[Keratomalacia in rheumatoid arthritis: immunohistologic and enzyme histochemical studies].

Corneal and conjunctival biopsies of 13 patients with rheumatoid arthritis and corneal ulceration (RA-keratomalacia) have been characterized by immunohistological and histochemical analysis. Biopsies from 13 patients with bacterial conjunctivitis, 7 patients with allergic conjunctivitis, 15 patients with senile cataract and 15 patients with keratokonus served as controls. The phenotypic composition of the conjunctival inflammatory infiltration of rheumatoid corneal ulceration was not significantly different from the other inflammatory eye diseases studied. However, conjunctival epithelial cell

Synovitis in angioimmunoblastic lymphadenopathy with dysproteinemia simulating rheumatoid arthritis.

We describe a patient with angioimmunoblastic lymphadenopathy with dysproteinemia who developed a symmetric, rheumatoid-like, peripheral polyarthritis. Radiographs of the involved joints revealed soft tissue swelling without erosions or cartilage loss. Rheumatoid factor and fluorescent antinuclear antibodies were negative, and C-reactive protein and erythrocyte sedimentation rate were normal. Synovial fluid analysis showed an inflammatory effusion (white blood cell count of 3,500/mm3, with 76% polymorphonuclear leukocytes). A closed synovial biopsy of the wrist revealed a mononuclear infiltrat

Middle ear function in patients with juvenile chronic arthritis.

The conductive hearing of 18 patients with juvenile chronic arthritis was studied. Tympanometry was therefore undertaken for the assessment of functional state of the tympanic membrane, the ossicular chain with its ligaments and muscles, and the air cushion of the tympanic cavity. Acoustic reflex was present in all patients but an abnormal tympanometric pattern (type As) was found bilaterally or unilaterally in 10 (mostly severe cases) patients. These findings indicate that juvenile chronic arthritis changes middle ear function, probably as a result of inflammation of the synovial ossicular jo

Abnormal T suppressor cell function in juvenile rheumatoid arthritis.

The purpose of this study was to analyze T suppressor cell function in juvenile rheumatoid arthritis (JRA). JRA is a chronic inflammatory childhood disease of unknown etiology that is characterized by arthritis and immunoregulatory abnormalities. T suppressor cell precursors (CD8+, CD28-) were purified from the peripheral blood of 24 JRA patients, using a combination of monoclonal antibodies. These cells were treated with histamine or concanavalin A, agents that are known to induce suppressor activity. They were also tested for their ability to inhibit the proliferative response of autologous

Clonal dominance among synovial tissue-infiltrating lymphocytes in arthritis.

T-cell receptor gene rearrangement using a C beta probe was evaluated in 12 patients with rheumatoid arthritis, 2 with juvenile rheumatoid arthritis, and 1 with systemic lupus erythematosus, and in all the samples a dominant T-cell receptor gene rearrangement was noted. In rheumatoid arthritis identical T-cell receptor gene rearrangement was noted in freshly isolated synovial tissue-infiltrating lymphocytes (TIL) and the corresponding interleukin 2-propagated culture. TIL from five different joints obtained from one rheumatoid arthritis patient shared one dominant band, and TIL from three join

Intravenous gamma globulin therapy in systemic juvenile rheumatoid arthritis.

Intravenous (IV) gamma globulin has been successfully used as replacement therapy for antibody-deficient patients and, more recently, in the treatment of autoimmune diseases such as idiopathic thrombocytopenic purpura, myasthenia gravis, and Kawasaki disease. In view of the successful treatment of these diseases, we initiated a pilot study of the effect of IV gamma globulin in systemic juvenile rheumatoid arthritis (JRA). Eight patients with active systemic JRA that was unresponsive to first-line agents, second-line agents, and/or corticosteroids received this therapy monthly for 6 months. Out

Total hip and knee arthroplasty in juvenile rheumatoid arthritis.

Total hip or knee arthroplasty is indicated in patients with juvenile rheumatoid arthritis when there is marked functional impairment and/or severe disabling pain from advanced structural hip or knee joint involvement. Relief of pain and dramatic improvement in function can be achieved in most patients. When both the hip and knee are involved, hip arthroplasty should probably be done first. Regional anesthesia is preferable. Careful preoperative planning is essential because custom prostheses are often required. Small bone size, osteoporosis, and soft-tissue contractures make the surgery techn

[Juvenile ankylosing spondylitis (JAS) and juvenile rheumatoid arthritis (JRA)].

The diagnosis of JAS in 19 of the 24 patients with oligo-articular and the polyarticular subtypes of JRA established primarily was reexamined. Coexistence of JAS and rheumatoid arthritis was found in 4 of the 19 patients with JAS. Of the 19 patients with JAS, 18 were male and 1 was female. The mean age of onset of the disease was 12.6 years (ranging from 8 to 16). Peripheral arthritis was the first symptom in all the 19 patients, predominantly in the joints of knee, hip and ankle. 69% of the 13 patients with hip involvement developed deformity. Twelve patients had lumbosacral pain. Arthritis o

Seronegative rheumatoid arthritis: a clinical study with HLA typing.

We examined both clinically and by determining HLA-A, -B, -C and -DR antigens 50 patients thought to have seronegative erosive polyarticular rheumatoid arthritis (RA) in Finland and the USSR. All the patients fulfilled at least 4 of the 1987 ARA criteria for RA. According to HLA typing and clinical findings, of which a part was collected by followup, the patients fell into 5 groups: HLA-B27 related diseases, putative psoriatic arthritis, putative juvenile chronic polyarthritis, and seropositive and seronegative RA. Our results indicate that most of the patients with seronegative RA had some ot

Primary meningococcal arthritis associated with adult respiratory distress syndrome.

A previously fit and well 20 year old man with primary meningococcal arthritis of the left knee is described. Despite intensive search there was no evidence of extra-articular meningococcal infection. He subsequently developed adult respiratory distress syndrome as a rare and previously undescribed complication.

Intra-articular amphotericin B treatment of Sporothrix schenckii arthritis.

Arthritis caused by Sporothrix schenckii may not respond satisfactorily to a full course of intravenous amphotericin B therapy. Left untreated, the fungus continues to be recovered from cultures of joint fluid, and the patient typically has serious joint disability. We have shown in one patient with sporotrichosis of the knee that direct low-dose injections of amphotericin B can be performed safely, resulting in eradication of the fungus. The patient has had continued useful range of motion and weight bearing on the involved knee.

Natural history of atlanto-axial subluxation in rheumatoid arthritis.

In 1969 the authors started their prospective study to determine the natural history of atlanto-axial subluxation in patients suffering from rheumatoid arthritis. Between January 1969 and July 1971 they accumulated 41 cases of atlanto-axial subluxation. These patients were studied clinically and radiologically, and the initial findings were published in the Journal of Bone and Joint Surgery, Volume 55B, August 1973. All of these patients were followed clinically and radiologically until 1981 or until their demise. Roentgenographically, 61% have remained unchanged, 27% have shown progression of

Benign polyarthritis and undifferentiated arthritis an epidemiological terra incognita.

Introduction of new diagnostic criteria and the development of HLA-typing and microbiological--serological techniques have aided in classifying and differentiating inflammatory rheumatic diseases. Reviewing epidemiological prevalence studies shows that inflammatory rheumatic diseases are often so called benign polyarthritides and that arthritides, classified as definite and probable rheumatoid arthritis (RA), may belong to a group of benign or undifferentiated arthritides. Own data show that only 25% of patients of an early synovitis out-patient clinic have definite or probable RA, 25% have an

Ultrastructural study of articular cartilage in experimental pyogenic arthritis.

Ultrastructural changes of the articular cartilage at the early stage of experimental pyogenic arthritis were studied by electron microscopy. The superficial zone of the cartilage at the site of the cartilage-synovium junction demonstrated far more changes than those at the site of the free surface of the articular cartilage. Changes were observed at the cartilage-synovium junction as early as four hours after the intraarticular injection of the infecting organism. The most interesting finding was migration of erythrocytes and polymorphonuclear leucocytes into the superficial zone of the carti

Family variables and disease management in juvenile rheumatoid arthritis.

Examined the relationship between family functioning and regimen knowledge, medication compliance, and disease activity for children with juvenile rheumatoid arthritis. A strong relationship was found across several measures of family functioning and a self-monitoring index of medication compliance. Family functioning was not related to regimen knowledge or disease activity. The implications of these findings for the continued study of chronically ill children are discussed.

[Broncho-alveolar lavage in pulmonary involvement in rheumatoid arthritis].

This study was designed to investigate by bronchoalveolar lavage (BAL) the characteristics of lung involvement in rheumatoid arthritis (RA). Twenty six patients with RA were included; ten were active smokers. Chest X Ray, pulmonary function tests and BAL were performed in all patients. Bronchiolitis was found in 7% of cases, pleurisy in 30% of cases. 83% of patients were found to have diffuse alveolar and/or interstitial disease, 17% rheumatoid nodules. Functional evaluation showed a restrictive pattern and a significant reduction in the carbon monoxyde diffusing capacity. Smokers had elevated

Rheumatoid arthritis: an editorial perspective based on cytokine imbalance.

Rheumatoid arthritis (RA), systemic lupus erythematosus (SLE) and Sjögren's syndrome (SS) are distinct systemic rheumatic and autoimmune diseases with overlapping clinical features and laboratory findings. Although the majority of patients fit the textbook descriptions of these disorders, there are occasional patients whose illness defies precise diagnostic classification. Examples are mixed connective tissue disease, rheumatoid arthritis and systemic vasculitis, and the overlap SS/SLE syndrome with anti-Ro autoantibodies [1]. Cytokine abnormalities are prominent in all rheumatic diseases. Thi
