Although elderly patients have physiologic impairments in wound healing, their wounds should be expected to heal with the same frequency of closure as those in younger populations, albeit at a slower rate. However, compared to the general population, the elderly population has a higher incidence of chronic wounds: diabetic foot ulcers, pressure ulcers, and venous stasis ulcers. Experimental and clinical data indicate physiologically impaired healing is characterized by decreased angiogenesis and synthesis of critical growth factors. Further, compared to younger populations, the elderly have a …

Split hand/split foot malformation (SHFM), which typically appears as lobster-like limb malformation, is a rare clinical condition caused by a partial deletion of chromosome 7q. Hearing impairment sometimes accompanies syndromic SHFM cases; a case of inner and middle ear malformation with SHFM is described in this report. We conducted a genetic evaluation of this patient and found a deleted region that overlaps a previously reported locus of SHFM as well as a DFNB14 locus that can cause nonsyndromic hearing impairment by autosomal recessive inheritance.

We describe three cases of the rare combination of lateral erythema and hyperkeratosis of the fingers that typify a condition known as 'Mechanic's Hands'. The first and the third cases were unusual in that the condition was associated with cutaneous involvement of the feet and interstitial pneumonia but not with myositis, or with only mild muscular involvement, while the second case was typical, being accompanied by myositis and detectable antibodies against histidyl transferase. We propose that Mechanic's Hands can occur in association with foot lesions and interstitial pneumonia, even if it …

The biomechanical properties of plantar tissues were investigated for four older neuropathic diabetic patients and four healthy younger subjects. Indentation tests were performed at four high-pressure areas with three postures in each subject. The tissue thickness and effective Young's modulus were measured by an ultrasound (US) indentation system. The system comprised a pen-size probe having a US transducer at the tip and a load cell connected in series with it. Results showed that the plantar soft tissues of the elderly diabetic patients were significantly stiffer and thinner when compared w

The purpose of this study was to establish and characterize the relationship between foot pressure analysis and radiographic measurements in children that underwent a lateral column lengthening procedure for pes planovalgus (PPV). Eight children (13 feet) with PPV that had failed non-operative treatment underwent a lateral column lengthening procedure. Pre-operative and post-operative standing AP and lateral radiographs and foot pressure data were obtained. The relationships between the radiographic and foot pressure measurements were investigated. The findings from this study demonstrated str…

To determine the lower extremity amputation rate and the risk factors for amputation, we analysed the medical records of 147 Turkish diabetic patients who have been referred to the clinic with diabetic foot. Eleven patients (7.5%) had type 1, and 136 patients (92.5%) had type 2 diabetes mellitus. Fifty-four patients (36.7%) have undergone amputation due to diabetic foot. Femoropopliteal by-pass has been performed in 4 patients in the non-amputees group who did not have gangrene. None of the patients in the amputees group has undergone a revascularisation procedure. Considering all lower-extrem…

In this study the efficacy and safety femoral intra-arterial administration of teicoplanin in the treatment of diabetic foot infections caused by gram-positive bacteria were evaluated. Twenty-five hospitalized diabetic patients with foot ulcers or with foot ulcers and metatarsophalangeal osteomyelitis were included in the study. In the ulcers Staphylococcus aureus was present alone in 16 patients and was associated with Pseudomonas aeruginosa in 2 patients, with Candida albicans in 2, and with coagulase-negative Staphylococcus in 1 patient. In 4 patients other gram-positive bacteria were isola…

Osteomyelitis of the foot is a well-known complication of diabetes mellitus. In this study, the validity of 111In-labeled human nonspecific immunoglobulin G (IgG) scintigraphy was studied in 16 diabetic patients with foot ulcers, gangrene or painful Charcot joints. In all patients, plain radiographs, conventional bone scan images and 111In-IgG images were recorded. The results were verified by histologic examination of surgical specimens in patients who did not respond to antibiotic treatment within 2-3 wk (10 lesions) or long-term clinical follow-up of at least 6-mo (16 lesions). On the bone …

This study investigated healing rates of chronic diabetic neuropathic foot ulcerations located on the plantar surface of the forefoot (n = 30) versus those located on other parts of the foot (n = 25). Each type of ulceration was treated with total contact casting. Ulcerations in the first group were located on the metatarsal heads and toes, while ulcerations in the second group were located on the dorsum of the foot, heel, plantar arch, ankle, medial aspect of foot, and toe or transmetatarsal amputation sites. Successfully healed diabetic neuropathic foot ulcerations treated with total contact…

BACKGROUND: Viral warts are a common infectious disease and liquid nitrogen cryotherapy is one of the most common methods for treatment of these warts. Hand-foot viral warts frequently recur and reduce quality of life as well.
OBJECTIVE: To find the ideal treatment interval between cryotherapy sessions that can influence not only the cure rate but also the recurrence rate for hand-foot viral warts.
METHODS: A retrospective study was designed to compare a 2 week interval and a 3 week interval between cryotherapy sessions on hand-foot viral warts with respect to cure rate, recurrence rate, tre…

Diabetic foot ulcers (DFU) present a severe clinical challenge, marked by poor healing, chronic inflammation, and elevated amputation risk. The M2 macrophage is known to support the wound healing of DFU, and syndecan-4 (SDC4) contributes to this process. However, the upstream regulatory mechanisms involving post-translational modifications and transcriptional networks remain unclear. The related molecule levels were measured by RT-qPCR and Western blot. The M2 macrophage biomarkers (IL-10, Arg-1) were detected by ELISA kits, while the number of F4/80+CD206+ cells was quantified via flow cytome…

BACKGROUND: The use of ankle-foot orthoses (AFOs) is a mainstay in the treatment of cavovarus deformity associated with Charcot-Marie-Tooth (CMT) disease, but their ability to reduce cavovarus deformity is poorly characterized. Similarly, associations between AFO design features and degree of deformity reduction are unknown. Weightbearing CT has been used to describe CMT disease-related cavovarus deformity through both two-dimensional (2D) radiographic measures and three-dimensional statistical shape modeling (SSM). However, neither method has been used to evaluate differences in foot posture …

BACKGROUND: Diabetic foot ulcers (DFUs) represent a major and growing complication of diabetes in the Gulf and wider Middle East and North Africa (MENA) region, which has the world's highest diabetes prevalence at approximately 17.6% of adults. DFUs carry a high burden of morbidity, mortality and economic cost, and their complex multifactorial aetiology, encompassing peripheral neuropathy, ischaemia, deformity and infection, is compounded by region-specific cultural, climatic and healthcare factors.
AIMS: To develop evidence-based, Gulf-specific clinical guidelines on the assessment, manageme…

Stroke is a leading cause of severe long-term disability, and foot drop is a common post-stroke gait impairment that may reduce toe clearance, increasing the risk of toe contacts and, in turn, trip-related falls. This case series evaluated the novel combination of transcranial direct current stimulation (tDCS), functional electrical stimulation (FES) to the tibialis anterior, and physical therapy training to improve minimum toe clearance (MTC) in individuals with chronic stroke. The rationale for this combined approach was that tDCS may prime the motor cortex and enhance neuroplastic responsiv…

Predicting early cognitive decline is important for timely interventions. However, existing approaches for early detection remain limited by cost, scalability, or concerns about their validity. This study evaluated the potential of lower-extremity force control capacities, a largely unexplored area, to improve prediction of cognitive status in older adults with and without Mild Cognitive Impairment (MCI). MCI refers to an early stage of cognitive decline that does not meet criteria for dementia. 224 participants aged 80-91 years completed a foot-pedal tracking task in which they continuously a…
