Inpatient management of severe malnutrition: time for a change in protocol and practice.

This review focuses on how to reduce the high mortality of severe acute malnutrition (SAM) in African hospitals. The World Health Organization's 1999 manual for physicians (protocol) has not resulted in case-fatality rates of under 5%, even in published research studies from Africa, far less in district and central hospitals which do not record case-fatality rates. It is suggested that the following eight changes to the protocol need to be considered if we are serious about reducing case-fatality rates in African hospitals: (1) use of low lactose, low osmolality milk feeds during the early sta

[Bacteriologic examination of gallbladder contents].

INTRODUCTION: Acute calculous (obstructive) cholecystitis develops as a consequence of cystic obstruction and obstruction of bile flow into choledochus. Most often it is a result of impacted gallstones in Hartman's pouch or the cystic duct. Their direct pressure on gallbladder mucosa causes ischemia, necrosis and ulceration with consequential wall edema and obstructed venous flow. This mechanism is further increasing and spreading the inflammatory process. Ulcerations may be that extensive, that mucosa is highly recognizable on the microscopic preparation. Leukocyte infiltration of all segment

Can results obtained with commercially available MicroScan microdilution panels serve as an indicator of beta-lactamase production among escherichia coli and Klebsiella isolates with hidden resistance to expanded-spectrum cephalosporins and aztreonam?

Among clinical isolates of Escherichia coli, Klebsiella pneumoniae, and Klebsiella oxytoca, there is an ever-increasing prevalence of beta-lactamases that may confer resistance to newer beta-lactam antibiotics that is not detectable by conventional procedures. Therefore, 75 isolates of these species producing well-characterized beta-lactamases were studied using two MicroScan conventional microdilution panels, Gram Negative Urine MIC 7 (NU7) and Gram Negative MIC Plus 2 (N+2), to determine if results could be utilized to provide an accurate indication of beta-lactamase production in the absenc

Extended-spectrum plasmid-mediated beta-lactamases.

Extended-spectrum beta-lactamases (ESBLs) are mutant enzymes which derive from TEM or SHV (class A) enzymes. They confer variable levels of resistance to cefotaxime, ceftazidime and other broad-spectrum cephalosporins and to monobactams such as aztreonam but have no detectable activity against cephamycins and carbapenems. Recently, new plasmid-mediated ESBLs, not derived from TEM or SHV enzymes but related to cephalosporinases of Enterobacteriaceae (class C enzymes), that confer resistance to all cephalosporins including cephamycins, have been reported. However, to date there have been no repo

Salmonella infections in infants and children.

BACKGROUND: Salmonella is an important pathogen for infants and children of which multiply resistant strains were recently reported. Our interest was the annual variation of clinical manifestations, serotype and the sensitivity rate for antibiotics during the past decade. Increased incidence of bacteremia in infants is reported. In order to find the predisposing factors of patients with acute gastroenteritis complicated with bacteremia, we conducted a retrospective analysis. METHODS: We reviewed the records of documented patients who had positive culture for Salmonella in our Pediatric Depart

Antibiotic prophylaxis in hysterectomy and induced abortion. A review of the evidence.

By the early 1980s, perioperative prophylaxis in vaginal hysterectomy had been shown consistently to be of such value in reducing postoperative infection that some authors maintained that further placebo-controlled studies were no longer ethical. The benefit of prophylaxis in abdominal hysterectomy was less uniformly demonstrated, in studies which were prospective, placebo-controlled, double-blind and randomised. Prophylaxis may significantly reduce the incidence of febrile morbidity and/or wound/pelvic infection, the duration of hospital stay, or the total usage of antibiotics. It is therefor

Carriage and susceptibility of Enterobacteria isolated from the local chickens at markets and peri-urban areas of Bobo-Dioulasso, Burkina Faso.

BACKGROUND: Salmonella and Escherichia coli are among the main causes of foodborne illness and negatively impact poultry industry. The aim of this study is to determine the carriage rate and sensitivity of Enterobacteria isolated from the cloacae of chickens. Samples were taken only from healthy local chickens of markets (55 samples) and outlying areas (45 samples) using the swabbing technique. The swabs were pre-enriched and inoculated onto Hektoen agar at 37 °C for 24 h. Bacteria and resistance genes were identified through molecular biology. Antimicrobial susceptibility was tested by Kirby

Prevalence of antimicrobial resistance in Tanzania: A systematic review and meta-analysis.

Antimicrobial resistance (AMR) threatens global health, and understanding resistance patterns aids in effective treatment and promotes responsible antimicrobial use. Despite the urgency of resistant pathogens, systematic reviews focusing specifically on Tanzania are limited, and while several studies report resistance patterns for individual pathogens, a consolidated analysis of overall prevalence is needed to inform policymaking and public health interventions. Therefore, this review and meta-analysis assessed the prevalence of antimicrobial resistance among clinically relevant pathogens in T

Phenotypic-genotypic characteristics of Corynebacterium striatum clinical isolates and diversified biofilm production capabilities in the presence of plasma proteins.

BACKGROUND: Corynebacterium striatum is an opportunistic pathogen associated with a wide range of hospital-acquired infections, in which biofilm production plays a critical role in nosocomial transmission and pathogenicity. Although genotypic heterogeneity among C. striatum clinical isolates has been increasingly recognized, factors influencing isolate-specific biofilm formation, particularly host-derived factors, remain poorly understood. This study investigated the biofilm formation characteristics of C. striatum isolates with distinct genotypic and phenotypic profiles under exposure to huma

Prevalence and bacteriological profile of pathogens causing neonatal sepsis in Tanzania: a systematic review with meta-analysis.

BACKGROUND: Neonatal sepsis is the significant cause of morbidity and mortality in low- and middle-income countries (LMICs) such as Tanzania, where 99% of global neonatal deaths occur. Epidemiological data regarding the causative agents and their resistance profiles can guide clinical practice and prevention strategies. A systematic review and meta-analysis was conducted to determine the prevalence, pathogens and the outcome of neonatal sepsis in Tanzania. METHODS: We conducted a systematic literature search using online databases such as PubMed/ Medline, Embase, Cochrane, Google Scholar, and

Probable European-profile Borrelia-associated myocarditis in an Australian patient with immune resolution following early therapy: a case report.

Tick-associated illnesses are increasingly recognised in Australia, yet the epidemiology and clinical manifestations of Borrelia burgdorferi sensu lato remain uncertain in the absence of confirmed local isolates and reliance on diagnostics validated for European and North American strains. These limitations complicate interpretation and may contribute to delayed recognition of systemic manifestations, with potential progression to Debilitating Symptom Complexes Attributed to Ticks (DSCATT). We describe a case consistent with European-profile Borrelia infection presenting with early inflammator

Prevalence, antimicrobial resistance, and associated factors of critical superbugs Klebsiella pneumoniae and Acinetobacter species among hospitalized patients in Northwest Ethiopia.

BACKGROUND: Antimicrobial resistant Klebsiella pneumoniae and Acinetobacter species are major contributors to healthcare-associated infections globally. These organisms have emerged as critical superbugs due to their capacities to acquire multidrug resistance (MDR), cause severe infections, and persist in hospital environments. Evidence on their burden and antimicrobial resistance profile remains limited in Northwest Ethiopia. This study aimed to determine the prevalence, antimicrobial resistance profiles, and associated factors of K. pneumoniae and Acinetobacter species among hospitalized pat

Sepsis neonatorum: Bacterial profile, antimicrobial resistance patterns, among neonates and associated factors, Dilla University Referral Hospital, southern Ethiopia, with special reference to WHO prioritized pathogens.

BACKGROUND: Sepsis neonatorum (neonatal sepsis) constitutes severe, life-threatening infections, frequently necessitating intensive care unit admissions, and is a leading indication for antibiotic prescription in neonatal clinical practices. In Ethiopia, neonatal sepsis is quite rampant, and the widespread empirical therapy results in several interrelated challenges, such as incorrect or misuse of antibiotics, bacterial virulence, and aggravation of drug resistance and treatment failures. AIMS: This study was done to assess the culture-positive sepsis, antibiotic susceptibility patterns, and
