Solid variant of aneurysmal bone cyst on the cervical spine of a child: case report, differential diagnosis and treatment rationale.

INTRODUCTION: Despite numerous descriptive publications, the nature, character, differential diagnosis and optimal treatment of aneurysmal bone cysts (ABCs), remain obscure. The authors report a case of the solid variant of aneurysmal bone cyst (S-ABC) occurring in the posterior components and body of C7 vertebra focusing on the differential diagnosis and surgical treatment rationale. CASE REPORT: Right shoulder and neck pain were the presenting symptoms of 9-year-old boy. Torticollis developed later on but no neurological deficit was found. Imaging revealed an osteolytic lesion with signific

Isolated neurosarcoidosis.

Sarcoidosis is an inflammatory granulomatous disease affecting multiple organ systems. Neurosarcoidosis (central nervous system involvement) is seen in approximately 25% of patients with systemic sarcoidosis, although it is subclinical in most of these cases. Clinical presentations and imaging findings in nervous system were varied. Cranial nerve abnormalities were the most common clinical presentation and involvement of the optic nerve in particular was associated with a poor prognosis for visual recovery. A patient is described who presented with decreased visual acuity, hypoesthesia of the

Temporal arteritis with scalp ulceration and blindness.

Temporal arteritis or giant cell arteritis is a systemic granulomatous vasculitis of medium and large-sized arteries, most frequently involving the temporal artery. It presents with headache, fever, elevated erythrocyte sedimentation rate and anemia. If untreated, complications like ischemic optic neuritis may even lead to blindness. Although very rare, scalp necrosis and ulcerations are the most common dermatologic findings. An 85-year-old woman presented with a two-month history of bilateral scalp ulcerations, followed by permanent loss of vision in the left eye due to the delay in diagnosin

Vasculitis presenting with myocardial infarction, stroke, and aortitis in a middle-aged man.

BACKGROUND: Vasculitis may cause inflammation in any single or group of blood vessels. Traditionally, giant cell arteritis involves the extracranial branches of the carotid, and Takayasu arteritis affects the aorta and its major branches. These diseases are quite rare, but have the potential to be fatal. OBJECTIVES: We describe the spectrum of overlap between these two historically distinct diseases, and use a case of a man with arteritis involving his carotid and vertebral vessels, aorta, and coronary arteries to illustrate this. We posit that large-vessel vasculitis should be considered in

Mesalazine-induced eosinophilic variant of Wegener's granulomatosis in an ulcerative colitis patient.

A 24-year-old woman presented with two weeks history of progressive shortness of breath associated with sharp chest pain. She had been on mesalazine for two years for Ulcerative Colitis. Chest X-ray showed bilateral pulmonary infiltrates with left sided pleural effusion. Blood investigations revealed, positive pANCA, negative cANCA and peripheral eosinophillia. Video assisted thoracoscopic lung biopsy specimen was consistent with eosinophillic variant of Wegener's granulomatosis. She responded to combination of withdrawal of mesalazine and high dose steroids. To our knowledge this is the first

An unusual presentation of filariasis: a case report.

BACKGROUND: Filariasis and its consequences are a major health problem in tropical countries, including the Indian subcontinent. Despite its high incidence, it is unusual to find microfilaria in fine needle aspiration cytology (FNAC) smears. We present a case of subcutaneous firm to cystic swelling, aspiration of which revealed a large number of microfilaria. CASE: A 30-year-old man presented with a chain of intermittent, firm swellings in both arms. FNAC of the swellings revealed a large number of 4 microfilariae with associated giant cell reaction and inflammatory cell-like eosinophils. CO

Fever of unknown origin: temporal arteritis presenting with persistent cough and elevated serum ferritin levels.

BACKGROUND: Fever of unknown origin (FUO) at the present time is most frequently caused by neoplasm and less commonly by infection. Currently, collagen vascular diseases (CVDs) are an uncommon cause of FUO because most are readily diagnosable by serologic methods and do not remain undiagnosed for sufficient time to present as FUOs. CVDs presenting as FUOs not readily diagnosable with specific tests include late-onset rheumatoid arthritis, adult juvenile rheumatoid arthritis, and polymyalgia rheumatica/temporal arteritis (TA). TA, or giant cell arteritis, is an uncommon arteritis of the mid- an

Does this patient have temporal arteritis?

CONTEXT: Clinicians must be able to confidently diagnose temporal arteritis (TA), since failure to make a correct diagnosis may lead to irreversible visual loss as well as inappropriate evaluation and treatment of headache, fatigue, and other potential presenting symptoms. The diagnostic value of particular signs and symptoms among patients with suspected TA is unknown. OBJECTIVE: To determine the accuracy of historical features, physical examination, and erythrocyte sedimentation rate (ESR) in diagnosis of TA. DATA SOURCES: We performed a MEDLINE search of English-language articles publishe

Temporal arteritis: a cough, toothache, and tongue infarction.

Temporal arteritis, the most common form of systemic vasculitis in adults, is a panarteritis that chiefly involves the extracranial branches of the carotid artery. The condition is illustrated in this article by the case of a 79-year-old woman with a dry cough, toothache, tongue infarction, and vision loss. The mean age of onset is 72 years and the disease rarely occurs in persons younger than 50 years. The most common presenting manifestations are headache, jaw claudication, polymyalgia rheumatica, and visual symptoms. Eighty-nine percent of patients have an erythrocyte sedimentation rate gre

[Clinical thinking and decision making in the practice. A patient with fever of unknown origin].

A 75-year-old woman was admitted because of fever of unknown origin (FUO). In the year before the current admission she developed myalgias and was treated for polymyalgia rheumatica with low-dose prednisone. Her complaints persisted and prednisone was discontinued. Five months before the present admission she developed fever (37.7-38.9 degrees C), malaise, fatigue and occipital headache. Laboratory tests showed an elevated erythrocyte sedimentation rate (98 mm in the first hour) and a severe hypochromic, slightly microcytic, anaemia. Although a recent temporal artery biopsy was negative, a sec

[Horton's disease: ophthalmological and neurological complication].

We report the case of a 74-year-old women admitted to the hospital because of rapidly developing diplopia. The admission anamnesis revealed fronto-temporal headaches and pain in the maxilla for three days. A cerebral cT scan was normal, and a corticosteroid therapy was initiated. Diagnosis of Horton's syndrome was confirmed by the good response to treatment and by biopsy of the temporal artery. Different ocular and neurologic complications of Horton's disease, their clinical appearances and therapeutic measures are discussed by the authors.

[Indications for biopsy of the temporal artery].

Indications for Temporal Artery Biopsy are often discussed in the diagnosis of temporal arteritis. In this retrospective study we have reviewed the clinical records of 150 patients who underwent temporal artery biopsy between 1983 and 1990 in Kantonsspital Aarau/Switzerland. In 18.6% biopsies were histologically classified as temporal arteritis. To judge the indication for temporal artery biopsy the combination of symptoms leading to biopsy and the histologic results were compared. Symptoms were divided in unspecific, such as fever, loss of weight or malaise, and more specific symptoms as head

[Recalcification of an 8 x 4 x 4.5 cm large brown tumor with spontaneous fracture following parathyroidectomy. Case report and course].

Primary hyperparathyroidism is typically accompanied by a vast array of symptoms and the occurrence of so-called brown tumors often localized in the metaphysis of bone. These tumors are radiologically and histologically hard to distinguish from giant cell tumors. Usually they recalcify after parathyroidectomy, except in case of genuine cysts. We report on an extended brown tumor of the tibia located just below the knee joint of a 45-year-old woman. It was a large as 8 x 4 x 4.5 cm and had been noticed after a spontaneous fracture. Complete recalcification occurred after parathyroidectomy and t

[Is the biopsy of the temporal artery necessary? A study of 204 biopsies].

Due to the controversy on the need of performing temporal artery biopsies for the diagnosis of giant cell arteritis (GCA), we have reviewed 204 biopsies of temporal arteries as well as the clinical histories of the 190 patients. Patients without local manifestations were excluded from this study. From the 173 remaining patients, 54 presented a diagnostic biopsy of GCA. The 119 patients with a negative biopsy were divided into different groups according to the final diagnosis. From our data we could not find a clinical pattern which could permit the prediction of biopsy positiveness. We could n

Temporal artery biopsy.

Though surgeons have little input in the selection of patients for temporal-artery biopsy, a knowledge of temporal-artery anatomy and the pathophysiology of temporal arteritis is important. All temporal-artery biopsies done at Carraway Methodist Medical Center between January 1980 and January 1985 were reviewed. Seventy-three biopsies were performed on 70 patients and eight (11.4%) were found to have temporal arteritis histologically. One patient was judged to have temporal arteritis clinically, despite a negative biopsy of short length. Six of eight patients with positive biopsies were female
