A Universal Model of Cation Effects in Electrocatalysis.

Electrolyte cations are conventionally viewed as inert spectators in electrocatalysis. However, a wealth of observations show that catalytic rates are often highly sensitive to cation identity. Despite their prevalence, these cation effects have resisted a unified mechanistic explanation, with different physical phenomena implicated across reaction chemistries, catalyst compositions, and choice of solvent. In this perspective, we describe a general framework for understanding cation effects in electrocatalysis based on electrostatics. We argue that cations influence reaction rates by modifying

Generalization of regional citrate anticoagulation for continuous renal replacement therapy is not associated with an increased rate of severe complications.

BACKGROUND: Contraindications to regional citrate anticoagulation (RCA) for continuous renal replacement therapy (CRRT) have recently been challenged. We aimed to assess the safety of the generalization of RCA to all CRRT sessions. METHODS: We reviewed all CRRT sessions performed in our ICU during two periods (P1:2018-2019 and P2:2020-2022). RCA was considered as contraindicated in situations at risk of citrate accumulation (lactate >4 mmol/L and/or prothrombin time < 40 %) during P1 but not P2. We reviewed CRRT modality, filter lifespan, and therapy-associated complications including citrate

Unexplained Coma and Sudden Death in Psychiatric Patients Due to Self-Induced Water Intoxication: Clinical Insights and Autopsy Findings From Two Fatal Cases.

Self-induced water intoxication is a life-threatening condition caused by excessive water intake that surpasses renal excretion capacity, resulting in hypotonic hyponatremia. This acute imbalance leads to cerebral and pulmonary edema, neurological deterioration, and potentially fatal outcomes. Psychiatric disorders such as schizophrenia and postpartum psychosis are significant contributors, often driving these behaviors through unique psychopathological mechanisms exacerbated by inadequate patient supervision. This study presents two fatal cases. The first involves a 42-year-old woman with chr

Systolic blood pressure targets below 120 mm Hg are associated with reduced mortality: A meta-analysis.

BACKGROUND: The optimal systolic blood pressure (SBP) target in patients with increased cardiovascular risk remains uncertain. This study evaluated the efficacy and safety of intensive SBP control (<120 mm Hg) compared to standard SBP control (<140 mm Hg) in patients with increased cardiovascular risk. METHODS: We conducted a systematic search of PubMed, Embase, Web of Science, and Cochrane Library for RCTs published from database inception through November 2024 that compared intensive SBP control (<120 mm Hg) with standard SBP control (<140 mm Hg) in adults with high cardiovascular risk. Hyponatraemia, AKI, and urinary retention following elective joint arthroplasty.

BACKGROUND: Acute kidney injury (AKI) and hyponatraemia are common postoperative complications following elective joint replacement surgery, often related to fluid and electrolyte imbalances. Dehydration can contribute to AKI, whereas excessive fluid administration may lead to hyponatraemia. Additionally, urinary retention can mimic AKI by reducing urine output, potentially resulting in unnecessary fluid administration and dilutional hyponatraemia. This study aims to improve postoperative fluid-electrolyte management by differentiating AKI from urinary retention and reducing the incidence of h

BEST4+ cells in the intestinal epithelium.

The recent development of single-cell transcriptomics highlighted the existence of a new lineage of mature absorptive cells in the human intestinal epithelium. This subpopulation is characterized by the specific expression of Bestrophin 4 (BEST4) and of other marker genes including OTOP2, CA7, GUCA2A, GUCA2B, and SPIB. BEST4+ cells appear early in development and are present in all regions of the small and large intestine at a low abundance (<5% of all epithelial cells). Location-specific gene expression profiles in BEST4+ cells suggest their functional specialization in each gut region, as ex

Metolazone Versus Chlorothiazide in Acute Heart Failure Patients With Diuretic Resistance and Renal Dysfunction: A Retrospective Cohort Study.

Guidelines recommend intravenous loop diuretics as first-line therapy for patients hospitalized with acute heart failure (AHF) and volume overload. Additional agents can be used for augmentation, but there is limited guidance on agent selection. The study objective was to determine if chlorothiazide or metolazone is associated with differences in diuretic efficacy or safety in loop diuretic-resistant patients with AHF and renal dysfunction (eGFR <45 mL/min/1.73 m²). We conducted a multicenter, retrospective cohort study of patients hospitalized with AHF and renal dysfunction who received metol

Personalized Hydration Strategy to Improve Fluid Balance and Intermittent Exercise Performance in the Heat.

Sweat rate and electrolyte losses have a large inter-individual variability. A personalized approach to hydration can overcome this issue to meet an individual's needs. This study aimed to investigate the effects of a personalized hydration strategy (PHS) on fluid balance and intermittent exercise performance. Twelve participants conducted 11 laboratory visits including a VO2max test and two 5-day trial arms under normothermic (NOR) or hyperthermic (HYP) environmental conditions. Each arm began with three days of familiarization exercise followed by two random exercise trials with either a PHS

From Syncope to Pregnancy: A Case Report of Gestational Transient Thyrotoxicosis.

With a multifactorial etiology, syncope is a fairly common clinical presentation in emergency care. During pregnancy, it can result from hemodynamic and hormonal changes. One such rare cause is hyperthyroidism. Although physiological maternal adaptation to fetal requirements can often induce mild, transient, and self-limited thyroid stimulation during the late first trimester, clinically significant thyrotoxicosis can still occur throughout pregnancy. In these cases, the most prevalent causes are gestational transient thyrotoxicosis (GTT) and Graves' disease. We report the case of a 24-year-ol

Case Report-Severe Hyponatremia at Birth in a Premature Infant.

BACKGROUND: Given the role of the placenta in maintaining maternal-fetal equilibrium, changes in maternal sodium levels affect the fetus. Clinicians must also account for the direct impact of maternal conditions and medications on the neonate. Gestational hyponatremia develops in approximately one-third of mothers with preeclampsia with severe features. Additionally, the use of selective antidiuretic (V2 receptor) agonist 1-deamino-8-D-arginine-vasopressin, commonly known as DDAVP, during pregnancy leads to maternal hyponatremia by inhibiting maternal diuresis. We present a case of severe hypo

Tenofovir-induced renal and bone toxicity: report of two cases and literature review.

Tenofovir Disoproxil Fumarate (TDF) is one of the drugs in the initial first-line antiretroviral regimen for the treatment of hepatitis B and HIV infections. Despite its effectiveness and few adverse effects, it is related to renal and bone toxicity. We described two cases of HIV-positive middle-aged women who had been using TDF for two and four years (cases 1 and 2, respectively) and were admitted to the emergency room. Case 1 presented with metabolic ileum and diffuse bone pain while case 2 presented with bilateral coxo-femoral pain after a fall from standing height. Both cases had similar l

Nationwide Readmission Rates and Hospital Charges for Patients With Surgical Evacuation of Nontraumatic Subdural Hematomas: Part 2-Burr Hole Craniostomy.

BACKGROUND: Nontraumatic subdural hematoma (SDH) is a common neurological disease that causes extensive morbidity and mortality. Craniotomy or burr hole craniostomy (BHC) is indicated for symptomatic lesions, but both are associated with high recurrence rates. Although extensive research exists on postoperative complications after BHCs, few studies have examined the underlying causes and predictors of unplanned 30-day hospital readmissions at the national level. OBJECTIVE: To compare causes for hospital readmission within 30 days after surgical SDH evacuation with BHC and evaluate readmission

Mast Cell Mediated Regulation of Small Intestinal Chloride Malabsorption in SAMP1/YitFc Mouse Model of Spontaneous Chronic Ileitis.

In Inflammatory Bowel Disease (IBD), malabsorption of electrolytes (NaCl) results in diarrhea. Inhibition of coupled NaCl absorption, mediated by the dual operation of Na:H and Cl:HCO3 exchangers on the brush border membrane (BBM) of the intestinal villus cells has been reported in IBD. In the SAMP1/YitFcs (SAMP1) mice model of spontaneous ileitis, representing Crohn's disease, DRA (Downregulated in Adenoma) mediated Cl:HCO3 exchange was shown to be inhibited secondary to diminished affinity of the exchanger for Cl. However, NHE3 mediated Na:H exchange remained unaffected. Mast cells and their

Multitargeted interventions to reduce dialysis-induced systemic stress.

Hemodialysis (HD) is a life-sustaining therapy as well as an intermittent and repetitive stress condition for the patient. In ridding the blood of unwanted substances and excess fluid from the blood, the extracorporeal procedure simultaneously induces persistent physiological changes that adversely affect several organs. Dialysis patients experience this systemic stress condition usually thrice weekly and sometimes more frequently depending on the treatment schedule. Dialysis-induced systemic stress results from multifactorial components that include treatment schedule (i.e. modality, treatmen
