Anterior Cruciate Ligament Transection-Induced Cellular and Extracellular Events in Menisci: Implications for Osteoarthritis.

BACKGROUND: The meniscus plays an important role in knee joint diseases such as osteoarthritis (OA). Meniscal injuries can be accompanied by joint catabolic events initiated by inflammation, leading to articular cartilage destruction, but the cellular events responsible for intrinsic meniscal injury and the extracellular matrix changes necessary for meniscal degradation are not well known. PURPOSE: To explore the cellular and matrix-related changes of menisci based on a mouse OA model of anterior cruciate ligament transection (ACLT). STUDY DESIGN: Controlled laboratory study. METHODS: A mou

Association of Anterior Cruciate Ligament Width With Anterior Knee Laxity.

CONTEXT: Greater anterior knee laxity (AKL) has been identified as an anterior cruciate ligament (ACL) injury risk factor. The structural factors that contribute to greater AKL are not fully understood but may include the ACL and bone geometry. OBJECTIVE: To determine the relationship of ACL width and femoral notch angle to AKL. DESIGN: Cross-sectional study. SETTING: Controlled laboratory. PATIENTS OR OTHER PARTICIPANTS: Twenty recreationally active females (age = 21.2 ± 3.1 years, height = 1.66.1 ± 7.3 cm, mass = 66.5 ± 12.0 kg). MAIN OUTCOME MEASURE(S): Anterior cruciate ligament width

Reliability and validity of using the Lokomat to assess lower limb joint position sense in people with incomplete spinal cord injury.

BACKGROUND: Proprioceptive sense (knowing where the limbs are in space) is critical for motor control during posture and walking, and is often compromised after spinal cord injury (SCI). The purpose of this study was to assess the reliability and validity of using the Lokomat, a robotic exoskeleton used for gait rehabilitation, to quantitatively measure static position sense of the legs in persons with incomplete SCI. METHODS: We used the Lokomat and custom software to assess static position sense in 23 able-bodied (AB) subjects and 23 persons with incomplete SCI (American Spinal Injury Assoc

Quadriceps Tendon Repair Using All-Suture Suture Button Anchors.

Traumatic quadriceps tendon ruptures are devastating injuries that hinder knee extension and drastically affect lower extremity function. In complete ruptures resulting in full extensor mechanism disruption, early diagnosis and surgical intervention are key to avoid tendon retraction, restore function, and limit morbidity and disability. Surgical management in the acute setting consists of direct repair with transosseous bone tunnels or suture anchors. Advantages of suture anchor repair include decreased operative time as well as the employment of smaller incisions. Additionally, studies have

Limited evidence for graft selection in pediatric ACL reconstruction: a narrative review.

Anterior cruciate ligament reconstruction is the preferred treatment to anterior cruciate ligament injury. With the increase in anterior cruciate ligament injuries in both adults and skeletally immature patients comes the need for individualized anterior cruciate ligament reconstruction graft selection whether it is the type of graft (auto vs. allograft) or the harvesting site (hamstrings, iliotibial band, quadriceps, patella). Several factors need to be considered preoperatively in order to optimize the patients' recovery and restore anterior cruciate ligament strength and function. These inc

Misdiagnosed cartilaginous PCL avulsion in young children.

Posterior Cruciate Ligaments injuries are rare in children and usually due to bony avulsion fractures or midsubstance tears. This study focused on cartilaginous avulsions initially misdiagnosed despite of MRI assessment. Two 6-year-old boys had cartilaginous avulsion fracture injury at the femoral attachment of the PCL. One had associated medial meniscal lesion and was reinserted. The other conducted to non-union. MRI second lecture reveals an original description with nail-biting sign on cartilage surface of anterior notch, and a close PCL angle without anterior tibial translation. No bone br

Alternative All-Inside Technique of Repairing a Vertical Meniscus Tear.

Lateral meniscus vertical tears are often seen with acute anterior cruciate ligament injuries. The inside-out technique of repairing the meniscal tear is currently the gold standard treatment. However, every technique has its own drawbacks. The inside-out technique has possible risk of infection and neurovascular complications. All-inside techniques are becoming increasingly popular. A novel all-inside technique and a variation of it for meniscus repair using an antegrade suture passer like a Knee Scorpion are described. These are cost effective using a 2-0 FiberWire for repair of vertical men

Intrinsic Healing of the Anterior Cruciate Ligament in an Adolescent.

The anterior cruciate ligament (ACL) is an important ligamentous stabilizer of the knee. The natural primary healing potential of the ACL has been shown to be extremely poor. We report the case of a 12-year-old boy who was involved in a motor vehicle collision and sustained a grade IIIB open tibial fracture, distal fibula fracture, medial and lateral meniscus tears, avulsion fracture of the femoral metaphysis, and anterior and posterior cruciate ligament tears. Because of delayed union of his tibia fracture, treatment of the ligamentous injuries was postponed to 16 months after initial injury.

Delayed diagnosis of traumatic hip dislocation mimicking Perthes disease in a child.

Traumatic hip dislocations in children are uncommon, yet even trivial injuries may result in dislocation. Avascular necrosis (AVN) occurs in 3-6% of dislocations if reduction is performed within the first 4 h, however, the incidence rises to 66% if the reduction is performed 24 h after dislocation. Awareness and early identification is therefore critical to long term prognosis. The authors report a case of a relatively trivial trauma resulting in hip dislocation in a 5-year-old boy. This case is useful in highlighting that (a) hip dislocation may occur in children with relatively trivial mecha

Ankle dislocation.

From 1980 to 1991, 14 patients with pure ankle dislocation unassociated with fracture were identified. All patients were young adults (18-41 years of age) with a male preponderance (11 of 14). The cause of injury in nine of the 14 cases was a motor vehicle accident and in the remainder, sporting events or fall from a height. Thirteen of the 14 injuries were open, and 12 patients underwent lateral ligamentous repair. Twelve of the 14 patients were available for follow-up, which ranged from 15 to 122 months. Two patients had poor results; the remainder (10 of 12) had good and excellent results.

A new consideration in athletic injuries. The classical ballet dancer.

The professional ballet dancer presents all of the problems of any vigorous athlete. The problems include osteochondral fractures, fatigue fractures, sprains, chronic ligamentous instability of the knee, meniscal tears, impingement syndrome, degenerative arthritis of multiple joints and low back pain. Attention to minor problems with sound conservative therapy can avoid many major developments and lost hours. Observations included the extraordinary external rotation of at the hip without demonstrable alteration in the hip version angle and hypertrophy of the femur, tibia and particularly the s

Sports injuries among male futsal players in Herat, Afghanistan: a cross-sectional study.

BACKGROUND: Futsal involves repeated high-intensity movements, rapid direction changes, and frequent player contact, all of which may contribute to injury risk. Evidence on futsal-related injuries from low-resource and post-conflict settings remains limited. This study aimed to describe the epidemiology and correlates of self-reported futsal-related injuries among male league players in Herat, Afghanistan. METHODS: A descriptive cross-sectional study was conducted among 229 active male futsal players registered in the Premier League and First Division leagues in Herat. Participants were recru

Anterior Cruciate Ligament Reconstruction and Mental Health: A Bidirectional Relationship.

BACKGROUND: New-onset mental health (MH) conditions are a concern for patients undergoing anterior cruciate ligament (ACL) reconstruction. Patients with pre-existing MH conditions have been observed to experience poorer postoperative outcomes. HYPOTHESIS: Patients with pre-existing MH diagnoses would report poorer outcomes after ACL reconstruction, and non-sport-related injuries would be associated with a greater incidence of new MH diagnoses. STUDY DESIGN: Retrospective cohort study. LEVEL OF EVIDENCE: Level 3. METHODS: Adults undergoing ACL reconstruction between October 2016 and October

Posterior Tibial Slope Characteristics and Their Relationships With Thigh Muscle Morphology, Activation Patterns, and Dynamic Knee Valgus in Male and Female Soccer Players Before and After Fatigue: A Descriptive Laboratory Study.

BACKGROUND: Soccer players are at high risk of anterior cruciate ligament injury as well as reinjury, with higher rates in women and in the supporting leg, and fatigue impairs the neuromuscular control of the knee joint. However, little is known about the bony anatomy of the knee joint and its relationship with neuromuscular measures related to knee stabilization, and studies investigating anatomic and biomechanical measures simultaneously are widely lacking. PURPOSE: To investigate (1) whether posterior tibial slope (PTS) characteristics and thigh muscle morphology differ between sexes; (2)

Validation of the KOOS, JR: A Short-form Knee Arthroplasty Outcomes Survey.

BACKGROUND: Medicare is rapidly moving toward using patient-reported outcome measures (PROMs) for outcomes assessment and justification of orthopaedic and other procedures. Numerous measures have been developed to study knee osteoarthritis (OA); however, many of these surveys are long, disruptive to clinic flow, and result in incomplete data capture and/or low followup rates. The Knee injury and Osteoarthritis Outcome (KOOS) physical function short-form (KOOS-PS), while shorter, ignores pain, which is a primary concern of patients with advanced knee OA. QUESTIONS/PURPOSES: Our objective was t

The anatomy and morphometry of the proximal semimembranosus muscle tendon - a cadaveric study with clinical implications.

INTRODUCTION: The semimembranosus muscle (SM) is a key component of the posterior thigh and an integral part of the hamstring group. It plays a crucial role in lower limb movement, particularly in knee flexion, hip extension, and medial rotation of the tibia (with the knee flexed), in cooperation with other muscles of the hamstring complex. The aim of this study was to assess the anatomy and morphometry of the SM proximal tendon. MATERIALS AND METHODS: The proximal tendon of the semimembranosus muscle was carefully dissected and examined in 60 cadaveric specimens (32 left and 28 right) obtain
