Wake-up strokes are linked to obstructive sleep apnea and worse early functional outcome.

BACKGROUND AND AIMS: Presence of sleep-disordered breathing (SDB) and especially obstructive sleep apnea (OSA) is a known risk factor for ischemic stroke. Additionally, SDB effects negatively on recovery after stroke. Up to one fourth of strokes are present on awakening. The link between OSA and wake-up stroke (WUS) has been suggested. We aim to determine the association between OSA and WUS in a Finnish stroke unit cohort. MATERIAL AND METHODS: An observational prospective longitudinal study consisted of 95 TIA (transient ischemic attack) and mild to moderate stroke patients referred to a Str

Pharmacologically Induced Ventilatory Depression in the Postoperative Patient: A Sleep-Wake State-Dependent Perspective.

Pharmacologically induced ventilatory depression (PIVD) is a common postoperative complication with a spectrum of severity ranging from mild hypoventilation to severe ventilatory depression, potentially leading to anoxic brain injury and death. Recent studies, using continuous monitoring technologies, have revealed alarming rates of previously undetected severe episodes of postoperative ventilatory depression, rendering the recognition of such episodes by the standard intermittent assessment practice, quite problematic. This imprecise description of the epidemiologic landscape of PIVD has thus

A decade of drug-resistant tuberculous meningitis: A wake-up call for patient-centric therapy.

On analyzing the drug susceptibility profile of 151 clinical isolates collected from patients of tuberculous meningitis (TBM) over 10 years, we reflect on few lessons learnt from the trend of susceptibility profile - drug resistance was not uncommon, fluoroquinolone resistance was observed even among otherwise susceptible isolates and hetero-resistance was observed against rifampicin, isoniazid and also fluoroquinolones. In the midst of widening gap between incidence of drug resistant TBM and availability of effective drugs, our data suggests that universal testing for drug resistance, careful

Light-based methods for predicting circadian phase in delayed sleep-wake phase disorder.

Methods for predicting circadian phase have been developed for healthy individuals. It is unknown whether these methods generalize to clinical populations, such as delayed sleep-wake phase disorder (DSWPD), where circadian timing is associated with functional outcomes. This study evaluated two methods for predicting dim light melatonin onset (DLMO) in 154 DSWPD patients using ~ 7 days of sleep-wake and light data: a dynamic model and a statistical model. The dynamic model has been validated in healthy individuals under both laboratory and field conditions. The statistical model was developed f
