A 17-year-old woman complained of a localized painful swelling in her foot 30 months after internal fixation of a closed tarsometatarsal joint fracture with interfragmentary poly-L-lactic acid screws. Aspiration revealed a sterile abscess. Radiographs displayed an osteolytic lesion corresponding to a screw track in the first tarsometarsal joint. Formation of a draining sinus tract required surgical excision of a small granulomatous lesion. Histologic analysis found deposits of birefringent polymeric particles surrounded by a nonspecific foreign-body type reaction. This represents the first rep

This was a study to compare the psychiatric consequences of closed head injury (CHI) in 37 patients with 39 demographically similar patients with fractured lower limb injury (FLLI), aged 16-55 years. The outcome measures were the Clinical Interview Schedule-Revised, the Bender Gestalt Test and the WHO AUDIT Core for alcohol abuse. The final diagnoses were made according to DSM-III-R diagnostic criteria. The findings indicated that CHI patients suffered more psychiatric consequences than FLLI controls (OR = 4.07; 95% CI = 1.30:13.14; p = 0.013). Depression and anxiety disorders were the most co

Blunt thoracic trauma are frequent and often severe. Their management is improved in the prehospital settings by the involvement of a medical team, and the admission in a specialized trauma center. Diagnostic and therapeutic steps are closely linked. They are based on the stabilization of life threatening problems and the appropriate use of recent advances in medical imaging. Endotracheal intubation and mechanical ventilation are indicated for respiratory distress. Immediate surgical management is mandatory for an hypovolemic shock related to massive hemothorax. On the contrary, multiple ribs

Four cases of complex dislocation of the index metacarpophalangeal joint prior to skeletal maturity were reviewed. Despite the generalized laxity present in these children, none of the dislocations could be anatomically reduced by closed methods. Two patients had a concomitant osteochondral fracture from the ulnar side of the metacarpal epiphysis; one subsequently developed a lesion similar to an epiphyseal osteochondroma. One patient developed ischemic necrosis leading to premature growth arrest; a shortened metacarpal, and a deformed metacarpal head. The possibility of vascular damage must b

This paper describes the design, development and early surgical experience with a stereotactic device to allow closed retrieval and interchange of intramedullary rods in children with osteogenesis imperfecta. This relatively atraumatic procedure may allow more frequent rod interchange than with other techniques, lessening the likelihood of deformity and fracture in the unsupported skeleton when the bone has outgrown the intramedullary rod. The procedure was developed by design studies in vitro followed by intramedullary rodding of tibiae of New Zealand white rabbits. It has been used in childr

Central fracture of the acetabulum is an uncommon traumatic lesion of the pelvis that is usually best managed by conservative measures. Adequate roentgenographic evaluation and closed manipulation followed by skeletal traction is essential to attain and maintain congruity of the weight-bearing dome with the femoral head. Reduction should be maintained with traction continued uninterrupted for three months. Open reduction and internal fixation may be warranted if the actabular dome elements are of sufficient size for reconstruction of the pelvic wall. An early exercise program will improve and,

The incidence of deep vein thrombosis (DVT) after foot and ankle surgery is generally believed to be low. However, little information is available regarding DVT as it specifically relates to foot and ankle trauma. The National Trauma Data Bank data set (2007 to 2009) was used to evaluate the incidence of thromboembolism in foot and ankle trauma. Also, the risk factors associated with the thromboembolic events were identified. Data regarding the demographics, comorbidities, procedures, trauma types, and complications, including DVT and pulmonary embolism (PE), were collected from the data set f

Patients with cystic fibrosis often have significant baseline endocrine comorbidities that may worsen following organ transplantation, necessitating long-term surveillance, and close follow-up. Post-transplant glycemic control is critical due to the risk of worsening cystic fibrosis-related diabetes or new-onset post-transplant diabetes mellitus associated with glucocorticoids, calcineurin inhibitors, and additional metabolic stressors. Poor glycemic control is linked to increased rates of hospitalization, graft dysfunction, rejection, morbidity, and mortality. Intensive patient education, clo

BACKGROUND: Currently Minimally Invasive Plate Osteosynthesis (MIPO) technique for tibial shaft fracture management has gained wide attention. However, an increased intracompartmental pressure after the plate insertion may result in postoperative acute compartment syndrome. We reported the difference of immediate effect of percutaneous plate insertion using 2 approaches of MIPO technique on anterior compartment pressure of the legs.
MATERIALS AND METHODS: Eight soft cadaveric legs (one female and three males) without previous history of skeletal trauma or surgery were infused with normal sali

BACKGROUND: Among diabetics, patients with peripheral neuropathy are at increased risk of developing complications following an ankle fracture. While the outcomes in these patients treated nonoperatively have been poor, the outcomes in those undergoing open reduction and internal fixation are at the best modest. We hypothesize that closed reduction and internal fixation with tibiotalocalcaneal nail is an effective primary procedure in this complication prone patient group.
METHODS: A retrospective review of diabetic patients with peripheral neuropathy who underwent acute treatment of an ankle

This case highlights the successful surgical management of a subacute subtrochanteric femur fracture encountered during a U.S. Navy humanitarian mission aboard the USNS Mercy in Asia. It underscores the clinical, logistical, and ethical challenges of providing surgical care in resource-limited environments, contributing to the growing literature on military medicine and global health ethics. A 27-year-old male presented with a 1-month history of a right subtrochanteric femur fracture that had not been reduced or stabilized due to lack of surgical resources and financial constraints. He had rem

BACKGROUND: Posterior shoulder instability is a complex and frequently underdiagnosed condition. In the context of primary manifestation, first-time (sub-)luxation (Moroder type A1) are distinguished from first-time traumatic dislocation (Moroder type A2). Posterior shoulder dislocation is particularly difficult to diagnose in emergency settings and is often overlooked.
DIAGNOSTICS: Clinical findings include a prominent coracoid and loss of active and passive external rotation. Radiographic features such as the lightbulb sign, vacant glenoid sign, or rim sign may be present, yet the diagnosis

Fatal animal-related injuries are uncommon in forensic practice and may occasionally mimic interpersonal violence, particularly in unwitnessed rural deaths. We report a fatal case of blunt force polytrauma caused by a domestic ram (Ovis aries) initially presenting as a suspicious death. A 62-year-old woman was found deceased in a field adjacent to her residence. External examination revealed numerous traumatic lesions involving the head, trunk, and extremities. Postmortem imaging and autopsy demonstrated extensive bilateral rib fractures, sternal disjunction, pulmonary contusions, hemothorax,

UNLABELLED: Discontinuation of denosumab may result in rebound bone resorption, fractures, and rarely hypercalcemia, a very uncommon manifestation. Our systematic review and case report highlight that although rare, calcium disturbances occur after treatment withdrawal. These findings emphasize the importance of sequential antiresorptive therapy and close monitoring in osteoporosis patients.
BACKGROUND: Denosumab discontinuation has been associated with rebound bone resorption, rapid bone loss, and vertebral fractures. Hypercalcemia remains exceptional in postmenopausal osteoporosis.

OBJECTIVE: Long non-coding RNA CASC15 plays a role in the development of various human diseases. However, the diagnostic role and molecular mechanisms of CASC15 in postmenopausal osteoporosis (PMOP) remain unclear.
METHODS: This study enrolled 137 healthy postmenopausal women and 141 PMOP patients. CASC15 and miR-152-3p levels in serum and HBMSCs were measured via RT-qPCR. Western blotting and RT-qPCR were employed to analyze the protein and mRNA expression levels of osteogenic markers (RUNX2, ALP, OCN, Col I). CASC15's diagnostic value was assessed by the ROC curve. Proliferation (CCK-8) and

INTRODUCTION: Neurofibromatosis Type 1 (NF1) is a genetic disorder that may lead to the development of plexiform neurofibromas (Pns). Selumetinib, a selective mitogen-activated protein kinase (MEK1/2) inhibitor, has shown clinical benefit in shrinking inoperable PNs. However, its long-term effects on the skeleton remain unclear. This case describes impaired bone healing and pathological fractures following limb lengthening in a child with NF1 receiving prolonged selumetinib therapy.
CASE REPORT: A 5-year-old boy with genetically confirmed NF1 and left lower limb PN-induced overgrowth presente

INTRODUCTION: Bilateral dislocations of the glenohumeral joint are most often posterior, secondary to convulsive seizures. We report a case of bilateral anterior dislocation with fracture of the two major tubercles following a very rare mechanism.
CASE REPORT: A 52-year-old patient admitted at the emergency department for bilateral closed shoulder trauma following a tonic-clonic seizure. The patient had a blood pressure of 160/100 mm Hg and a clear consciousness. Symmetrical and bilateral clinical examination of the shoulders shows the signs of bilateral anterior dislocation of the shoulders.
