Transaxillary first rib resection for thoracic outlet syndrome: follow-up of 9 cases.

Nine cases of Thoracic Outlet Syndrome (TOS) were treated by transaxillary 1st rib resection over a three year period. Mean follow-up was 26 +/- 4 months. Patients were recalled every 3 months post-operatively for surveillance. Parameters for evaluation included physical examination, return of patient to (full-range or partial range) pre-illness activity, nerve conduction velocities, somato-sensory evoked potentials, Duplex imaging and venography or arteriography if the latter two were indicated. Neurogenic involvement with or without vascular compression was present in 7 cases (78%) and satis

Outward chloride/potassium co-transport in insect neurosecretory cells (DUM neurones).

The mechanism underlying outward chloride transport in the cell body and in the neuritic field of cockroach Dorsal Unpaired Median (DUM) neurones was assessed using the intracellular microelectrode technique. The chloride equilibrium potential was indirectly estimated from the reversal potentials of responses to gamma-aminobutyric acid (GABA) pressure ejections and of inhibitory postsynaptic potential (IPSP) evoked by electrical stimulation of the anterior connectives. Changes in intracellular chloride concentration [Cl-]i following various treatments were estimated from the amplitude changes

Neurobehavioral and neurophysiological outcome of chronic low-level tetrachloroethene exposure measured in neighborhoods of dry cleaning shops.

The effects of chronic low-level tetrachloroethene (TCE) exposure on functions of the central nervous system (CNS) were measured in subjects living in the neighborhood of dry cleaning shops with a mean residential time of 10.6 years. Neurobehavioral tests were performed using a German version of the NES battery. Additionally, a pattern reversal visual-evoked potentials (VEPs) were recorded. the mean blood TCE concentration in the subjects was 17.8 micrograms/liter and the median indoor TCE air concentration measured in the residences was 1.36 mg/m3. The outcome of the NES subtests for vigilanc

Vitamin B12 metabolism and massive-dose methyl vitamin B12 therapy in Japanese patients with multiple sclerosis.

Serum vitamin B12 levels and unsaturated vitamin B12 binding capacities were measured in 24 patients with multiple sclerosis (MS), 73 patients with other neurological disorders and 21 healthy subjects. There was no decrease in the vitamin B12 levels, however, a significant decrease in the unsaturated vitamin B12 binding capacities was observed in patients with MS when compared with other groups. A massive dose of methyl vitamin B12 (60 mg every day for 6 months) was administered to 6 patients with chronic progressive MS, a disease which usually had a morbid prognosis and widespread demyelinati

Auditory abnormalities, including 'precocious presbyacusis', in myotonic dystrophy.

Auditory function tests were performed on 13 patients with myotonic dystrophy (MD). Seven patients had a sensorineural high-frequency hearing loss (HFL) of 30-85 dB at 8 kHz in their pure-tone audiogram, which was in excess of that expected for their age and could be attributed to MD. Their hearing loss resembled 'precocious presbyacusis', i.e. if the patients had been considerably older (or 'functionally' older) than they really were, their HFL could (to some extent) have been attributed to presbyacusis alone. The HFL showed the phenomenon of (genetic) anticipation. Tympanograms and acoustic

[Complete neurological recovery following prolonged cardiac arrest. Description of a clinical case].

We report a case of protracted intraoperative cardiac arrest (15 minutes), occurring in a patient with haemorrhagic shock in consequence of a bullet wound to the right hemithorax; external cardiac massage was immediately started and carried on until the restoration of spontaneous cardiac activity. In the immediate postoperative period, the patient presented seizure activity, treated with TPS in continuous infusion (2 mg/kg/h) and DPH (250 mg/die). Anticonvulsant therapy didn't allow a correct neurologic evaluation of the patient; that was possible only when treatment was suspended (4 days afte

Shunting in chronic post-traumatic hydrocephalus: demonstration of neurophysiologic improvement.

The neuronal metabolic state that forms the cellular basis for cognitive functioning and motor behavior is dependent on cerebral perfusion. As intracerebral pressure increases, cerebral perfusion is compromised. The management of acute hydrocephalus after head trauma has been extensively described. In this article, we present evidence of clinical, cerebral perfusion, and neurophysiologic improvement in a 19-year-old patient with chronic post-traumatic hydrocephalus. Lethargy, gaze palsy, torticollis, and triplegia were noted 2 years postinjury in a neurologically stable patient. Computed axial

The influence of military low-altitude flight noise on the inner ear of the guinea pig. Part I: Hearing threshold measurements.

The auditory thresholds of about 60 anaesthetized guinea pigs were determined at 3 or 4 frequencies between 2 and 20 kHz using the acoustically evoked brain stem potentials. The animals were then exposed to electro-acoustically reproduced MLAF noise with peak levels between 120 and 130 dB(A). The exposure occurred either once or else four times within either 3 or 60 minutes. The level of noise increased at 7.5 and 75 dB/s (slow and fast increase). TTS was then determined. PTS was measured one week later. In over half the animals, exposure to rapidly increasing noise level with a peak of 126 dB

[A case of pure trigeminal motor neuropathy].

A 38-year-old man suffering from pure trigeminal motor neuropathy was admitted with a complaint of occipitalgia. Neurological examination showed severe wasting of the left masseter muscle on chewing. The jaw deviated to the left when he opened his mouth. Cerebrospinal fluid analysis was normal. The CT scan demonstrated a low density area in the left masseter, temporalis, lateral and medial pterygoid muscles. On MRI, the lesion of the left masseter, temporalis, lateral and medial pterygoid muscles showed high intensity of T1 and T2 weighted images. The cervical MRI revealed the non-communicatin

[Neurologic manifestations in Wolfram's syndrome].

Wolfram's syndrome, also known as DIDMOAD syndrome, includes juvenile diabetes mellitus and optic atrophy variously associated with diabetes insipidus and deafness. We describe the neurological findings in 5 patients with Wolfram's syndrome. All patients had a neurological examination and were subjected electrophysiological and brain imaging including CT scan and, in one patient, MRI. There were two pairs of brothers and a sporadic case with paternal consanguinity suggesting recessive inheritance. Neurological abnormalities were found in four patients including dysarthria, seizures, anosmia, n

Intraoperative long-latency reflex activity in idiopathic scoliosis demonstrates abnormal central processing. A possible cause of idiopathic scoliosis.

Segmental reflex regulation in 37 patients with idiopathic scoliosis and 8 patients with nonidiopathic scoliosis was studied by recording evoked reflex muscle potentials from four muscle groups in each lower extremity during partial neuromuscular blockade. Effects on reflex activity mediated through descending systems arising in the brain stem were investigated by recording from proximal-distal and flexor-extensor muscles. Ipsilateral and contralateral long-latency complex polysynaptic activity was present in all 37 patients with idiopathic scoliosis. This reflex activity was absent in eight n

Electrodiagnosis of retrograde changes in carpal tunnel syndrome.

We measured evoked mixed nerve action potentials (EMNAP) in the forearm segment of 108 hands as an indicator of retrograde degeneration in the carpal tunnel syndrome (CTS). This was made by recording at the elbow and stimulating the median nerve 4 cm proximal to the wrist crease. The amplitude of this EMNAP decreased in proportion to the prolongation of the motor terminal latency (MTL) or the decrease in the amplitude of the compound muscle action potential (CMAP) from the abductor pollicis brevis (APB) muscle. This suggests that retrograde degeneration progresses as the severity of CTS increa

[A case of flaccid paraplegia associated with HTLV-I infection].

A 63-year-old female began to have progressive gait disturbance at the age of 50. Neurologic examination revealed weakness and hypotonus of bilateral leg muscles. Deep tendon reflexes were hypoactive with positive Babinski sign and vibration sense was diminished in the legs. HTLV-I antibody titers were elevated both in serum (x 131,072) and in CSF (x 512). The result of nerve conduction velocities and short latency somatosensory evoked potentials suggested mild peripheral nerve lesion. Needle EMG studies showed myogenic changes in the upper limbs and also, neurogenic changes in the lower limbs

The influence of bacterial collagenase on regeneration of severed rat sciatic nerves.

Regeneration of peripheral nerve fibers is impeded by the formation of scar tissue at the site of injury. The possible beneficial effect of collagenase on nerve regeneration was studied using clinical, neurophysiological (evoked potentials) and histological (nerve fiber counts) methods. The sciatic nerves of rats were transected and the severed ends abutted and sewn together. In one series, the area about the lesion was covered with fibrin adhesive and infused with either isotonic saline (controls) or collagenase (treatment group). In the other series, the severed ends of the nerve were insert

Side asymmetry of the jaw jerk in human craniomandibular dysfunction.

The jaw jerk elicited by tapping the chin with a reflex hammer was electromyographically recorded in 14 patients with craniomandibular dysfunction, who were selected because of their strictly unilateral symptoms. Mandibular deviation, as measured by means of a kinesiograph, was on the same side as the pain. Neurological and neurophysiological investigations, including the recording of masseter motor potentials evoked by transcranial stimulation, showed normal function of the sensory and motor trigeminal nerve fibres. Latency and amplitude of the jaw jerk recorded in postural position and inter

Hemodynamic and electrophysiological evaluation following extracranial/intracranial bypass surgery.

We examined 50 patients with an extracranial/intracranial bypass using transcranial Doppler blood flow mapping, somatosensory evoked potentials, and computer-assisted EEG analysis. The investigation was supplemented by temporary compression of the extracranial supplying vessel. Transcranial Doppler sonography revealed in addition to 4 patients with nonpatent anastomoses, a bypass-mediated retrograde flow in the ipsilateral middle cerebral artery beyond a depth of 40 mm in the majority of patients. In 9 patients, retrograde flow could be demonstrated in the whole proximal (M1) MCA segment. The

Neurotoxic effect of two root canal sealers with calcium hydroxide on rat phrenic nerve in vitro.

Root canal sealers placed into contact with the inferior alveolar nerve can cause paraesthesia in the area innervated by this nerve. The paraesthesia is due to neurotoxic effects of the sealers. In this study we examined the effect on nerve conduction in vitro of two calcium hydroxide-containing root canal sealers, Sealapex and CRCS. An isolated rat phrenic nerve was placed between two suction electrodes in a bath containing Tyrode solution. The sealers were brought into contact with the nerve, and the evoked compound action potentials (cAP) were recorded before and after contact with the mate

The site of the lesion causing hearing loss in bacterial meningitis: a study of experimental streptococcal meningitis in guinea-pigs.

The pathological basis of hearing loss in bacterial meningitis was investigated using an animal model of Streptococcus suis meningitis. Forty guinea-pigs were infected after their hearing had been assessed by brain stem auditory evoked potentials. In 17 animals, it was possible to repeat the procedure at the onset of meningitis; this included one animal with subclinical disease. Fifteen animals showed evidence of hearing loss, which on subsequent histological examination was found to be associated invariably with suppurative labyrinthitis. The remaining two animals without hearing loss had nor

Ocular manifestations of the congenital varicella syndrome.

Three children developed the congenital varicella syndrome following a maternal varicella infection during the second trimester of pregnancy. Systemic findings included a bulbar palsy, mild hemiparesis, cicatricial skin lesions, developmental delay, and learning difficulties. Ocular findings included chorioretinitis, atrophy and hypoplasia of the optic discs, a congenital cataract, and Horner's syndrome. Electroretinograms and pattern-reversal visual-evoked potentials recorded from the four eyes with chorioretinitis were attenuated and degraded. Two of the children had been misdiagnosed as hav
