Effectiveness of Interventions to Decrease Emergency Department Visits by Adult Frequent Users: A Systematic Review.

OBJECTIVES: Frequent emergency department (ED) users are high-risk and high-resource-utilizing patients. This systematic review evaluates effectiveness of interventions targeting adult frequent ED users in reducing visit frequency and improving patient outcomes. METHODS: An a priori protocol was published in PROSPERO. Two independent reviewers screened, selected, rated quality, and extracted data. Third-party adjudication resolved disagreements. Rate ratios of post- versus pre-intervention ED visits were calculated. Data sources were from a comprehensive search that included seven databases a

Risk factors for and expression of immune and inflammatory factors in atopic dermatitis in Chinese population: A birth cohort study.

OBJECTIVE: To explore the risk factors for atopic dermatitis (AD) and disclose the relationship between immune inflammatory factors (Immunoglobulin E (IgE), interleukin (IL)-4, IL-18) and the prevalence of AD in a Chinese population. METHODS: To evaluate the risk factors for infant AD, a total of 921 mother-newborn pairs were recruited through a questionnaire survey conducted during 2009-2011. Venous blood was collected from the mothers during birth hospitalization and umbilical cord blood was collected during delivery. Thirty-five infants with AD paired with their mothers served as the patie

'Super-Enrichment' Reveals Dose-Dependent Therapeutic Effects of Environmental Stimulation in a Transgenic Mouse Model of Huntington's Disease.

BACKGROUND: Huntington's disease (HD) is caused by a tandem repeat expansion and involves progressive cognitive decline, psychiatric abnormalities and motor deficits. Disease onset and progression in HD mice can be substantially delayed by a housing environment with enhanced sensorimotor and cognitive stimulation. However, the proposed benefits of environmental enrichment (EE) are always taken in the context of 'deprived' standard housing and investigation is warranted into the graded effects of enrichment. OBJECTIVE: To assess if a higher level of environmental stimulation ('super-enrichment

Disproportionate exposure to early-life adversity and sexual orientation disparities in psychiatric morbidity.

OBJECTIVES: Lesbian, gay, and bisexual (LGB) populations exhibit elevated rates of psychiatric disorders compared to heterosexuals, and these disparities emerge early in the life course. We examined the role of exposure to early-life victimization and adversity-including physical and sexual abuse, homelessness, and intimate partner violence-in explaining sexual orientation disparities in mental health among adolescents and young adults. METHODS: Data were drawn from the National Longitudinal Study of Adolescent Health, Wave 3 (2001-2002), a nationally representative survey of adolescents. Par

Sex partnerships, health, and social risks of young men leaving jail: analyzing data from a randomized controlled trial.

BACKGROUND: Young men involved in the criminal justice system face disproportionately high rates of sexual risk behavior, drug, use, and violence. Little is known about how their involvement in sex partnerships might mitigate their unique health and social risks. This study explores whether sex partner experience protects against harmful sexual behaviors, drug problems, violence, and recidivism in 16-18-year-old Black and Latino men leaving a US jail. METHODS: Data were drawn from the Returning Educated African-American and Latino Men to Enriched Neighborhoods (REAL MEN) study conducted betwe

Angiogenesis blockade as a new therapeutic approach to experimental colitis.

BACKGROUND: Neoangiogenesis is a critical component of chronic inflammatory disorders. Inhibition of angiogenesis is an effective treatment in animal models of inflammation, but has not been tested in experimental colitis. AIM: To investigate the effect of ATN-161, an anti-angiogenic compound, on the course of experimental murine colitis. METHOD: Interleukin 10-deficient (IL10(-/-)) mice and wild-type mice were kept in ultra-barrier facilities (UBF) or conventional housing, and used for experimental conditions. Dextran sodium sulphate (DSS)-treated mice were used as a model of acute colitis.

Results of animal experiments using an undulation pump total artificial heart: analysis of 10 day and 19 day survival.

An undulation pump is a special rotary blood pump in which rotation of a brushless DC motor is transformed to an undulating motion by a disc in the pump housing attached by means of a special link mechanism. In the blood pump, a closed line between the disc and housing moves from the inlet to the outlet by this undulating disc motion, which sucks and pushes the blood from the inlet to the outlet. Because the same phenomena occurs at both sides of the disc, a continuous flow is obtained when the motor rotational speed is constant. The pump flow pattern can be easily changed from continuous flow

A comparative study of cefixime and chloramphenicol in children with typhoid fever.

Both financial disadvantage and poor housing conditions are recognised as social determinants of poorer health and health inequalities across and within countries. In the UK and some other nations, low income and housing problems in interaction manifest as 'fuel poverty'-a measure of a household's ability to keep their homes warm. In these countries, tackling fuel poverty has become a priority, not least because of the known negative effects on health. Nevertheless, there is no gold standard measurement of fuel poverty, and there is also, more importantly, scant evidence on the relationship be

Indoor mite allergens in patients with respiratory allergy living in Porto, Portugal.

We investigated the levels of mite allergens (Der p 1, Der f 1, Der 2, and Lep d 1) in dust samples from the homes of 59 patients with asthma, 36 sensitized to house-dust mites (HDM) and 23 to grass pollen (controls), living in Porto, northern Portugal. The relationship between exposure and sensitization to HDM and the influence of housing conditions on mite-allergen levels were also evaluated. Der p 1 (median 9.2 micrograms/g) and Der 2 (4.6 micrograms/g) were the main allergens, while Der f 1 and Lep d 1 levels were always < 1 microgram/g dust and undetectable in 11% and 47% of samples, resp

The entrepreneurship of survival among urban adults experiencing homelessness and mental illness.

AIMS: Using an entrepreneurship lens, this study examined the narratives of urban adults experiencing homelessness and living with mental illness, to explore strategies used for day-to-day survival. METHODS: Semi-structured qualitative interviews were conducted with 14 females, 30 males, and one individual identifying as "other," living in a mid-sized Canadian city. The average age was 39 years. Data were transcribed verbatim and analyzed using thematic analysis informed by grounded theory. FINDINGS: Participants described creative and intentional strategies for managing life on the street w

History of co-occurring disorders and current mental health status among homeless veterans.

BACKGROUND: Homeless veterans are at high risk for co-occurring disorders (COD), defined as mental illnesses that include at least one alcohol or other drug use disorder and at least one non-drug related mental disorder. However, epidemiological studies examining the prevalence of COD and associated mental health status in this population are limited. The objectives of the study were: (1) to describe a history of diagnosed mental disorders among homeless veterans admitted to a transitional housing program, and (2) to examine the associations of the prior diagnosed COD and other mental disorder

Prevalence of Homelessness in the Emergency Department Setting.

INTRODUCTION: According to the National Alliance to End Homelessness, the national rate of homelessness has been cited as 17.7 homeless people/10,000 people in the general population, and 24.8 homeless veterans/10,000 veterans in the general population. However, it is unknown what the prevalence of homelessness is in the emergency department (ED) setting. We set out to determine the prevalence of homelessness or at risk for homelessness in the ED setting. METHODS: Using a five-question screening tool derived from the U.S. Department of Housing and Urban Development, Health and Human Services

The ecological characteristics of Anopheles mosquitoes associated with low malaria transmission in Siwa oasis, Egypt.

The ecology of Anopheles sergentii and An. multicolor was investigated over eight months to examine the entomological factors maintaining the low Plasmodium vivax transmission in Siwa oasis. The two species were encountered as larvae while the dominant species, Anopheles sergentii was also collected as adults. Breeding sites were characterized for the two species in six localities. Larvae of both species were common during June and November. In Bahi El Din as an indicator village with a higher endemicity, An. sergentii females were endophilic but more common inside animal sheds than in houses,

Suicide as a wicked problem: The case for a systems approach.

ObjectiveDespite sustained investments in conventional approaches, population-level suicide rates have not improved meaningfully in Australia. Suicide's multifactorial origins - spanning mental illness, social isolation, economic hardship, housing, and cultural factors - resist the linear, clinically focused models that currently dominate prevention practices. This paper argues that the determinants of the act of suicide work as a Complex Adaptive System (CAS) and require a fundamental reorientation towards systems thinking.ConclusionThe defining properties of a CAS - feedback, delay, adaptati

Crusted Scabies leading to osteomyelitis and bilateral below-the-knee amputations - a case report.

Crusted scabies is a rare, highly contagious dermatologic condition associated with severe mite infestation, primarily affecting immunocompromised or socioeconomically vulnerable individuals. We report the case of a 38-year-old man with type two diabetes, neuropathy, and housing instability who was misdiagnosed with a hyperkeratotic skin disorder and subsequently developed bilateral foot osteomyelitis. Delayed recognition of crusted scabies led to progressive infection and ultimately necessitated bilateral below-the-knee amputations. Diagnosis was confirmed by bedside potassium hydroxide prepa

Social determinants of visceral leishmaniasis elimination in Eastern Africa.

Visceral leishmaniasis is a vector-borne, protozoan disease with severe public health implications. Following the successful implementation of an elimination programme in South Asia, there is now a concerted endeavour to replicate these efforts in Eastern Africa based on the five essential elimination pillars of case management, integrated vector management, effective surveillance, social mobilisation and operational research. This article highlights how key social determinants (SD) of health (poverty, sociocultural factors and gender, housing and clustering, migration and the healthcare syste

One Health for fascioliasis control in human endemic areas.

Fasciola hepatica and F. gigantica are liver flukes causing fascioliasis, a worldwide zoonotic, complex disease. Human infection/reinfection occurs in endemic areas where preventive chemotherapy is applied, because of fasciolid transmission ensured by livestock and lymnaeid snail vectors. A One Health control action is the best complement to decrease infection risk. The multidisciplinary framework needs to focus on freshwater transmission foci and their environment, lymnaeids, mammal reservoirs, and inhabitant infection, ethnography and housing. Local epidemiological and transmission knowledge

Improving Access to Legal Gender Affirmation for Transgender Women Involved in the Criminal-Legal System.

Transgender women of color experience interlocking systems of oppression rooted in racism and transphobia, which fuel economic vulnerability and overrepresentation in the criminal-legal system. Legal gender affirmation, which refers to changing one's name and gender marker on official documents, has the potential to mitigate these issues by improving access to employment, housing, education, health care, and social services. These services are particularly important for transgender women of color with criminal records, a history of incarceration, or other legal infractions; however, 23 states
