Friction of total hip replacements with different bearings and loading conditions.

Metal-on-ultra-high molecular weight polyethylene (UHMWPE) total hip replacements have been the most popular and clinically successful implants to date. However, it is well documented that the wear debris from these prostheses contributes to osteolysis and ultimate failure of the prosthesis, hence alternative materials have been sought. A range of 28 mm diameter bearings were investigated using a hip friction simulator, including conventional material combinations such as metal-on-UHWMPE, ceramic-on-ceramic (CoC), and metal-on-metal (MoM), as well as novel ceramic-on-metal (CoM) pairings. Evaluation of haemodynamic flow to the hip in patients with systemic lupus erythematosus.

OBJECTIVE: One of the groups with high risk for osteonecrosis is patients with systemic lupus erythematosus (SLE). No predictive test, however, is known to detect osteonecrosis. The purpose of this study was to evaluate haemodynamic flow to the proximal femur in SLE patients. METHODS: Twenty-two SLE patients without osteonecrosis were evaluated in comparison with 15 healthy controls. Medial and lateral circumflex arteries of 44 hips in the 22 SLE patients and 30 hips in the healthy controls were examined using ultrasound equipment with colour and power Doppler capability. Arterial pulsatility

[Immunohistochemical analysis of periprosthetic osteolysis in aseptic loosening of hip arthroplasty].

AIMS: The main complication in total hip arthroplasty is aseptic loosening. Wear-induced inflammatory processes lead to periprosthetic granulation tissue, which induces bone degradation due to aseptic osteolysis. At the site of bone degradation, the cell populations have not been further analysed so far. Therefore, the present study was designed to analyse bone degradation tissue in comparison to periprosthetic granulation tissue. METHODS: Tissue specimens of 14 patients with failed modular cup components of cementless total hip arthroplasties were obtained during revision surgery. Immunohisto

Treatment of osteonecrosis of the hip: comparison of extracorporeal shockwave with shockwave and alendronate.

BACKGROUND AND PURPOSE: Extracorporeal shockwave therapy (ESWT) and alendronate are reported effective in early osteonecrosis of the femoral head (ONFH). We hypothesized that joint effects of ESWT and alendronate may produce superior results. This prospective study compared the results of ESWT and alendronate with that of ESWT without alendronate in early ONFH. PATIENTS AND METHODS: Forty-eight patients with 60 hips were randomly divided into tow groups. There were 25 patients with 30 hips in group A and 23 patients with 30 hips in group B. Both groups showed similar demographic characteristi

Operative treatment of bilateral hip dislocations in a child with metatropic dysplasia.

We present a case of bilateral hip dislocations with metatropic dysplasia. Radiographic features such as narrow thorax, dense wafer vertebral bodies, narrowing interpedicular distances, kyphoscoliosis, crescent-shaped iliac wings, dumbbell-shaped tubular bones, and inferiorly directing lesser trochanter conform to the findings of metatropic dysplasia. Hip dislocations were misdiagnosed by ultrasonographic and radiographic hip screening, owing to the severely deformed femoral heads and acetabulums. The diagnosis of hip dislocations became possible by magnetic resonance imaging. The bilateral hi

Comparison of trot kinetics between dogs with stifle or hip arthrosis.

The purposes of this study were: 1) to describe and compare the trotting gait of normal and lame dogs secondary to stifle (GONOA) or hip (COXOA) osteoarthritis (OA) using multiple ground reaction forces (GRF) parameters, and 2) to pinpoint any characteristic in gait profile ('signatures') which could help to discriminate a lameness secondary to GONOA or COXOA. Fifty-one large breed dogs with OA (19 GONOA, 32 COXOA) and 22 normal dogs were included in the study. The vertical and cranio-caudal (braking-propelling) GRF were collected. The total stance time, and for each orthogonal vector, the pea

Periacetabular osteotomy for the treatment of dysplastic hip with Perthes-like deformities.

We retrospectively evaluated 17 hips in 16 patients who underwent a periacetabular osteotomy for the treatment of dysplastic hip with Perthes-like deformities. These residual deformities were graded using the Stulberg classification system. There were three class II hips, 11 class III hips and three class IV hips preoperatively. The average age of the patients at surgery was 36.9 years and the average follow-up was 6.6 years. The average Harris hip score significantly improved from the preoperative value of 68.2 points to 91.1 points postoperatively. The average postoperative range of motion i

Operative treatment of primary synovial osteochondromatosis of the hip. Surgical technique.

BACKGROUND: Primary synovial osteochondromatosis of the hip, a rare benign condition characterized by multiple intra-articular osteochondral loose bodies and synovial hyperplasia, may result in mechanical symptoms and degenerative arthritis if untreated. The purpose of this study was to report the results of arthrotomy alone or combined with anterior dislocation of the hip to perform synovectomy and removal of loose bodies in patients with this condition. METHODS: We retrospectively reviewed a consecutive series of twenty-one patients (twenty-one hips) with primary synovial osteochondromatosi

Arthroscopic anatomy and surgical techniques for peritrochanteric space disorders in the hip.

Disorders of the lateral or peritrochanteric space (often grouped into the greater trochanteric pain syndrome), such as recalcitrant trochanteric bursitis, external snapping iliotibial band, and gluteus medius and minimus tears, are now being treated endoscopically. We outline the endoscopic anatomy of the peritrochanteric space of the hip and describe surgical techniques for the treatment of these entities. Proper portal placement is key in understanding the peritrochanteric space and should be first oriented at the gluteus maximus insertion into the linea aspera, as well as the vastus latera

Hip and sacroiliac disease: selected disorders and their management with physical therapy.

Many problems in the hip area show movement dysfunctions of the hip joint in combination with the lumbar spine, sacroiliac joint, neurodynamic structures, and the muscular systems. Muscle strain injuries pertinent to the canine hip have been reported in the iliopsoas, pectineus, gracilis, sartorius, tensor fasciae latae, rectus femoris, and semitendinosus muscles. Physical diagnoses of this type of injury require palpation skills and the ability to specifically stretch the suspected musculotendinous tissue. Treatments shall incorporate modalities, stretches, specific exercises, and advisement

Vitamin D and intact PTH status in patients with hip fracture.

INTRODUCTION: The prevalence of hypovitaminosis D in patients with acute hip fracture was examined in a population on Sado Island in Japan. There were 85 cases of hip fracture among this population in 2004, giving an overall incidence of hip fracture of 121.4 per 100,000 population per year. This study included 50 of the 85 cases, and these cases were defined as the hip fracture group. Patients older than 70 years without established osteoporosis who were admitted to the hospital on the island during almost the same period for treatment of an orthopedic condition other than a hip fracture were

Hip and buttock implants to enhance the feminine contour for patients with HIV.

The antiretroviral therapy for patients with human immunodeficiency virus (HIV) causes lipodystrophy, or a change in the distribution of body. Treatment for the facial changes is well addressed and covered in the recent literature, but female patients also report changes in their buttocks and lower limbs. There is no treatment for the lower limb deformity, but plastic surgeons can do something for the buttock. The authors propose a classification for the deformities of these patients and a new solution to improve the contour of this area and to reduce the social impact of deformity on women wi

Total hip replacement in a patient with systemic mastocytosis: a case report.

Systemic mastocytosis is a rare condition that often involves the bone marrow. We report the case of a patient with systemic mastocytosis who underwent total hip replacement. Technical difficulties encountered during the procedure included a narrow medullary canal and abnormally hard bone, later confirmed by laboratory measurements. Follow-up at five years showed a good clinical and radiological outcome.

Developmental dysplasia of the hip: definition, recognition, and prevention of late sequelae.

When a child presents with a dislocated hip after the walking age, parents are upset, pediatricians are distraught, and lawsuits often follow. The prevention of late presentation is a goal that all practitioners should strive for. However, at this time, using current diagnostic techniques, the late presentation of DDH can be minimized but not eliminated. The AAP Subcommittee on DDH has estimated that approximately 15% of DDH at birth is not detectible, even by experienced examiners or ultrasonographers. In addition to preventing the late presentation of a developmentally dislocated hip, the pr

Rate of referral of breech infants for hip ultrasound: an audit cycle.

BACKGROUND: Infants with risk factors for developmental dysplasia of the hip (DDH) should have hip ultrasounds performed shortly after birth to detect and treat the condition at an early stage. Breech presentation is associated with increased risk of DDH. We embarked on an audit cycle to assess and improve our rate of referral of breech infants for hip ultrasound. METHODS: Two retrospective audits (phases I and II) were carried out before, and after, the introduction of a new pro-forma which encouraged recognition of breech presentation at the time of the routine neonatal examination. [Developmental dislocation of the hip is still important problem--therapeutic guidelines].

The authors are describing currently important problem--developmental dislocation of the hip. Guidelines for the treatment have been given according to literature date and upon their own experience. Therapeutic suggestions for the first twelve months of life are based on the ultrasound typing--it is advised to perform nonoperative treatment (abduction devices, "over head" traction, Pavlik harnesses). During the second year of life a pause in the treatment should be adviced in order to avoid postreduction avascular hip necrosis as a very important complication. After that period surgical treatm

Posterior fracture dislocation of the hip from a Warrior turret injury.

Fracture of the femoral head after hip dislocation is a relatively rare injury and is often associated with a poor functional outcome. Most result from high impact motor vehicle accidents. We report the case of a soldier who trapped his foot in a traversing Warrior turret and suffered a severe abduction injury to his hip. He sustained a Pipkin Type I fracture-dislocation of the hip not amenable to closed reduction and required transfer to the American Hospital in Baghdad where he underwent emergency open reduction and internal fixation of the fracture. This case report illustrates an unusual m

Diagnosis and management of traumatic and atraumatic hip instability in the athletic patient.

Although relatively uncommon compared with the shoulder, hip instability can be a source of significant disability and is a commonly unrecognized injury. Hip instability can be traumatic or atraumatic in origin. Our understanding and treatment plan for hip instability due to traumatic events is well established. However, our understanding and treatment modalities for hip instability due to atraumatic events or repetitive motion in high level athletes are not as well defined. In this article, we will review the spectrum of traumatic and atraumatic hip instability and discuss the relevant anatom
