Results of interruption of the vena cava for thromboembolic complications are reported. Greenfields filter was used in 40 cases, its introduction appearing to be a more delicate procedure than when other materials (caval clip in 40 cases and Mobin-Uddin's filter in 40 cases) were used. Results with regard to recurrence of emboli were comparable for the different materials, but Greenfield's filter becomes thrombosed less often and provokes fewer major sequelae.

At present, the Kimray-Greenfield filter appears to be the best intraluminal method for preventing thromboembolism, as it is simple, effective and innocuous. Our experience with the placement of intracaval filter devices in patients with malignancies, is reviewed. Seventy patients had either a Mobin Uddin or Greenfield Umbrella Filter placed since 1971. The indications for caval interruption in our series included contraindications to anticoagulant therapy, bleeding with anticoagulant therapy, and recurrent pulmonary embolism with anticoagulant therapy. The Mobin-Uddin vena cava filter has been used at Boston University Medical Center since 1971. The umbrella filter has been placed in 128 high-risk patients with a mean age of 60 years, 42% of whom had malignancies. The effect of sodium orthovanadate (vanadate) on kidney function and hemodynamics was tested in chloralose anesthetized, fasting cats. Vanadate given i.v. caused a reversible, marked decrease in glomerular filtration rate and urine production without significantly affecting the fractional water excretion rate. Kidney parenchymal size was estimated on urograms from 22 children with unilateral vesico-ureteral reflux (VUR), 14 children with bilateral VUR and seven children with unilateral heminephrectomy. Netilmicin is a new aminoglycoside antibiotic with pharmacological similarities to gentamicin, tobramycin and sisomicin. Fourteen of 15 patients with a severe or complicated urinary tract infection were cured by treatment with a seven day course of netilmicin. In one patient the infecting organism was not eradicated. No significant side effects were noted and no ototoxicity was detected. Renal clearance studies were performed on rats both during the administration of streptozotocin and sequentially during the following 8 days as hyperglycemia progressed.
