Bilateral actinomycotic blepharokeratoconjunctivitis.

Actinomycotic conjunctivitis is usually unilateral and secondary to canaliculitis. We report an unusual case of bilateral actinomycotic blepharokeratoconjunctivitis in the absence of canaliculitis. Keratitis occurred without any preceding ocular trauma. Both eyes had remissions and relapses in response to the antibiotic-steroid drops. Diagnosis of A. israelii infection was made on the basis of Gram staining, culture and biochemical characteristics. Complete excision of the conjunctival ulcer along with penicillin therapy resulted in cure. This case highlights that microorganisms of the order A

Ocular manifestation of disseminated Mycobacterium simiae infection in a cat.

Disseminated mycobacterial disease was diagnosed in an eight-year-old domestic shorthaired cat, with involvement of the skin, lungs, lymph nodes and one eye. Mycobacterium simiae was cultured from skin biopsies on solid agar and in liquid media. This organism is known to cause pulmonary, cutaneous or disseminated infection in human patients with acquired immunodeficiency syndrome but has never been encountered as a pathogen in companion animals. Combination treatment with rifampicin, enrofloxacin and clarithromycin resulted in complete clinical remission within six months, with no side effects

Ocular toxoplasmosis: a 50th anniversary tribute to the contributions of Helenor Campbell Wilder Foerster.

In 1952, Helenor Campbell Wilder (later Helenor Campbell Wilder Foerster) confirmed the growing suspicion that Toxoplasma gondii was a cause of uveitis in otherwise healthy adults by identifying the presence of parasites in eyes enucleated because of severe intraocular inflammation. Ocular toxoplasmosis was previously known to occur only in newborns with congenital T gondii infection. Her report ushered in a new era in the field of uveitis in which toxoplasmosis, rather than tuberculosis, was confirmed to be the most common cause of retinochoroiditis. Fifty years later, issues raised in her la

Central retinal vein occlusion due to herpes zoster as the initial presenting sign in a patient with acquired immunodeficiency syndrome (AIDS).

Central retinal vein occlusion (CRVO) due to herpes zoster has rarely been reported. Varicella zoster virus is a common opportunistic infection in patients with AIDS. This case report is about a 40-year-old man with herpes zoster ophthalmicus and central retinal vein occlusion of the right eye who is HIV-positive. Although the lesion resolved following treatment with intravenous acyclovir and oral steroid, the patient subsequently developed florid disc neovascularization and vitreous hemorrhage. The paper highlights CRVO as the initial presentation in an AIDS patient with herpes zoster ophthal

Ocular cysticercosis--a review of 25 cases.

OBJECTIVE: To study the clinical manifestations, management and outcome of cases of ocular cysticercosis in India. METHODS: Retrospective analysis of records of patients presenting to the ophthalmology clinics, with cysticercosis, during years 1990-98. RESULTS: A total of 25 patients had ocular cysticercosis, during the period studied. Majority could be surgically removed without residual effects. However four patients (16%) had no useful vision left in the involved eye. CONCLUSION: Cysticercosis is still endemic in India and contributes to preventable blindness. Improving sanitation and he

[Back again: the clothes louse (Pediculus humanus var. corporis)].

In two homeless men aged 38 and 32 years, who suffered from itching, infestation with body lice (Pediculus humanus var. corporis, a.k.a. clothes lice) was diagnosed. This infestation is rare in the Netherlands. In 1993 and 1994 and infection with body lice was registered 41 times in 31 patients at the clinic for homeless of the Community Health Service of Utrecht. The body louse can be seen by the naked eye. Treatment is by hygienic measures, pediculicides if necessary, and by prevention. Important to recognize is that P. humanus can be the vector of trench fever (Bartonella quintana), relapsi

Pathological findings in a cat with cryptococcosis and feline immunodeficiency virus infection.

This report describes the gross, histopathological, immunocytochemical and electron microscopic findings in a cat with systemic cryptococcosis and feline immunodeficiency virus (FIV) infection. Lymphadenopathy and cloudiness of leptomeninges were the major gross findings. Numerous cryptococcal yeasts were found in lymph nodes, brain, and lung, and were less common in the kidney and the eye. The inflammatory reaction varied in intensity and cell type (mononuclear through granulomatous) depending on the organ involved. Yeasts were mainly within phagocytes as revealed by electron microscopy. Some

[Retinal lesions in HIV seropositive patients].

118 patients with HIV infection underwent complete ophthalmologic examination to detect the presence of any eye pathology. The study was intended to show a correlation between such pathology and the total CD4 + lymphocyte count considered to be indicative of the immunologic status. The most frequently encountered lesions were cotton-wool patches, vascular congestion, hemorrhages, chorioretinitis, segmental vasculitis and optic disc pallar. Almost all the patients with ocular manifestations, also had a CD4 + count of less than 200/mm3 which suggest that ocular lesions are of bad prognostic sign

Retrobulbar optic neuritis in a patient with acute type B hepatitis.

A case is described of retrobulbar optic neuritis in a 33-year-old female with acute type B hepatitis. The ocular distress, principally affecting the left eye, followed the normalisation of liver function tests and subsided after steroid therapy. At the onset of ocular symptomatology, complement activation, involving both classic and alternative pathways, and high levels of circulating immune complexes were present. HBsAg was positive. No relapse was observed during follow-up for 3 years. An association between HBV-infection, optic neuritis and an immune complexes-mediated neurotoxic activity

Oculocerebral malformations. A reappraisal of Walker's 'lissencephaly'.

We describe an infant with multiple ocular and cerebral malformations. The eye lesions included microphthalmia, detachment and dysplasia of the retina, hypoplasia of the optic nerve, persistence of the primary vitreous, cataracts, obliteration of the anterior chamber, and vascularization of the cornea. In the brain, the salient features were microencephaly, agyria of an unusual type, cerebellar hypoplasia and dysplasia, persistent corpus pontobulbare, obliteration of the subarachnoid space, and a midline arachnoid cyst in the posterior fossa. We suggest that these lesions were the end result o

The concentration of microfilariae in the skin near the eye as a simple measure of the severity of onchocerciasis in a community and as an indicator of danger to the eye.

The parsitological and ophthalmological findings in an onchocerciasis survey in 8 villages (832 persons) and 5 primary schools (285 children) in Cameroon are reportedl The use of the coneoscleral punch/microtiter plate quantitative skin snip technique greatly facilitated this study, and its advantages over the weighed skin snip method are discussed. This simple technique could be used by paramedical personnel to detect the number of people in a community who have more than 15 microfilariae in a snip near the eye. This is shown to be a measure of the intensity of infection, and a good indicator

Paxlovid shows organ-specific and age-specific impacts on risk of developing post-acute sequelae of COVID-19.

BACKGROUND: The impact of antiviral therapies, including Paxlovid, on post-acute sequelae of COVID-19 (PASC) remains inconclusive. METHODS: We analyzed data from 19,413 patients (age > 18) from a validated PASC research cohort in New England who experienced at least one COVID-19 infection episode between January 1, 2022, and June 7, 2022, totaling 22,094 episodes. Multivariable logistic regression with inverse probability weights was used to infer the causal effects of Paxlovid treatment during acute infection and the risk of PASC overall (primary outcome), stratified by age group and organ s

Complications of minimally invasive surgery for primary open-angle glaucoma in patients with diabetic retinopathy: a retrospective cohort study.

OBJECTIVE: Minimally invasive glaucoma surgery (MIGS) has become increasingly popular for treating primary open-angle glaucoma (POAG). However, data regarding complications for patients with comorbid diabetic retinopathy (DR) are limited. This study aimed to compare complications after MIGS in POAG patients with and without DR. METHODS: This is a retrospective cohort study using the TriNetX global health network. Adult patients with POAG who underwent MIGS were identified, with one group having comorbid DR and the other without. Propensity score matching was applied, yielding 518 patients per

Quantitative comparison of mean macular thickness in COVID-19 patients versus healthy individuals using optical coherence tomography.

BACKGROUND: SARS-CoV-2 is associated with systemic inflammation, vascular dysfunction, and potential ocular involvement. While structural retinal changes have been observed in some patients, the long-term impact of COVID-19 on macular architecture remains unclear, §particularly in unvaccinated populations. OBJECTIVE: To evaluate differences in central macular thickness (CMT) between post-COVID-19 patients and healthy individuals using optical coherence tomography (OCT), with age-stratified analysis. DESIGN: A prospective case-control study conducted at a specialized ophthalmology center in Bra

Long-term outcome in corneal endotheliitis with molecular detection of herpes simplex virus 1 and human herpes virus 6: a case report.

BACKGROUND: Human herpesvirus 6B (HHV-6B) is known to cause exanthema subitem and has been detected in various ocular diseases, including keratitis, uveitis, optic neuritis, and endophthalmitis; however, the long-term outcome after the reactivation of HHV-6B has not been well-addressed. Sugita et al. previously reported the concomitant presence of HHV-6B with herpes simplex virus-1 (HSV-1) in the aqueous fluid at the onset of corneal endotheliitis. We focused on the same patient with corneal endotheliitis, in whom both HSV-1 and HHV-6B sequences were observed, and reported the clinical course
