Pregnancy and epilepsy.

The management of women with epilepsy who are planning a pregnancy or who are currently pregnant remains one of the most perplexing and engaging clinical issues for neurologists. A refinement of surveillance methods, along with a greater understanding of teratogenesis, has provided us with better information regarding specific antiepileptic drug (AED)-associated teratogenic risk than in the past. A recent finding from multiple worldwide AED pregnancy registries is an increased teratogenic risk with valproate compared with the other AEDs, supporting previous retrospective reports. Valproate use

Tuberculoma of the brain: a series of 16 cases treated with anti-tuberculosis drugs.

OBJECTIVES: To describe the clinical presentation, radiological findings and outcome of treatment with antituberculosis drugs in 16 cases of intracranial tuberculoma. DESIGN: Consecutive cases admitted with tuberculoma to the National Center for Neurological Diseases in Khartoum, Sudan, were included in the study. The diagnosis was based on clinical and neuro-imaging features and response to anti-tuberculosis treatment. Histopathology of material from the brain or other extracranial tissues was available in seven cases. RESULTS AND CONCLUSIONS: The commonest presenting features were headache

Transient and permanent magnetic resonance imaging abnormalities after complex partial status epilepticus.

Epileptic seizures, especially status epilepticus can produce MRI changes. In contrast to convulsive status epilepticus (CSE), permanent parenchymal loss is not well documented with nonconvulsive status epilepticus (NCSE) and the observed MRI changes are transient. We describe a patient with non-lesional right-sided temporal lobe epilepsy with complex partial seizures and repeated episodes of untreated complex partial status epilepticus (CPSE). Diffusion-weighted MRI exhibited marked and extended signal changes within the right temporal, frontal, insular and cingulate regions. The affected are

Management of severe and complicated malaria.

Severe malaria is invariably caused by Plasmodium falciparum. In India, both adults and children are affected by severe malaria. However, children are more prone for developing anemia and convulsions as manifestations of severe malaria, while acute renal failure and jaundice are more common among adults. Pregnant women are vulnerable to hypoglycemia, anemia and pulmonary complications. The case-fatality rate due to severe malaria is 10-15% in spite of therapy but it increases in the presence of renal failure or respiratory distress (pulmonary edema or ARDS). Of late, multi-organ failure and hi

Glomerulocystic kidney in a domestic dog.

Glomerulocystic kidney was diagnosed in a 5-year-old female Shiba dog, which died from chronic renal failure with convulsions, vomiting and diarrhoea. Haematological examination revealed non-regenerating anaemia, azotaemia and high serum creatinine. Grossly, both kidneys were mildly atrophic with multiple small cysts in the cortex. Histopathological examination revealed marked dilatation of Bowman's space, often with glomerular atrophy or loss, and mild interstitial fibrosis. Bowman's basement membranes (BMs) were tortuous and thickened, with patchy calcification. Glomerulo-tubular junctions i

Disadaptive disorders in women: allopregnanolone, a sensitive steroid.

Allopregnanolone, a neurosteroid acting as a potent anxiolytic agonist of the gamma-aminobutyric acid A receptor, has been shown in animal models to modify its concentrations at central and peripheral levels according to the estrous cycle. Moreover, it modulates behavioral and biochemical responses to acute and chronic stress, anxiety, depression, aggressiveness, convulsions, anesthesia, sleep, memory, pain and feeding. These observations suggest that fluctuations of allopregnanolone might be involved in the development, course and prognosis of some mental disorders in humans. This has been hy

[Against influenza in children, which actions?].

Each year, at any age, children are at risk of influenza illness during the epidemics. Children, especially those at school (attack rate close to 30%), have a major role in viral dissemination. Most of influenza illnesses occur in healthy children. Manifestations are typical, as in adults, in children over 5 years of age and less specific in younger children explaining misdiagnosis and underestimation in the youngest. Respiratory complications in outpatients include acute otitis media (close to 30% in children less than 3 years of age) and pulmonary disease (5 up to 10%). High fever 240 degree

Severe tension pneumocephalus caused by opening of the frontal sinus by head injury 7 years after initial craniotomy--case report.

A 46-year-old man presented with severe tension pneumocephalus triggered by mild head injury 7 years after craniotomy. He had a history of subarachnoid hemorrhage due to ruptured anterior communicating artery aneurysm, coating of the aneurysm performed via a craniotomy, and a ventriculoperitoneal (VP) shunt inserted. He fell from bed in a rehabilitation hospital. Eight hours after the injury, he became comatose and suffered general convulsion. He was then transferred to our hospital. Radiography and computed tomography (CT) revealed a large amount of intracranial air and a widely opened fronta

A review of neurological disorders presenting at a paediatric neurology clinic and response to anticonvulsant therapy in Gambian children.

A clinical review of 128 children presenting at the paediatric neurology clinic of the Royal Victoria Hospital, The Gambia was conducted between July 2001 and January 2002. The aim was to assess the spectrum and burden of neurological disease presenting at this tertiary-level clinic and to evaluate the effectiveness of anticonvulsant therapy. The principal clinical diagnoses most frequently made were seizures (57%), neuromotor problems (15%) and developmental delay (11%). Many children also had associated developmental delay or learning difficulties (55%), speech and language problems (42%) an

Case report: Angelman syndrome in an individual with a small SMC(15) and paternal uniparental disomy: a case report with reference to the assessment of cognitive functioning and autistic symptomatology.

The case of a 15-year-old male with Angelman syndrome, paternal uniparental disomy of chromosome 15, and a small supernumerary marker chromosome is discussed. Assessment of cognitive functioning revealed an uneven profile of ability across different domains; in particular, receptive language ability was found to be superior to expressive language ability, whilst both gross and fine motor skills were found to be relatively well developed. Assessment using the Autism Diagnostic Observation Schedule showed very little evidence of autistic symptomatology. The patient showed an interest in social i

[Neurologic aspects of eclampsia].

The difficult types of preeclampsia and eclampsia are presented with the neurological symptoms. The break of cerebral autoregulation mechanism plays the most important role in pathogenesis of cerebral vasospasm. Nevertheless, eclampsia isn't just an ordinary hypertensive encephalopathy because other pathogenic mechanisms are involved in its appearance. The main neuropathologic changes are multifocal vasogenic edema, perivascular multiple microinfarctions and petechial hemorrhages. Neurological clinical manifestations are convulsions, headache, visual disturbances and rarely other discrete foca

[A case of neurocysticercosis suggestive of a reinfection, 20 years after the initial onset].

A 70-year-old man, who had frequently visited the Southeast Asian countries as a tour conductor, presented convulsion as an initial symptom twenty years ago. A diagnosis of neurocycticercosis was made by a stereotactic brain biopsy. Thereafter, anticonvulsant therapy was initiated, and he continued his travelling without any troubles. After travelling to Philippines, he suffered high-grade fever and consciousness disturbance, and his general status deteriorated. His consciousness level was evaluated as Japan Coma Scale 3 and his status was bedridden on admission to our hospital in December 199

Prolactin-releasing peptide (PrRP) promotes awakening and suppresses absence seizures.

Prolactin releasing peptide (PrRP) is a recently identified neuropeptide that stimulates prolactin release from pituitary cells. The presence of its receptor outside the hypothalamic-pituitary axis suggests that it may have other functions. We present here evidence that PrRP can modulate the activity of the reticular thalamic nucleus, a brain region with prominent PrRP receptor expression that is critical for sleep regulation and the formation of non-convulsive absence seizures. Intracerebroventricular injection of PrRP (1-10 nmol) into sleeping animals significantly suppresses sleep oscillati

Antagonism of cyanide poisoning by dihydroxyacetone.

Dihydroxyacetone (DHA) effectively antagonized the lethal effect of cyanide in mice and rabbits, particularly if administered in combination with thiosulfate. Oral DHA (2 and 4 g/kg) given to mice 10 min before injection (i.p.) of cyanide increased the LD50 values of cyanide from 5.7 mg/kg to 12 and 17.6 mg/kg, respectively. DHA prevented cyanide-induced lethality most effectively, if given orally 10-15 min before injection of cyanide. A combination of pretreatment with oral DHA (4 g/kg) and post-treatment with sodium thiosulfate (1 g/kg) increased the LD50 of cyanide by a factor of 9.9. Furth

Neurologic complications due to catheterization.

In our university cardiac center, the incidence of a cohort of children with acute neurologic complications resulting from cardiac catheterization performed for acyanotic or cyanotic congenital heart disease is 0.38% (14 children of a total of 3,648 catheterization procedures). Neurologic complications consisted of convulsion (n = 10), stroke (n = 6), intracranial hemorrhage (n = 2), extrapyramidal features (n = 1), paraplegia (n = 1), visual impairment (n = 1), hearing impairment (n = 1), and brachial plexus injury (n = 1). The main risk factors included prolonged duration of catheterization

Perioral myoclonia with absence seizures: a rare epileptic syndrome.

We present the clinical and video-EEG data on an epileptic boy whose absence seizures with marked perioral movements had started at the age of 1.5 years. From age 12 years, he experienced frequent episodes of typical absence status epilepticus (ASE) lasting 1-2 hours with marked perioral myoclonia and moderate confusion. Initial therapy with carbamazepine was substituted by valproate because of worsening of the absence seizures. At the age of 17, the patient was admitted to our clinic with his usual, but long lasting ASE attack, accompanied by 2 generalized tonic-clonic convulsions. ASE was co

Thrombotic stroke in a child with diarrhea-associated hemolytic-uremic syndrome with a good recovery.

A boy aged 3.5 years with post-diarrheal hemolytic-uremic syndrome (HUS) was referred to our hospital because of convulsion and stupor. He had been admitted to a regional hospital with a 3-day history of bloody diarrhea, colic abdominal pain and fever. Two days later, he had complained of generalized seizures and oliguria. On admission, he developed anuria, and serum blood nitrogen and creatinine increased to 56 mg/100 ml and 2.8 mg/100 ml, respectively. Platelets decreased to 42,000/microl. Under the diagnosis of HUS, a continuous hemodiafiltration treatment had to be instituted. Computed tom

[Molecular genetic aspects of the susceptibility to develop idiopathic epilepsy].

INTRODUCTION: There are more than 40 clinical types of epilepsy classified according to aetiology and/or mode of clinical onset. The term idiopathic epilepsy is reserved for cases with convulsions but no detectable structural lesions of the brain or neurological anomalies. DEVELOPMENT AND CONCLUSIONS: In spite of many studies confirming the importance of genetic factors in the occurrence of idiopathic epilepsy, these appear to be complex and probably involve a locus of variable expression or several loci with similar phenotype expression (epistaxis). Also, environmental factors have variable

Inherited ion channel disorders.

UNLABELLED: The inherited ion channel disorders (channelopathies) are a group of disorders caused by mutations in genes encoding ion channels. Ion channel disorders can affect any tissue, but the majority affect skeletal muscle or the central nervous system. These disorders include skeletal muscle sodium channelopathies causing hyperkalaemic periodic paralysis, paramyotonia congenita and potassium-aggravated myotonia. Skeletal muscle calcium channelopathies can cause hypokalaemic periodic paralysis, malignant hyperthermia and central core disease. Skeletal muscle chloride channelopathies can c

Dose-related differences in the distribution of cocaine in the maternal-fetoplacental compartments in rats.

Our goals were to examine whether a high dose of cocaine to causing CNS toxic manifestations in the pregnant rats influences the delivery of cocaine to the fetus, and whether the non-placental compartments have a significant role in the distribution of cocaine to the fetal tissues. Either a low or high dose of cocaine was infused intravenously to near-term pregnant rats. Arterial blood pressure and heart rate were monitored. Cardiac output and uterine and placental blood flows were measured by using radiolabeled microspheres. Plasma and tissue samples were obtained from the mother, placenta, a
