Rapid cardiovascular effects of dexamethasone in rabbits with meconium-induced acute lung injury.

Glucocorticoids may improve lung function in newborns with meconium aspiration syndrome (MAS), but information on the acute side effects of glucocorticoids in infants is limited. In this study using a rabbit model of MAS, we addressed the hypothesis that systemic administration of dexamethasone causes acute cardiovascular changes. Adult rabbits were treated with 2 intravenous doses of dexamethasone (0.5 mg/kg each) or saline at 0.5 h and 2.5 h after intratracheal instillation of human meconium or saline. Animals were oxygen-ventilated for 5 h after the first dose of treatment. Blood pressure,

Effects of lesions of the caudal cerebellar vermis on cardiovascular regulation in awake cats.

The vestibular system is known to participate in cardiovascular regulation during movement and postural alterations. The present study considered whether lesions of two regions of the posterior cerebellar vermis (the nodulus and uvula) that provide inputs to vestibular nucleus regions that affect control of blood pressure would alter cardiovascular responses during changes in posture. Blood pressure and heart rate were monitored in awake cats during nose-up tilts up to 60 degrees in amplitude before and following aspiration lesions of the nodulus or uvula; in most animals, cardiovascular respo

The effects of heat stress on neuromuscular activity during endurance exercise.

This study analysed the effect of hot (35 degrees C) and cold (15 degrees C) environments on electromyographic (EMG) signal characteristics, skin and rectal temperatures and heart rate during progressive endurance exercise. Eight healthy subjects performed three successive 15-min rides at 30%, 50% and 70% of their peak sustained power output and then cycled at increasing (15 W/min) work rates to exhaustion in both 35 degrees C and 15 degrees C environments. Skin and rectal temperatures, heart rate and EMG data were measured during the trials. The skin temperatures were higher and the subjects

Hand-scoring of MESAM 4 recordings is more accurate than automatic analysis in screening for obstructive sleep apnoea.

The MESAM 4 system, developed to monitor breathing sounds, heart rate, arterial oxygen saturation (Sao2) and body position, was proposed as a screening method for obstructive sleep apnoea (OSA). The aim of the study was to assess the accuracy of hand-scoring versus automatic-scoring in screening for obstructive sleep apnoea. The study population consisted of 56 patients, 51 males, and 5 females, mean age 47 +/- 10 yrs, suspected of having obstructive sleep apnoea. Full polysomnography and MESAM 4 recordings were performed simultaneously. The apnoea+hypopnoea index was hand-scored in polysomnog

Analysis of risk factors influencing imminent distress in growth-retarded fetuses undergoing oxygen test.

We previously demonstrated a prognostic significance of maternal oxygen test in predicting imminent fetal distress. The purpose of this study was to investigate eventual other factors related to the length of the time interval elapsing between the Doppler diagnosis of brain sparing effect and abnormal fetal heart rate patterns. To this end we considered 101 growth-retarded fetuses free of structural and chromosomal abnormalities with a ratio between the pulsatility indices of umbilical and middle cerebral artery above the 95th centile in presence of a normal fetal heart rate pattern. The facto

Parasympathetic overactivity and its evaluation in patients with sinus nodal dysfunction.

In 17 controls, and 17 patients with sinus nodal dysfunction, an electrophysiologic study was made of sinus nodal function and atrioventricular nodal conduction in the basal state. The study was then repeated in all patients after atropine. The heart rate, mean sinus cycle length, variations of sinus cycle length, sinus node recovery times, sinuatrial conduction time, AH interval, and atrioventricular nodal Wenckebach threshold were significantly different in patients from those of controls. All these parameters changed significantly in patients after atropine, and were comparable to those of

Pressor tests in clinical pharmacology: response morphology heterogeneity.

Heart rate, blood pressure and venous plasma catecholamine responses to passive upright tilt, immobile erect standing, upright sitting, immersion of hand or foot in cold water, isometric handgrip and delayed auditory feedback were assessed in young healthy volunteers. Each of the tests was characterized by its own typical response morphology. Postural stress caused mainly a rise in heart rate and diastolic blood pressure with little change in systolic blood pressure. The cold pressor test caused a rapid rise in heart rate, systolic and diastolic blood pressure which then was attenuated on furt

Decrease in arterial blood pressure in spontaneously hypertensive rats by the TXA2 receptor antagonist BM 13177 and by acetylsalicylic acid.

The effects of the selective TXA2 receptor antagonist BM 13177 and of the cyclooxygenase inhibitor acetylsalicylic acid on blood pressure and heart rate were investigated in anaesthetized spontaneously hypertensive rats (SHR). After intravenous injection of BM 13177 (60 mg/kg) there was a strong short-lasting decrease in arterial blood pressure (BP) with a maximum reduction of 21.6% in the third minute after injection. The heart rate was only slightly decreased. The formation of TXB2 and 6-keto-PGF1 alpha in serum, aorta thoracica, renal medulla, and renal cortex, which was determined by radio

Altered circadian blood pressure rhythm in patients with Cushing's syndrome.

The circadian blood pressure rhythm was compared in patients with Cushing's syndrome, essential hypertension, and primary aldosteronism. In patients with essential hypertension or primary aldosteronism, a clear nocturnal fall in systolic and diastolic blood pressure and heart rate was observed. This fall was seen in untreated subjects as well as in patients receiving combined treatment with a calcium antagonist, diuretic, converting enzyme inhibitor, alpha-blocker and beta-blocker, or sympatholytic drug. In these groups, there was a positive correlation between heart rate and systolic or diast

Naloxone does not modify the hemodynamic and neuroendocrine effects of clonidine in normal humans.

The relationship between central opioidergic and noradrenergic central control mechanisms of blood pressure was investigated in normal men by evaluating the interference exerted by naloxone, a specific opiate antagonist, on the cardiovascular (blood pressure and heart rate) and neuroendocrine [human growth hormone (HGH) stimulation] effects of clonidine, a centrally acting alpha-adrenergic agonist, according to two different protocols. In series 1, the effects of placebo (normal saline), clonidine (0.15 mg i.v.), and naloxone (0.4 mg i.v.) were compared with that of clonidine plus naloxone (0.

Comparative physiological responses to exercise stress.

We determined the effect of exhaustive exercise on the acid-base balance, O2 consumption, and cardiac output in dogs and pigs to examine which species was comparable with humans in its physiological response to exercise. We ran 11 dogs and 11 pigs on a motor-driven treadmill at steady-state (heart rate 75% maximum) and exhaustive (maximum heart rate) exercise levels. Measuring heart rate, cardiac output, and aortic pressure via implanted probes we obtained arterial and venous blood samples before, during, and after exercise to determine hematocrit, blood gas tensions, pH, and lactic acid level

Lack of cardiovascular effects of delta-9-tetrahydrocannabinol in chemically denervated men.

We have previously reported that 25 micrograms/kg of intravenous (i.v.) delta-9-tetrahydrocannabinol (delta-9-THC) produces marked increases in heart rate, prolongation of left ventricular ejection time corrected for heart rate (LVETc), and a shortening of the pre-ejection period in normal volunteers. Beta-adrenergic blockade partially attenuates these responses. To elucidate further the mechanism of action of delta-9-THC, we gave 10 normal volunteers 0.1 mg/kg of i.v. propranolol and 2 mg of i.v. atropine before they received 25 micrograms/kg of i.v. delta-9-THC. Systolic time intervals were

Evaluation and management of bradycardia in neonates and children.

UNLABELLED: Heart rate is commonly used in pediatric early warning scores. Age-related changes in the anatomy and physiology of infants and children produce normal ranges for electrocardiogram features that differ from adults and vary with age. Bradycardia is defined as a heart rate below the lowest normal value for age. Pediatric bradycardia most commonly manifests as sinus bradycardia, junctional bradycardia, or atrioventricular block. As a result of several different etiologies, it may occur in an entirely structurally normal heart or in association with concomitant congenital heart disease

Mechanical assist device as a bridge to heart transplantation in children less than 10 kilograms.

BACKGROUND: Despite the remarkable advances with the use of ventricular assist devices (VAD) in adults, pneumatic pulsatile support in children is still limited. We report a retrospective review of our experience in very small children (<10 kg of body weight). METHODS: Ten consecutive children weighing less than 10 kg were offered mechanical support with Berlin Heart (Berlin Heart AG, Berlin, Germany) as a bridge to heart transplant from March 2002 to March 2010. RESULTS: The median patient age was 10.4 months (38 days to 2.2 years). The median patient weight was 6.4 kg (2.9 to 10 kg). Prior

Efficacy of ivabradine in HIV-associated dilated cardiomyopathy.

Ivabradine is a pure heart rate-lowering drug that acts directly on the sinus node and does not affect atrioventricular or intraventricular conduction times, myocardial contractility or ventricular repolarization. Ivabradine is used in the treatment of adults with New York Heart Association (NYHA) class II-IV chronic heart failure (CHF) with systolic dysfunction, in sinus rhythm, with a heart rate of ≥75 beats per minute (bpm). This report describes the beneficial effect of ivabradine on clinical parameters in a HIV-infected patient with dilated cardiomyopathy presenting with NYHA class III CH

[Risk of second and third reoperation on prosthetic heart valves].

As the volume of heart valve replacements increases, more patients are undergoing repeated operations. There are many reasons for reoperations that include incomplete or imperfectly executed primary operation, infection on valve prostheses, valve related complications. Repeated operations have their own specific technical problems, and from them depends mortality and complications rate. At Vilnius University Heart Surgery Clinic from 1967 we performed 6200 heart valve replacement operations. At the same time we performed 428 reoperations for 394 patients. Two reoperations were performed for 28

Pacemaker treatment in familial amyloidosis with polyneuropathy.

Involvement of the heart is common in familial amyloidosis with polyneuropathy (FAP) and is manifested by disturbances of atrioventricular and intraventricular conduction and dysfunction of the sinus node. Pacemaker implantation was performed in 20 patients with FAP in northern Sweden between 1968 and 1983 for the following indications: complete heart block (11), second degree heart block (1), sinus node dysfunction (5), and atrial fibrillation with a slow ventricular rate (3). There was a prompt improvement of symptoms attributable to a slow heart rate. Dislodgement of the electrode occurred

Pediatric donor heart allocation in the United States, 2006-2017: Current patterns and potential for improvement.

OBJECTIVE: To understand current donor heart allocation practices for pediatric transplantation. BACKGROUND: Despite high waitlist mortality rates among pediatric patients awaiting transplant, a substantial proportion of donor hearts go unused. Analysis of UNOS match run data may identify opportunities to optimize organ utilization. METHODS: Using UNOS/OPTN data, we evaluated all match runs for pediatric (<18 years) donor hearts from 1/1/2006 to 3/31/2017. We assessed final disposition of donor hearts, reasons for donor refusal, and other match run characteristics. Variation in total offers

Using smart speakers to contactlessly monitor heart rhythms.

Heart rhythm assessment is indispensable in diagnosis and management of many cardiac conditions and to study heart rate variability in healthy individuals. We present a proof-of-concept system for acquiring individual heart beats using smart speakers in a fully contact-free manner. Our algorithms transform the smart speaker into a short-range active sonar system and measure heart rate and inter-beat intervals (R-R intervals) for both regular and irregular rhythms. The smart speaker emits inaudible 18-22 kHz sound and receives echoes reflected from the human body that encode sub-mm displacement

Left cerebral hemisphere contributions to tachycardia: evidence and recommendations.

Six topics were discussed in which research has shown a stimulus or activity to be independently related to increases in human heart rate and to left cerebral hemisphere functioning: lateralized touching, reasoning, pleasantness, talking, lateralized feedback, and resistance to persuasion. On the basis of this converging evidence, strategies were suggested for altering heart rate by use of the method of induced lateral orientation of attention as a means of selective hemisphere activation.
