Prevention of postoperative subdural fluid collections following transcortical transventricular approach.

BACKGROUND: Subdural fluid collections appear in about 39% of patients after the removal of intra- and paraventricular tumors. This extracerebral fluid collection requires surgical intervention when progressive fluid accumulation takes place. The authors retrospectively and prospectively studied the efficacy of gelfoam and fibrin adhesive in closing cortical and ependymal defects after intraventricular and/or paraventricular lesion resection to prevent the development of SFCs. METHODS: From 1999 to 2004, we used gelfoam and fibrin adhesive on the cortical and ependymal defects of 28 patients

Preperitonal pelvic packing for hemodynamically unstable pelvic fractures: a paradigm shift.

BACKGROUND: The current management of pelvic fracture patients who are hemodynamically unstable in the United States consists of aggressive resuscitation, mechanical stabilization, and angioembolization. Despite this multidisciplinary approach, our recent analysis confirms an alarming 40% mortality in these high-risk patients. Therefore, we pursued alternate therapies to improve patient outcomes. European trauma groups have suggested the technique of pelvic packing via laparotomy to directly address the venous bleeding that comprises 85% of pelvic fracture hemorrhage. We hypothesized that a mo

How to make the blockage between the nasal cavity and intracranial space using a four-layer sealing technique.

BACKGROUND: Craniofacial surgery for facial advancement or correction of severe craniofacial malformations such as orbital hypertelorism, Crouzon's disease, and Apert's syndrome may carry great risk. Postoperative infection after craniofacial surgery is a life-threatening complication. Ascending infection via nasofrontal communication in frontofacial monobloc advancement, intracranial Le Fort III osteotomy, correction of hypertelorism (intracranial approach), and acute trauma of cribriform plate can lead to life-threatening meningitis and meningoencephalitis. METHODS: A four-layer sealing tec

Sclerotherapy for gastric fundal variceal bleeding: is complete obliteration possible without cyanoacrylate?

BACKGROUND: Many studies have suggested that endoscopic obliteration using cyanoacrylate for bleeding gastric fundal varices is effective. However, serious complications by injection of cyanoacrylate into varices have also been reported. METHODS: Thirty patients with bleeding gastric fundal varices underwent endoscopic injection sclerotherapy using 5% ethanolamine oleate under fluoroscopic guidance plus infusion of vasopressin and a transdermal nitroglycerin patch. The injection of 5% ethanolamine oleate was continued until it filled the varices and their feeder veins under fluoroscopic guida

Tracheoinnominate artery fistula: a rare and often fatal complication of indwelling tracheostomy tubes.

Fistula formation between the innominate artery and the trachea is a rare but potentially catastrophic complication after tracheostomy. Although surgery is the definitive treatment of tracheoinnominate artery fistula, the responsibility for making the proper diagnosis and stabilizing the patient before surgery often falls on the personnel in the emergency department. We describe the emergency department management of a 14-year-old girl with a tracheoinnominate artery fistula. A discussion of the risk factors, diagnostic considerations, and emergency department management strategies of tracheoi

Optimal injection volume of epinephrine for endoscopic prevention of recurrent peptic ulcer bleeding.

BACKGROUND: Although the initial rate of hemostasis achieved by endoscopic epinephrine injection for peptic ulcer bleeding is high, bleeding recurs in 14.6% to 35.5% of patients. The aim of this study was to compare rates of recurrent bleeding after endoscopic injection of two different volumes of epinephrine in patients with peptic ulcer bleeding. METHODS: A total of 72 patients with peptic ulcer with active bleeding or a non-bleeding visible vessel were randomly assigned to 15 to 25 mL or 35 to 45 mL injections of a 1:10,000 solution of epinephrine. RESULTS: The two groups were similar wit

Guided tissue regeneration-based root coverage utilizing collagen membranes: technique and case reports.

Gingival recession defects have traditionally been treated with various grafting procedures. Recently, guided tissue regeneration with collagen membranes has shown promising results. This article reviews the rationale, indications, contraindications, and clinical methods for the use of bioabsorbable collagen membrane barriers. Several properties make collagen membranes attractive candidates for use as barriers in guided tissue regeneration-based root coverage procedures. These include the inhibition of epithelial migration and promotion of new connective tissue attachment; the ability to aggre

Is there a possible role for haemostasis in the development of perigraft reaction complicating the modified Blalock Taussig shunt?

The perigraft reaction is an unusual complication found in patients in whom a modified Blalock Taussig shunt has been created using a polytetrafluoroethylene graft. We found that, in two infants, consistent laboratory findings during such a perigraft reaction were hypofibrinogenemia, increased levels of thrombin-antithrombin III complex, prothrombin fragment 1 and 2 and products of degradation of fibrin. Normalization of the levels of fibrinogen produced resolution of the perigraft reaction.

Optimizing MANTA Closure and Bailout in Large-Bore Femoral Access: Computed Tomography Angiography-Derived Measurements and Rescue Strategies.

BACKGROUND: The MANTA (Teleflex, Wayne, PA, USA) facilitates hemostasis in large-bore femoral access. However, precise measurement of the common femoral artery depth is required before arteriotomy upsizing. This measurement may be unavailable in emergencies or bailout scenarios for suture-based closures. AIMS: This study aimed to evaluate computed tomography (CT) angiography-derived common femoral artery depth measurement as an alternative to the MANTA Depth Locator. METHODS: This retrospective study at MedStar Washington Hospital Center included all patients with available preprocedural ili
