OBJECTIVES: Surgical treatment of degenerated aortic bioprostheses is associated with an increased risk of morbidity and mortality, especially in elderly patients with significant co-morbidities. Left ventricular geometric remodeling and regression of hypertrophy were assessed after aortic valve replacement with a mechanical prosthesis in 37 patients with aortic stenosis and 39 with aortic insufficiency, aged 54.2 +/- 14.3 and 52.6 +/- 16.6 years, respectively. The follow-up period was 2 years. In patients with aortic insufficiency, ejection fraction increased from 54.4 +/- 3.5 preoperatively to 59.6 +/- 3.4 after 6 months and 61.7 +/- 2.7 after 2 years. In patients with aortic stenosis, ejection fraction increased from 56.6 +/- 5.1 preoperatively to 63.9 +/- 4.4 after 6 months and 71.

A patient with aortic root abscess was successfully treated with a newly developed suture technique named "circumferential transmural sutures". This suture technique provides 1)a tight attachment of the prosthesis to the aortic wall and 2)a secure healing of infection by exposure of the abscess cavity to blood flow without complicated procedures. This technique would be a useful alternative for the aortic root abscess without left ventriculo-aortic disruption.

BACKGROUND: Bacterial endocarditis is a potentially life-threatening disease associated with significant morbidity and mortality. It is estimated that one patient per 1 million will develop endocarditis after elective abortion.
CASE: A young woman with no history of cardiac defects developed acute bacterial endocarditis after elective abortion performed without appropriately timed antibiotic prophylaxis. She subsequently required surgical debridement of the tricuspid valve and replacement of the aortic valve. She experienced numerous postoperative sequelae eventually culminating in death.

A method of complete chordal preservation based on posterior transposition of anterior leaflet is presented. The anterior leaflet and chordae are completely detached from the annulus and reimplanted as a large patch under the posterior leaflet. The excess tissue without chordae is excised and the remnant with all the chordae is plicated by sutures used to implant the prosthesis. The posterior leaflet is left intact. Mitral valve myxomas are extremely uncommon, and their presence raises differential diagnosis with other pathologic conditions. We report on the case of a patient with an unspecific clinical presentation and an equivocal echocardiographic finding: mass prolapsing through the mitral valve into the left ventricle. The tumor was strongly attached by a wide pedicle, involving both the anterior and posterior mitral valve leaflets. Tumor excision was not feasible, and the mitral valve was replaced. Histologic analysis confirmed the myxomatous nature of the mass.
