In many skin diseases, itching and scratching is a vicious circle, which prolongs the disease. The aim of this study was to investigate the mechanisms which make itching skin diseases more chronic. The patients consisted of seven diagnostic groups--79 inpatients all together. We reviewed longitudinal nail biopsies performed at Henry Ford Hospital on patients with acquired nail dystrophies to see whether the procedure did, indeed, provide useful diagnostic information and to see which microscopic features were most helpful in histopathologic diagnosis. The oral ulcerations caused by aphtous lesions, herpetic lesions, candidiasis, ulcerative lichen planus, mucous membrane pemphigoid, and pemphigus vulgaris are managed in a step-up approach that can involve topical, intarlesional, and systemic pharmacologic management. This article reviews the common treatment agents, modalities, and dosages. The emphasis is on local pharmacologic therapies, yet systemic conditions that often present with such oral lesions are briefly reviewed, along with the appropriate management.

Diagnosis of vulval inflammatory disease is difficult. In this study, we reviewed 31 vulval biopsies from 23 patients with clinical follow-up. We devised 2 scoring systems from recent publications to determine whether these could help to distinguish between lichen sclerosus (LS) and lichen planus (LP). We found that scoring systems could help in distinguishing LS from LP but that they were no better than using some select pathologic criteria, and were much more time-consuming. Mucocutaneous diseases affecting the oral cavity are predominantly immune mediated or inflammatory. Three of the most common and clinically significant conditions are reviewed in this article: lichen planus, pemphigus vulgaris, and mucous membrane pemphigoid. Because oral manifestations may be the first or only manifestation of mucocutaneous diseases, oral health professionals play a critical role in the early diagnosis and multidisciplinary management of these conditions.

Lipidomics is a term used to define the field that analyzes the structure, functions, and interactions of lipids. Inflammatory dermatoses and lipid disturbances are interrelated, especially due to chronic inflammatory conditions. This review discusses lipidomics in selected inflammatory skin diseases: psoriasis, lichen planus, and atopic dermatitis, as well as the less commonly mentioned hidradenitis suppurativa, rosacea, and acne vulgaris.
