Pharmaceutical therapeutics for articular regeneration and restoration: state-of-the-art technology for screening small molecular drugs.

Articular cartilage damage caused by sports injury or osteoarthritis (OA) has gained increased attention as a worldwide health burden. Pharmaceutical treatments are considered cost-effective means of promoting cartilage regeneration, but are limited by their inability to generate sufficient functional chondrocytes and modify disease progression. Small molecular chemical compounds are an abundant source of new pharmaceutical therapeutics for cartilage regeneration, as they have advantages in design, fabrication, and application, and, when used in combination, act as powerful tools for manipulat

Control of skeletal morphogenesis by the Hippo-YAP/TAZ pathway.

The Hippo-YAP/TAZ pathway is an important regulator of tissue growth, but can also control cell fate or tissue morphogenesis. Here, we investigate the function of the Hippo pathway during the development of cartilage, which forms the majority of the skeleton. Previously, YAP was proposed to inhibit skeletal size by repressing chondrocyte proliferation and differentiation. We find that, in vitro, Yap/Taz double knockout impairs murine chondrocyte proliferation, whereas constitutively nuclear nls-YAP5SA accelerates proliferation, in line with the canonical role of this pathway in most tissues. High-Temperature Requirement A1 Protease as a Rate-Limiting Factor in the Development of Osteoarthritis.

Preserving the mature articular cartilage of joints is a critical focus in the prevention and treatment of osteoarthritis. We determined whether the genetic inactivation of high-temperature requirement A1 (HtrA1) can significantly attenuate the degradation of articular or condylar cartilage. Two types of mouse models of osteoarthritis were used, a spontaneous mutant mouse model [type XI collagen-haploinsufficient (Col11a1+/-) mice] and two post-traumatic mouse models [destabilization of the medial meniscus (DMM) on the knee and a partial discectomy (PDE) on the temporomandibular joint]. [Effects of exercise on joints.].

Joints are composed of several different tissues(cartilage, capsule, meniscus, and ligament), and articular cartilage plays an important role in maintaining mechanical competence during exercise. Weight-bearing exercise has several benefit, including improved blood and synovial fluid circulation in a given joint. Consistent moderate activities facilitate cycles of anabolism and catabolism. Mechanical stresses are crucial for the maintenance of the morphologic and functional integrity of articular cartilage. Healthy cartilage is exposed by hydrostatic pressure and tensile strain, when cartilage

Evaluating the effects of material properties of artificial meniscal implant in the human knee joint using finite element analysis.

Artificial meniscal implants may replace severely injured meniscus and restore the normal functionality of the knee joint. Implant material stiffness and shape influence the longevity of implantations. This study, using 3D finite element analysis, aimed to evaluate the effects of material stiffness variations of anatomically shaped artificial meniscal implant in the knee joint. Finite element simulations were conducted on five different cases including intact knee, medial meniscectomized knee, and the knee joint with the meniscal implant with three distinct material stiffness. Cartilage contac

Does increasing applied load lead to contact changes indicative of knee osteoarthritis? A subject-specific FEA study.

This study investigated whether increased loading (representing obesity) in the extended knee and flexed knee led to increased stresses in areas of typical medial and lateral osteoarthritis cartilage lesions, respectively. We created two paired sets of subject-specific finite element models; both sets included models of extended knees and of flexed knees. The first set represented normal loading; the second set represented increased loading. All other variables were held constant. The von Mises stresses and contact areas calculated on the tibial cartilage surfaces of the paired models were the

Rhinosurgery in children: basic concepts.

In newborns, the main supporting structure of the nose is the dorsoseptal cartilage, a T-bar-formed complex of septum and upper lateral cartilages, which is essentially an external extension of the cartilage of the anterior cranial base. Later the anatomic situation gradually changes -- a potential pitfall for surgeons and radiologists. The vulnerability of various processes underlying postnatal development of the facial skeleton is discussed. The cartilaginous septum is the dominant growth center. Loss of septal cartilage at different ages leads to different facial syndromes involving nose, m

Imaging of osteochondral injuries.

Acute injuries can produce fragments consisting of cartilage alone or cartilage and underlying bone. A purely cartilaginous fragment creates no direct radiographic abnormalities, whereas one containing calcified cartilage and bone appears as a radiodensity. The advent and refinement of MR imaging have led to the detection of occult injuries of subchondral bone and cartilage that can escape identification on routine radiographic analysis. The available data appear to indicate a substantial role for MR imaging in the analysis of lesion stability in cases of osteochondritis dissecans; however, fu

Cartilaginous bending spring for preventing tympanic membrane graft medialisation in anterior or subtotal tympanic membrane perforations-how I do it.

BACKGROUND: The reconstruction of anterior or subtotal tympanic membrane perforations is critical due to the risk of anterior graft medialisation and retraction or recurrent perforation. METHOD: After reconstruction of the tympanic membrane by means of grafting, a rectangular cartilage strut (length 6 mm, breadth 2 mm, thickness 0.1 mm) is prepared using a cartilage knife and scalpel. This strut graft is placed between the cartilage graft and the promontory in the anterior inferior part of the middle ear cavity. CONCLUSION: Our experience shows that using a U-shaped cartilage strut to sustai

Staging and comorbidities.

Articular cartilage lesions of the distal femur and patella are common. To provide an accurate diagnosis of a clinically symptomatic cartilage lesion and subsequent appropriate planning for potential treatment options, a proper staging is required. This includes clinical exam, radiographic imaging as well as arthroscopy. Once the staging is completed, other comorbidities may need to be addressed that may require additional surgical procedures. These can either be planned as staged procedures or concomitantly with a cartilage repair procedure. This article will discuss this staging and evaluati

The pathology of canine hip dysplasia.

Although the complete pathogenesis of cartilage damage in canine hip dysplasia has not been completely worked out, it appears that joint laxity with resultant excessive stresses upon articular cartilage is the initiating factor. Grossly, this damage is characterized by cartilage thinning with potential exposure of subchondral bone, osteophyte formation, and pannus formation. Microscopically, there is a marked variation in chondrocyte cellularity, with focal regions of hypocellularity and chondrocyte clusters and synovial lining cell hyperplasia and hypertrophy as well as a decrease in proteogl

Chondrodystrophy in calves associated with manganese deficiency.

A severe congenital chondrodystrophy in Charolais calves is described. Thirty-two calves were born on the same farm with shortened limbs and enlarged joints. Histologically, there was poor cartilage maturation with excessive amounts of rarefied cartilage matrix. There were degenerative changes in the chondrocytes and a severe reduction in the mucopolysaccharide content of all body hyaline cartilage. The cartilage matrix, when examined ultrastructurally, showed loose, disorganised fibres in a rarefied matrix, and extensive vacuolation of the chondrocytes. A low level of manganese was found in t

Surgery of the incompetent nasal valve.

One of the causes of nasal airway obstruction during inspiration is an incompetent nasal valve. An incompetent valve is one that collapses during quiet normal breathing due to narrowing of the valve area, a loss of upper and/or lower lateral cartilage support, or a combination of these. The most common etiology is a previous rhinoplasty; trauma and aging are other causes. Treatment is surgical and consists of replacing what is missing with similar tissue. If only the vestibular skin is scarred, a skin graft is used. If cartilage alone is missing, a cartilage graft is inserted. If both skin and

Legg-Calvé-Perthes (L.C.P.) disease in the light of recent findings.

In Legg-Calvé-Perthes (L.C.P.) disease, histological and histochemical investigations have revealed, in the proximal epiphysis of the femur, areas of abnormal cartilage containing nutrient vessels for the ossific nucleus which appeared to be obliterated. The epiphyseal cartilage also exhibited different histochemical characteristics from normal cartilage and appeared to be more attenuated. These observations lend further support to Ponseti's hypothesis (1956), namely that necrosis of the cephalic nucleus is only one of the anomalies of the cartilage of the proximal femoral epiphysis. In the li

Microfluidics-Enabled Nanoparticle for Multiscale Synergetic Osteoarthritis Therapy.

Osteoarthritis (OA) is a complex degenerative joint disease perpetuated by a vicious cycle of biomechanical, biochemical, and chondrocyte homeostasis imbalances, ultimately leading to progressive cartilage degradation. A simultaneous approach to biomechanical disturbances, chondrocyte metabolic imbalances, and maintaining cartilage homeostasis simultaneously holds promise in tackling this challenge. Herein, we report a microfluidics-enabled nanoparticle, termed ZCMC, formed by grafting a magnesium-modified chondroitin sulfate polymer layer onto a zein core preloaded with cannabidiol (CBD), for

Metal-Organic Framework-Integrated Nanoplatform Orchestrates Osteochondral-Synovial Homeostasis for Osteoarthritis Therapy.

Osteoarthritis (OA) remains a formidable clinical challenge due to the intricate interplay of cartilage degradation, synovitis, and subchondral bone remodeling. Clinical intra-articular therapies are hindered by rapid drug clearance, inadequate cartilage penetration, and a lack of strategies targeting multifactorial pathogenesis. Herein, we engineered a hybrid nanoplatform for multiaction therapeutics by integrating boundary lubrication and "osteochondral-synovial synergistic effect." The kartogenin (KGN)-loaded Mg/Zn-based metal-organic framework (Mg-ZIF) core, encapsulated within a cartilage

99mTc-NTP 15-5 is a companion radiotracer for assessing joint functional response to sprifermin (rhFGF-18) in a murine osteoarthritis model.

With the emergence of disease modifying osteoarthritis drugs (DMOAD), imaging methods to quantitatively demonstrate their efficacy and to monitor osteoarthritis progression at the functional level are urgently needed. Our group showed that articular cartilage can be quantitatively assessed in nuclear medicine imaging by our radiotracer 99mTc-NTP 15-5 targeting cartilage proteoglycans. In this work, surgically induced DMM mice were treated with sprifermin or saline. We investigated cartilage remodelling in the mice knees by 99mTc-NTP 15-5 SPECT-CT imaging over 24 weeks after surgery, as wells a

Cytosolic phospholipase A2 as a therapeutic target for degenerative joint diseases.

Osteoarthritis (OA) and intervertebral disc degeneration (IVDD) are degenerative musculoskeletal disorders characterized by degeneration of cartilaginous tissues and inflammation. While inflammation is implicated in the pathogenesis of OA and IVDD, and cytosolic phospholipase A2 (cPLA2) is a key mediator of inflammation, direct evidence linking cPLA2 to chondrocyte homeostasis and cartilage degeneration is lacking. This study aims to investigate the role of cPLA2 in chondrocytes and its contribution to the development of cartilage degenerative conditions such as OA and IVDD. Here, single-cell

Early lesions of articular osteochondrosis in the distal femur of foals.

Failure of the cartilage canal blood supply to epiphyseal growth cartilage has been implicated in the pathogenesis of articular osteochondrosis in horses and other animal species. In a previous study of the developmental pattern of the blood supply in the tarsus of foals, early lesions of osteochondrosis were consistently found in regions where the cartilage canal vessels traversed the chondro-osseous junction. The developmental pattern of blood vessels has also been described in the distal femoral epiphysis; however, the group of foals examined in that study did not have lesions of osteochond
