[The facial nerve and the retrosigmoid approach].

The author gives an account of his experience with the retrosigmoid approach, and emphasizes the preservation of the facial nerve and hearing. A detailed description of the operative technique is given. This approach is used when it is intended to conserve the hearing. The series consists of 2 stage III cases and 26 stage I and II cases. The two stage III cases had total deafness. For the other stages, 25 patients had normal facial function postoperatively. The hearing was preserved in 9 cases in the frequency range 500-1000-2000 Hz, with thresholds of less than 30 dB, and in 7 of cases these

[Fibrodysplasia (myositis) ossificans progressiva].

A seven-year-old boy with mental retardation and congenital skeletal malformations in the thumbs and big toes developed recurrent lumps in the shoulder and interscapular region. The lumps subsided slowly but left severe stiffness in the affected areas. The first biopsy revealed oedema and a chronic inflammatory response with fibromyxoid proliferation of the soft tissue. A second biopsy revealed ossification of muscle and fascia. The patient is a typical case of fibrodysplasia ossificans progressiva, an autosomal dominant inherited disorder characterized by congenital skeletal malformations in

Identification of mutations in the alpha 3(IV) and alpha 4(IV) collagen genes in autosomal recessive Alport syndrome.

Alport syndrome (AS) is an hereditary disease of basement membranes characterized by progressive renal failure and deafness. Changes in the glomerular basement membrane (GBM) in AS suggest that the type IV collagen matrix, the major structural component of GBM, is disrupted. We recently isolated the genes for two type IV collagens, alpha 3(IV) and alpha 4(IV), that are encoded head-to-head on human chromosome 2. These chains are abundant in normal GBM but are sometimes absent in AS. We screened for mutations in families in which consanguinity suggested autosomal recessive inheritance. Homozygo

Prevalence and pattern of major neurological disorders in rural Kashmir (India) in 1986.

In 1986 in the Kuthar Valley in the Anantnag District of south Kashmir (northwestern India), we studied the population to ascertain the prevalence and pattern of various neurological diseases. A house-to-house survey was done in a rural population of 63,645 (according to a World Health Organization protocol, 1981). 616 cases of major neurological disorders were detected, yielding a prevalence of ratio of 9.67/1,000 as of prevalence day November 1, 1986. The prevalence ratios for various common neurological disorders were: epilepsy 2.47/1,000; stroke 1.43/1,000; paralytic poliomyelitis 2.18/1,0

[Otorhinolaryngologic aspects of diving sports].

ENT disorders are the most common of all medical problems of diving. This review summarizes the specific conditions and ENT diseases in Scuba diving. During compression failure to equalize the pressure of air-filled cavities surrounded by bone deprives the middle ear or sinuses of aeration. Middle ear barotrauma is the most common barotrauma encountered in divers while sinus barotrauma and especially inner ear barotrauma (with rupture of the round or oval window) are less common. Decompression sickness in primarily the result of inert gas bubbles; deafness and vertigo may result if the inner e

Surgery for congenital anomalies of the middle ear with mobile stapes.

In a series of 104 patients with congenital middle ear anomalies operated on from 1964 to 1986, 27 cases were found in which the stapes footplate was mobile and the conductive deafness was due to an anomaly in the remaining part of the ossicular chain. In 8 cases the middle ear anomaly was caused by discontinuity of the ossicular chain owing to congenital malformation. In the other 19 ears, epitympanic fixation of the ossicular chain was observed, whether or not in combination with malformation of the stapes, incus or malleus. Ossicular chain reconstruction produced an improvement of at least

Hearing aid prescribing: is the specialist opinion necessary?

A restructuring of the service for provision of hearing aids has been proposed by the Royal National Institute for the Deaf. This is based on the assumption that very few patients referred for hearing aids have significant ear disease and it is not necessary for them to see an ENT specialist. The case notes of 200 consecutive patients referred to the Hearing Aid clinic were reviewed. In only half of these would a hearing aid have been prescribed without a specialist opinion. The remainder either did not need a hearing aid or required further investigation and surgical or medical treatment. In

[Forensic evaluation of physical damage in the early Middle Ages--a comparison with current occupational disability guidelines].

The legal systems of the Germanic tribes in the early Middle Ages elaborated detailed catalogs of forfeits in compensation for certain physical injuries. The perpetrator had to pay the forfeit to the injured person, or in case of manslaughter, to the tribe of the dead. By doing so he could avert the feud which otherwise faced him. These catalogs of forfeits exactly reflect the relative value that was appointed to certain parts of the body and to sensory functions. The catalog of the Lex Saxonum (c. 802), in which physical injuries are listed, ranging from loss of single phalanges, differentiat

Ear involvement in Wegener's granulomatosis.

In 16 of 19 patients with biopsy-proved Wegener's granulomatosis the early manifestations were limited to the ear and nose. The audiological data of 13 patients revealed middle ear involvement in 16 of 26 ears. Twenty-one of 26 ears presented with a low to moderate sensorineural hearing loss. One ear remained deaf after a sudden hearing loss in the early stage of the disease. Serologically, 4 of 6 tested patients with sensorineural hearing loss demonstrated antibodies against sarcolemma. One patient showed antinuclear antibodies. It is remarkable that these antibodies can often be detected in

Ménière's disease. A practical approach to management.

The management of Ménière's disease continues to provide a formidable clinical challenge, mainly because its precise aetiology is unknown. The unpredictable natural history of the condition has consistently bedevilled attempts to evaluate therapeutic efficacy, and there have been no entirely satisfactory prospective controlled clinical trials of any specific medical or surgical treatment. Although no form of medication has been convincingly shown to influence the long term course of the disease, many drugs may be useful in the control of vertigo. In recent years conservation of hearing has ass

Endolymphatic subarachnoid shunt for Ménière's disease in the only hearing ear.

This report describes a series of 18 patients, deaf in one ear, upon whom an endolymphatic subarachnoid shunt procedure was performed on the remaining hearing ear affected by Ménière's disease. As with other patients with Ménière's disease, those whose problem involves their only hearing ear can have differing priorities and etiologies requiring variations in diagnosis and management. The preservation of hearing is of major consideration as is the need for thorough study and careful diagnosis in the determination of when to operate. Special surgical techniques that help minimize the risks from

Identity of two syndromes. Proteolytic, lipolytic, and amylolytic deficiency of the exocrine pancreas with congenital anomalies.

We describe further a patient previously reported to have "proteolytic and lipolytic deficiency of the exocrine pancreas." Features not previously reported include absent permanent teeth, hypoplastic alae nasi, scalp defect, normal thyroid function, and normal gonadotropins. Moreover, the normal amylase activity initially assumed to be of pancreatic origin proves to be of salivary origin. Thus the pancreatic deficiency is not only proteolytic and lipolytic but amylolytic as well. This disorder is identical with the "syndrome of congenital aplasia of the alae nasi, deafness, hypothyroidism, dwa

Audiological findings in children with a congenital rubella syndrome.

This is a report of the hearing in 22 consecutive children seen in an audiology clinic because of intrauterine infection with rubella-virus. The diagnosis of congenital rubella may be difficult and sometimes impossible to make retrospectively. It is often a probability diagnosis made by a combination of laboratory tests and various deformities in the heart, the central nervous system, the eyes, and the inner ear. Audiometrically the findings are very much differentiated, and there is no typical configuration. The results reported in a scientific paper from USA mentioning a relatively good high

The Johanson-Blizzard syndrome: case report and autopsy findings.

We report the case of a boy with the Johanson-Blizzard syndrome who died at the age of 8 years with complications of pancreatic exocrine insufficiency, and at autopsy was found to have a small thyroid filled with colloid, virtually complete replacement of the pancreas with adipose tissue, and a brain of normal size but with evidence of a cortical developmental defect consisting of abnormalities of gyral formation and of cortical neuronal organization. In addition the boy had postnatal growth failure, apparent severe mental retardation, congenital scalp defects and scalp hair patterning abnorma

Normocalcaemic pseudohypoparathyroidism with unusual phenotype.

We describe a boy who presented at 4 years of age with radiological hyperparathyroidism, osteosclerosis, and necrosis of the femoral heads. Plasma biochemistry was normal but the parathyroid hormone (PTH) level was very high. He was deaf and had an unusual facies but did not have the phenotype of Albright's hereditary osteodystrophy. Plasma and urine cyclic AMP reponses to bovine PTH were markedly subnormal. Vitamin D produced sustained hypercalcaemia and a fall in plasma phosphorus. After four hyperplastic parathyroid glands were removed he became hypocalcaemic and plasma phosphorus rose. Aft

Nursing and midwifery students' perceptions of sign language training in Ghana: relevance, instructional methods, and assessment practices.

INTRODUCTION: Effective communication between healthcare practitioners and patients is essential in ensuring quality healthcare delivery and the attainment of positive health outcomes. However, many healthcare providers lack adequate training to communicate appropriately with patients with hearing loss or hard-of-hearing patients, resulting in avoidable barriers to quality care. Despite the recent integration of sign language training into the curriculum of nursing and midwifery, research examining its application and effectiveness remains limited. OBJECTIVE: This study examines the perceived

Patient and public involvement and engagement (PPIE): how valuable and how hard? An evaluation of ALL_EARS@UoS PPIE group, 18 months on.

BACKGROUND: ALL_EARS@UoS is a patient and public involvement and engagement (PPIE) group for people with lived experience of hearing loss. The purpose of the group is to share experiences of hearing loss and hearing healthcare, inform research and improve services for patients at University of Southampton Auditory Implant Service. A year after inception, we wanted to critically reflect on the value and challenges of the group. Four members of ALL_EARS@UoS were recruited to an evaluation steering group. This paper reports the evaluation of the group using the UK Standards for Public Involvement

Ototoxicity-related changes in GABA immunolabeling within the rat inferior colliculus.

Several studies suggest that hearing loss results in changes in the balance between inhibition and excitation in the inferior colliculus (IC). The IC is an integral nucleus within the auditory brainstem. The majority of ascending pathways from the lateral lemniscus (LL), superior olivary complex (SOC), and cochlear nucleus (CN) synapse in the IC before projecting to the thalamus and cortex. Many of these ascending projections provide inhibitory innervation to neurons within the IC. However, the nature and the distribution of this inhibitory input have only been partially elucidated in the rat.
