Distribution of Vitamin D Metabolites in Early Childhood: Findings from a Multicenter Belgian Cross-Sectional Study.

INTRODUCTION: Vitamin D metabolism is essential for calcium and phosphate homeostasis, yet reference values for its key metabolites in early childhood are lacking. This study aimed to establish percentile-based intervals for 1,25-dihydroxyvitamin D [1,25(OH)₂D], 24,25-dihydroxyvitamin D [24,25(OH)₂D], and the Vitamin D Metabolite Ratio (VMR= 24,25(OH)₂D/25(OH)D x 100%) in healthy, well vitamin D-supplemented children under 2 years of age, providing a benchmark for disorders such as idiopathic infantile hypercalcemia. METHODS: This cross-sectional study analyzed blood samples from 208 healthy

Protective Effects of Vitamin D Against Doxorubicin Chemotherapy-Induced Hepatotoxicity in Wistar Albino Rats: Evidence from 99mTc-Pyrophosphate Scintigraphy and Oxidative-Inflammatory Pathways.

Objectives: Doxorubicin, a widely used chemotherapeutic agent, is known to induce hepatotoxicity through oxidative stress and inflammatory pathways. Vitamin D has been reported to exert antioxidant and immunomodulatory effects; however, its potential protective role in doxorubicin-induced liver injury remains insufficiently characterized. Materials and Methods: Adult male Wistar albino rats were randomly assigned to six groups (n = 7): Control, Vitamin D (5000 IU/kg), Vitamin D (60,000 IU/kg), Doxorubicin, DOX + Vitamin D (5000 IU/kg), and DOX + Vitamin D (60,000 IU/kg). ALPL Mutations With Dominant-Negative Effect in Infantile Hypophosphatasia Monozygotic Twins.

BACKGROUND AND AIMS: Hypophosphatasia (HPP) is a rare inborn error of metabolism caused by ALPL gene mutations, resulting in deficient tissue-nonspecific alkaline phosphatase (ALP) activity. We investigated genotype-phenotype correlations in a monozygotic female twin pair with infantile HPP. METHODS: Peripheral blood samples were collected from two female twins with HPP and their family members. Genomic DNA was extracted, and variations were detected using whole exome sequencing. Zygosity was confirmed via KING 2.3.1 software. Pathogenic variants were validated using Sanger sequencing, mutati

The Impact of Nutritional Status on Survival and Development of Sarcoidosis: A Scoping Review of Current Evidence and Research Gaps.

Background: Sarcoidosis is a heterogeneous, multisystem inflammatory disease with an unpredictable clinical course and limited prognostic markers. Increasing attention has focused on nutritional and metabolic factors-particularly obesity, body composition, and calcium-vitamin D metabolism-as potentially modifiable elements associated with disease development and clinical phenotype. However, the available literature remains fragmented and methodologically heterogeneous. Objective: To systematically map current evidence on the relationship between nutritional status and the development, clinical

Global surge in hyperparathyroidism: Trends in incidence, prevalence, and parathyroidectomy rates.

INTRODUCTION: The incidence and prevalence of primary hyperparathyroidism (PHPT) are known to be increasing, yet few studies have explored these trends on a global scale. This study aims to examine worldwide trends in PHPT incidence, prevalence, and parathyroidectomy rates, while identifying potential factors contributing to the observed rise in PHPT cases. METHODS: A retrospective analysis of the TriNetX Global database was conducted from 1998 to 2023 to assess the incidence and prevalence of PHPT. Patients with renal hyperparathyroidism or other secondary causes of hypercalcemia were exclud

No overshoot of bone turnover after withdrawal of denosumab treatment of adults with Langerhans cell histiocytosis: a prospective clinical trial.

UNLABELLED: Denosumab (Dmab) discontinuation in osteoporosis leads to overshoot (rebound) of bone turnover, but its cause remains largely unclear. In a prospective trial, Dmab-treated Langerhans cell histiocytosis (LCH) patients showed no overshoot of bone turnover markers despite high short-term dosing. Findings suggest that total Dmab dose does not drive the overshoot of bone turnover markers. PURPOSE: In patients with osteoporosis, the length of treatment with denosumab (Dmab) is an important risk factor for the overshoot (rebound) of bone turnover markers following its discontinuation. Postoperative Denosumab Therapy in Aneurysmal Bone Cysts and Osteoblastomas: A Case Series and Review of Current Literature.

Denosumab, a monoclonal antibody targeting receptor activator of nuclear factor-kappa B ligand (RANKL), has demonstrated efficacy in giant cell tumors of bone and has been increasingly explored as a treatment option for aneurysmal bone cysts (ABCs) and osteoblastomas. While its use as a neoadjuvant therapy has been documented, its postoperative role remains unclear. This study presents two cases in which denosumab was administered after surgical resection to evaluate its potential for recurrence prevention and bone remodeling. The first patient, a 36-year-old female, was diagnosed with a spina

PTH1 receptor agonists for fracture risk: a systematic review and network meta-analysis.

Osteoporosis, defined by reduced bone mineral density and macro- and micro-architectural degradation, leads to increased fracture risk, particularly in aging populations. While randomized controlled trials (RCTs) demonstrate that PTH1 receptor agonists, teriparatide and abaloparatide, are effective at reducing fracture risk, real-world evidence (RWE) remains sparse. This study reviews and compares the anti-fracture efficacy of these agents, against each other and against other osteoporosis treatments using both RCTs and RWE. We systematically searched Medline, Embase, and Cochrane up to May 20

Burosumab in adults with X-linked hypophosphatemia: real-world experience from a retrospective study in Sydney.

X-linked hypophosphatemia (XLH) is a chronic disabling hereditary musculoskeletal disorder associated with inactivating PHEX mutations and elevated circulating FGF-23 concentrations. In a placebo-controlled trial of adults with XLH, burosumab (anti-FGF-23 antibody) demonstrated durable improvements in phosphate concentrations, and self-reported stiffness and physical limitation. However, real-world data regarding burosumab efficacy and tolerability in adults with XLH are lacking. A retrospective audit was performed of patients (age ≥18-years) who commenced 4-weekly subcutaneous burosumab for X

Current Controversies on Adequate Circulating Vitamin D Levels in CKD.

Management of secondary hyperparathyroidism (SHPT) in chronic kidney disease (CKD) has evolved dramatically over the past five decades, driven by discoveries that have fundamentally reshaped our understanding of the vitamin D endocrine system and its role in disease progression. This review synthesizes the key pathophysiological insights and clinical evidence underlying three critical paradigm shifts. The first shift moved beyond simple calcitriol replacement with the development of selective vitamin D receptor activators (VDRAs) designed to minimize hypercalcemia while maximizing PTH suppress

G protein-coupled receptor (GPCR) gene variants and human genetic disease.

Genetic variations in the genes encoding G protein-coupled receptors (GPCRs) can disrupt receptor structure and function, which can result in human genetic diseases. Disease-causing mutations have been reported in at least 55 GPCRs for more than 66 monogenic diseases in humans. The spectrum of pathogenic and likely pathogenic variants includes loss of function variants that decrease receptor signaling on one extreme and gain of function that may result in biased signaling or constitutive activity, originally modeled on prototypical rhodopsin GPCR variants identified in retinitis pigmentosa, on

HTLV-1 infected T cells cause bone loss via small extracellular vesicles.

Adult T cell leukaemia (ATL), caused by infection with human T- lymphotropic virus type 1 (HTLV-1), is often complicated by hypercalcemia and osteolytic lesions. Therefore, we studied the communication between patient-derived ATL cells (ATL-PDX) and HTLV-1 immortalized CD4+ T cell lines (HTLV/T) with osteoclasts and their effects on bone mass in mice. Intratibial inoculation of some HTLV/T leads to a profound local decrease in bone mass similar to marrow-replacing ATL-PDX, despite the fact that few HTLV/T cells persisted in the bone. To study the direct effect of HTLV/T and ATL-PDX on osteocla
