One manifestation of cystic fibrosis (CF) is the presence of a viscid mucus secretion in the lungs. Bronchial injury due to blunt chest trauma is rare, and its rarity and the fact that it has two distinct modes of presentation may considerably delay diagnosis. Two recent cases illustrate the two main types of injury and presentation. In the first the rupture is intrapleural and air escapes into the pleural space; insertion of a chest drain leads to a continuous air leak. In the second type the rupture is largely extrapleural with little communication with the pleural cavity; initially symptoms may be mild or absent but complications may occur later.

Thirty-nine paraffin-embedded transbronchial biopsies from ten lung transplant recipients were evaluated by the polymerase chain reaction for the presence of Epstein-Barr virus (EBV). Although the characteristics of the histopathologic changes present in subjects who die with status asthmaticus are well documented, the structural changes present in subjects with mild to moderately severe asthma are not well described and the inflammatory changes in the large airways of subjects with chronic airflow limitation (CAL) and asthma have not been compared. Ten subjects with asthma, five taking inhaled corticosteroids and five taking beta 2-agonist aerosols, five subjects with CAL, and four subjects with no respiratory illness had four biopsies taken from airways 10 mm in diameter.

We report the clinical picture and the outcome after medical and surgical treatment of eleven patients who had a traumatic rupture of the tracheobronchial tree between 1971 and 1990. The lesions occurred after a blunt chest trauma. All patients but one had evidence of extrapulmonary air leak on admission; one patient complained of hemoptysis. We report a case of PAP which proved to be fatal despite whole lung lavage. The need for early BAL and transbronchial biopsies in diffuse infiltrative lung disorders of unknown etiology is highlighted. The occurrence of PAP in association with Fanconi's anemia and psoriasis raises the possibility of a common pathogenetic defect which may be related to abnormal cytokine metabolism. Investigation of cytokine metabolism in PAP is warranted.

Physiological and Immunopathological parameters were investigated in 15 patients with diagnosed asthma, and 6 non-asthmatics presenting with other chest symptoms. Atopy is often regarded as a risk factor for the development of asthma, particularly childhood asthma and occupational asthma. This could reflect an association with non-specific bronchial responsiveness (NSBR), though atopy could influence asthma independently. Clinical experience during one-lung anesthesia using a new right-sided endobronchial tube (Portex Ltd, Hythe, Kent, UK) is reported in 148 patients with cancer of the left lung.
