This pilot study investigated the feasibility of a nurse-led fall prevention strategy in community-dwelling older persons. The sample included 126 subjects (mean age = 76 years) who could rise from a chair and transfer independently. During a home visit, a research nurse identified individuals at risk: a history of >or= 2 falls in the previous year or difficulties in gait and/or balance. Patients not at risk received an educational leaflet. Older persons at risk received an evaluation of risk factors for falling. Whenever problems were identified, the nurses gave specific advice and subjects w

BACKGROUND: Gait speed is an independent prognostic factor in patients with heart failure (HF) and is recognized as a therapeutic target. Although a single cutoff of 0.98 m/s has been proposed based on post-discharge prognosis, the continuous relationship between gait speed and outcomes suggests the possible presence of multiple cutoffs. Identifying these thresholds may help establish stepwise, attainable goals for a broad range of older HF patients. This study aimed to determine whether additional prognostic gait speed cutoffs exist at discharge in older HF patients.
METHODS: We conducted a

AIM: To determine whether frailty is associated with activities of daily living (ADL) and other clinical outcomes in hospitalized older adults.
METHODS: MEDLINE, Embase and CENTRAL were searched for English-language cohort studies published from 1 January 2001 to 23 October 2025. Eligible studies included hospitalized adults aged ≥ 65 years (or samples with ≥ 80% aged ≥ 65 years), used a defined frailty measure and compared frail with non-frail participants. Two reviewers independently selected studies, extracted data and assessed risk of bias using the Quality In Prognosis Studies tool. Rand

INTRODUCTION: Postoperative delirium (POD) is a common complication in elderly patients, linked to prolonged hospitalization, increased morbidities, and mortality. Midazolam, a widely used perioperative benzodiazepine, has controversial associations with POD: some studies suggest risks, while others find no significant link, but evidence quality remains low. This trial explores whether midazolam is non-inferior to placebo in POD incidence among elderly spinal surgery patients.
METHODS AND ANALYSIS: A single-center, randomized, double-blind, placebo-controlled non-inferiority trial at Ningbo N

PURPOSE: To investigate whether patient-controlled sedation (PCS) with remimazolam provides more precise sedation control and superior overall outcomes compared to conventional sedation (CS) in elderly patients undergoing lower limb surgery under spinal anesthesia.
PATIENTS AND METHODS: 120 patients aged ≥65 years (ASA physical status II~III) were randomized to receive either PCS (n=60) or CS (n=60). PCS received a loading dose (1 mg), background infusion (0.75 mg·h-1), and patient-demand boluses (0.5 mg; lockout interval 60s). CS received a loading dose (0.05~0.1 mg·kg-1) followed by a conti

1. Postoperative cognitive dysfunction (POCD) is a major issue in elderly surgical patients. Oxycodone (Oxy) reduced POCD risks versus other anaesthetics, but its mechanism is unknown. This study probes microRNA (miR)-150-5p's possible role.2. Preoperative and postoperative serum samples were obtained from elderly patients, categorised into the POCD and non-POCD groups after oxycodone anaesthesia. In vivo POCD mice and in vitro LPS-induced cell models were constructed, both treated with Oxy. miR-150-5p Levels were measured by RT-qPCR. ROC curve evaluated its diagnostic potential for POCD. Morr

PURPOSE OF REVIEW: The review aims to synthesize the current evidence on decision-making and cancer treatment planning for older adults with pre-existing cognitive impairment, Alzheimer's disease, and other related dementias.
RECENT FINDINGS: Current decision-making practices are not standardized, and evidence suggests that oncology physicians conduct burden-benefit analyses to guide treatment planning. There was a consensus on the importance of involving caregivers into the decision-making process. However, caregivers experience feelings of anxiety, uncertainty, and extra burden when decidin

BACKGROUND: Socially isolated older adults incur increased risks of adverse health outcomes, though the strength of this association is unclear. We examined whether changes in physical frailty moderated the associations between changes in social relationships and changes in health outcomes among older adults.
METHODS: This longitudinal study is based on three waves of the FRéLE study among 1643 Canadian community-dwelling older adults aged 65 years and older over 2 years. We performed latent growth curve modelling (LGMs) to assess changes with the assumption of missing not at random, adjustin

BACKGROUND: The five-time chair stand test (5CST) as an indicator of muscle strength and physical function is the first step in assessing sarcopenia. We aimed to determine the optimal cut-off value of the 5CST for assessing older adults with sarcopenia in the Chinese community.
METHODS: We used a stratified cluster random sampling method to recruit older adults from Chinese communities. The handgrip strength was assessed using an electronic handgrip dynamometer. The 5CST and gait speed were assessed by the trained researchers. The bioimpedance analysis device was used to evaluate the skeletal

The progressive aging of the population has led to a rise in geriatric pathologies, with sarcopenia, characterized by muscle mass and function loss, becoming a crucial prognostic indicator. This study investigates sarcopenia in elderly hospitalized patients with advanced chronic liver disease (cirrhotic) and non-liver disease patients, comparing their prevalence and exploring correlations with anthropometric and biochemical factors. The cohort of 115 patients, including 50 cirrhotic and 65 non-cirrhotic individuals, exhibited significant comorbidities and a mean age of 78.4 years. Cirrhotic pa

BACKGROUND AND OBJECTIVES: Early identification of risk factors and timely intervention can reduce the occurrence of cognitive frailty in elderly patients with multimorbidity and improve their quality of life. To explore the risk factors, a risk prediction model is established to provide a reference for early screening and intervention of cognitive frailty in elderly patients with multimorbidity.
METHODS: Nine communities were selected based on multi-stage stratified random sampling from May-June 2022. A self-designed questionnaire and three cognitive frailty rating tools [Frailty Phenotype (

BACKGROUND: Although the frailty index (FI) is designed as a continuous measure of frailty, thresholds are often needed to guide its interpretation. This study aimed to introduce and demonstrate the utility of an item response theory (IRT) method in estimating FI interpretation thresholds in community-dwelling adults and to compare them with cutoffs estimated using the receiver operating characteristics (ROC) method.
METHODS: A sample of 1,149 community-dwelling adults (mean[SD], 68[7] years) participated in this cross-sectional study. Participants completed a multi-domain geriatric screen fr

BACKGROUND: Frailty and sarcopenia are common in older people and are associated with adverse outcomes including increased mortality and morbidity. It is unclear whether screening for frailty and sarcopenia would identify specific populations most at risk of poor outcomes during unplanned hospital admissions, which screening tools should be used and what the trajectory of both conditions are over the course of an admission. The TYSON study is an observational cohort study aiming to determine the prevalence, trajectory and outcomes associated with frailty and sarcopenia in different patient coh
