Clinical impact of chronic renal insufficiency on endovascular aneurysm repair.

Endovascular aneurysm repair of abdominal aortic aneurysms has become a viable alternative to open repair. A significant proportion of this patient population has chronic renal insufficiency. The surgical outcomes associated with endovascular repair in 342 patients, with and without chronic renal insufficiency, are reported. Perioperative mortality, length of admission, length of intensive care unit admission, and rates of acute renal failure, congestive heart failure, myocardial infarction, conversion to open surgery, progression to hemodialysis, and incidence of endoleaks were retrospectivel

Outcomes and Safety Among Patients With Obstructive Sleep Apnea Undergoing Cancer Surgery Procedures in a Freestanding Ambulatory Surgical Facility.

BACKGROUND: Patients with obstructive sleep apnea (OSA) may be at increased risk for serious perioperative complications. The suitability of ambulatory surgery for patients with OSA remains controversial, and several national guidelines call for more evidence that assesses clinically significant outcomes. In this study, we investigate the association between OSA status (STOP-BANG risk, or previously diagnosed) and short-term outcomes and safety for patients undergoing cancer surgery at a freestanding ambulatory surgery facility. METHODS: We conducted a retrospective analysis of all patients h

Noninvasive pressure support ventilation: physiological and clinical results in patients with COPD and acute respiratory failure.

Noninvasive ventilation may bring considerable benefits in the treatment of acute respiratory failure, especially in patients with chronic obstructive pulmonary disease. Reduction in the need for endotracheal intubation has been well-documented. In selected groups a reduction in morbidity, mortality, and length of hospital stay have been demonstrated. The efficacy of the technique is very dependent on the skill and motivation of the team.

Selected perinatal factors and child abuse.

A matched pair case-control study was conducted to compare birth weights, gestations, and length of hospital stay (LOS) of 532 children subsequently reported as physically abused with comparable data from a group of control children matched on age, sex, race, and maternal education. Our results indicated a moderate association between both low birthweight and very short gestation with physical abuse. Results for LOS were inconclusive.

Enteral Nutrition Delivery Methods and Clinical Outcomes in Critically Ill Patients: A Retrospective Cohort Study.

BackgroundAdequate enteral nutrition is essential in critical illness, yet optimal delivery strategies remain debated. Rate-based and volume-based are commonly used, with prior studies suggesting improved caloric delivery with volume-based. However, real-world clinical outcomes, particularly with mixed protocols and in COVID-19 patients, are unclear. We hypothesized that volume-based would improve caloric delivery versus rate-based or mixed, and that critically ill COVID-19 patients would achieve a lower goal intake.MethodsThis single-center retrospective cohort study included ICU patients (20

Assessment of Adverse Events Using the Therapy-Disability-Neurology (TDN) Grading System in a Cohort of Aneurysmal Subarachnoid Hemorrhage Patients: A Single-Center Retrospective Cohort Study.

BACKGROUND/OBJECTIVES: Adverse events (AE) associated with neurosurgical interventions can cause neurological deficits and impaired functional outcomes. The Therapy-Disability-Neurology (TDN) grade classifies AE severity based on treatment requirements, disability, and neurological deficits, but has not been validated in aneurysmal subarachnoid hemorrhage (aSAH). We aimed to validate the TDN grade in predicting functional outcomes and length of hospital stay (LOS) in aSAH patients, treated surgically and/or endovascularly. METHODS: We conducted a single-center retrospective cohort study of a

Effects of intraoperative hypothermia on patients undergoing laparoscopic surgery: A retrospective cohort study.

OBJECTIVE: This study aims to explore the association between intraoperative hypothermia and outcomes in adult patients undergoing laparoscopic surgery. METHODS: A retrospective analysis of 2048 adult laparoscopic surgery patients treated between 2020 and 2021 was conducted at Songklanagarind Hospital, Thailand. Intraoperative hypothermia, defined as a core temperature below 36°C, was recorded as either one or more than one episode. Patient demographics, clinical information and postoperative outcomes were extracted from the hospital information system. The outcomes were intraoperative and po

A comparative retrospective analysis on robot-assisted laparoscopic surgery compared to conventional laparoscopy in case of myomectomy: experience in a third-level hospital of Southern Italy.

Uterine myomas are the most common gynecological disease in reproductive-aged women, present several symptoms, and require effective medical and/or surgical strategies. This study aimed to compare robotic-assisted laparoscopic myomectomy (RALM) with laparoscopic myomectomy (LM) in terms of operative times, intraoperative estimated blood loss, pre- and post-hemoglobin levels drop, and length of hospital stay. Data from 50 clinical records (25 RALM in Group A and 25 LM in Group B) of patients with uterine fibroids were collected from December 2022 to December 2023 at Gynecological Unit of DAI Ma

Implementation of an enhanced recovery program in pediatric laparoscopic colorectal patients does not worsen analgesia despite reduced perioperative opioids: a retrospective, matched, non-inferiority study.

BACKGROUND AND OBJECTIVES: Enhanced recovery protocols (ERPs) decrease length of stay and postoperative morbidity, but it is important that these benefits do not come at a cost of sacrificing proper perioperative analgesia. In this retrospective, matched cohort study, we evaluated postoperative pain intensity in pediatric patients who underwent laparoscopic colorectal surgeries before and after ERP implementation. METHODS: Patients in each cohort were randomly matched based on age, diagnosis, American Society of Anesthesiologists classification, and surgical procedure. The primary outcome was

Do All Patients With Cerebral Palsy Require Postoperative Intensive Care Admission After Spinal Fusion?

STUDY DESIGN: Retrospective review of a prospective cohort. OBJECTIVE: To identify patient and surgical factors that alter the length of postoperative intensive care unit (ICU) stays after spinal fusion/instrumentation in patients with neuromuscular scoliosis secondary to cerebral palsy (CP). SUMMARY OF BACKGROUND DATA: High perioperative complication rates in patients with CP contribute to the practice of utilizing the ICU postoperatively for monitoring. However, this is costly and little is known regarding which patients truly need this increased acuity of care. METHODS: A prospective, mu

Hospitalization costs associated with homelessness in New York City.

BACKGROUND: Homelessness is believed to be a cause of health problems and high medical costs, but data supporting this association have been difficult to obtain. We compared lengths of stay and reasons for hospital admission among homeless and other low-income persons in New York City to estimate the hospitalization costs associated with homelessness. METHODS: We obtained hospital-discharge data on 18,864 admissions of homeless adults to New York City's public general hospitals (excluding admissions for childbirth) and 383,986 nonmaternity admissions of other low-income adults to all general
