Reconstruction of the Achilles tendon using LARS artificial ligament in bilateral xanthoma: a case report and literature review.

BACKGROUND: Achilles tendon xanthoma (ATX) is a rare benign lesion characterized by lipid deposition within the tendon and is often associated with hyperlipidemia and genetic diseases. Surgery is challenging due to the risk of recurrence and its impact on patients' quality of life. Reconstruction using a synthetic ligament has not been previously reported for this condition. CASE PRESENTATION: A 35-year-old female patient presented with a progressive mass in both heels over 4 years, accompanied by limited ankle dorsiflexion (5°-10°), a Visual Analogue Scale (VAS) score of 6, and an American O

Transtibial Anterior Cruciate Ligament Reconstruction Using a Synthetic Graft Combined With Remnant Repair: A Hybrid Technique.

The purpose of this technical note is to describe a hybrid arthroscopic technique that combines anterior cruciate ligament reconstruction using a synthetic graft with concomitant remnant repair. Following diagnostic arthroscopy and assessment of remnant quality, tibial and femoral tunnels are created using a transtibial approach with attention to graft isometry. Anterior cruciate ligament remnant fibers are selectively preserved and repaired using a cinch-loop suture followed by an additional encircling stitch to form a sleeve-like construct. A synthetic ligament is then passed through the pre

Effects of anterior cruciate ligament resection and total knee arthroplasty on lower limb alignment and tibiofemoral rotation.

BACKGROUND: The anterior cruciate ligament (ACL) is a key structure that restricts excessive anterior translation of the tibia and maintains rotational stability. This study aims to dynamically and quantitatively assess the direct effects of ACL resection and total knee arthroplasty (TKA) on lower limb alignment and tibiofemoral joint rotation under passive, unloaded conditions and in the pathological state of osteoarthritis. METHODS: This retrospective study included 110 patients with varus knee osteoarthritis who underwent Mako for robotic-assisted TKA. Dynamic intraoperative measurements o

Effects of Quadriceps Strength Recovery in the First Year After Anterior Cruciate Ligament Reconstruction on Gait Biomechanics.

BACKGROUND: Aberrant walking biomechanics, a factor linked to knee osteoarthritis (KOA), are common after anterior cruciate ligament (ACL) injury. Although quadriceps weakness often persists after ACL reconstruction (ACLR), the extent to which strength recovery influences gait biomechanics over the first postoperative year, a critical window for implementing rehabilitation strategies, remains unclear. HYPOTHESIS: Distinct quadriceps strength recovery trajectories will emerge after ACLR, with the lesser strength recovery group exhibiting more aberrant gait biomechanics compared with the greate

Combining Anterior Talofibular Ligament Ultrasonography With Anterior Drawer Displacement Refines Stratification of Chronic Ankle Instability.

Chronic ankle instability (CAI) is commonly defined using symptom-based criteria, but these do not explicitly incorporate structural instability after lateral ankle sprain. Whether ATFL findings and anterior displacement reflect symptoms and function remains unclear. This study examined the relationships among ATFL ultrasonography findings, anterior ankle displacement, perceived ankle instability, and self-reported function. This cross-sectional study included 187 participants classified as healthy, coper, or CAI using International Ankle Consortium criteria and the Cumberland Ankle Instabilit

Morphometric Analysis of the Talocrural Joint and Lateral Ankle Ligament Complex in Human Cadavers: A Population-Specific Study.

BACKGROUND: Chronic ankle instability is a common musculoskeletal condition associated with recurrent sprains and functional impairment. Morphological variations in the talocrural joint and lateral ankle ligaments play a significant role in determining joint stability. However, population-specific anatomical data remain limited in the Indian population. OBJECTIVE: To evaluate the morphometric characteristics of the talocrural joint and lateral ankle ligaments in cadaveric specimens and establish baseline anatomical reference values. METHODS: This observational cadaveric study was conducted i

Brain and knee joint degeneration following anterior cruciate ligament injury in a mouse model of Alzheimer's disease.

Alzheimer's disease (AD) and osteoarthritis (OA) are 2 of the most common health conditions affecting the elderly. Recent studies in mice and humans have described an association between OA and AD, with prevalent OA increasing the risk of developing AD-like cognitive declines. In this study, we investigated brain and knee joint degeneration and neurocognitive dysfunction in a mouse model of AD (APP/PS1 double transgenic mice) in the presence or absence of OA induced by non-invasive ACL rupture. We hypothesized that OA would be associated with increased beta amyloid (Aβ) and phosphorylated tau

Mitochondrial transfer and transplantation in tendon, ligament, and enthesis repair: current evidence, mechanistic rationale, and regenerative opportunities.

Tendon, ligament, and enthesis injuries often heal through fibrovascular scar rather than restoration of native matrix organization, mechanical function, and graded interface architecture. Mitochondrial dysfunction has emerged as a recurrent feature of these repair failures, linking impaired bioenergetics, oxidative stress, persistent inflammation, cell death, and defective matrix remodeling. In parallel, mitochondria are increasingly recognized as transferable organelles that can move between cells or be delivered therapeutically as isolated mitochondria, mitochondria-rich extracellular vesic

Anterior Controllable Antedisplacement and Fusion (ACAF) versus laminoplasty in suppressing the progression of ossification of the posterior longitudinal ligament.

Laminoplasty (LP) is a standard motion-preserving procedure for multi-segmental ossification of the posterior longitudinal ligament (OPLL), yet it may permit continued ossification progression due to retained segmental mobility. The anterior controllable antedisplacement and fusion (ACAF) technique offers ventral decompression with segmental stabilization, but its effect on OPLL fate remains unclear. To compare radiographic OPLL progression and neurological outcomes between ACAF and LP in patients with multilevel cervical OPLL. This retrospective study included 100 patients (50 ACAF, 50 LP) tr

[Clinical efficacy of anterior cervical mortise-tenon corpectomy and fusion for the treatment of ossification of the posterior longitudinal ligament].

OBJECTIVES: For the segmental ossification of cervical posterior longitudinal ligament (OPLL) accompanied by spinal cord compression, anterior cervical corpectomy and fusion (ACCF) can achieve direct decompression through removal of most of the involved vertebral body. However, reconstruction-related complications such as titanium mesh cage subsidence, loss of cervical lordosis, and poor bone fusion are common after surgery. This study aims to investigate the clinical efficacy of a novel anterior cervical mortise-tenon corpectomy and fusion (ACMTF) technique, designed based on principles of bi

Preliminary Assessment of Pisotriquetral Joint Kinematics Following Transverse Carpal Ligament Release: A Cadaveric Pilot Study.

Introduction Transverse carpal ligament (TCL) release is the standard treatment for carpal tunnel syndrome; however, its potential to alter carpal biomechanics is unclear. Specifically, it has been hypothesized that TCL transection may destabilize the pisotriquetral (PT) joint or unmask latent arthritis. This pilot study investigated the immediate kinematic effects of TCL transection on the PT joint in an embalmed cadaveric model. Methods Six embalmed cadaveric wrists were mounted on a custom stabilization apparatus. Colored markers were affixed to the ulna, pisiform, and fourth metacarpal bon

Spiral ligament dysfunction and endocochlear potential loss drive hearing impairment in Niemann-Pick C1 mice.

BACKGROUND: Sensorineural hearing loss is increasingly recognized in Niemann-Pick disease type C (NPC), but the underlying cochlear lesion remains undefined. While prior work emphasized hair-cell (HC) involvement, whether auditory dysfunction instead arises from lateral-wall failure and endocochlear potential (EP) decline is unknown. METHODS: Npc1-/- mice and littermate controls underwent auditory function test and electrophysiological recordings at postnatal day (P) 35 and P63. Cochlear cytoarchitecture was evaluated using immunohistochemistry and transmission electron microscopy (TEM). To p

Advanced liver dissection with left triangular ligament section for enhanced surgical access to right posterior lesions.

BACKGROUND: Laparoscopic liver resection (LLR) has gained worldwide acceptance over the past decade, though its adoption remains incomplete due to technical challenges. These challenges, particularly in replicating traditional open surgery techniques, make controlling hemostasis and liver mobilization more difficult. The IWATE scoring system classifies the complexity of LLR, with expert-level procedures involving difficult-to-access segments. This study investigates whether extensive dissection around the inferior vena cava (IVC), including sectioning the left triangular ligament, improves liv

Does an alar ligament tubercle exist? Anatomical study with application to imaging interpretation and skull base approaches.

Although numerous anatomical and radiological studies describe a "tubercle" of the alar ligaments at the medial OC as the alar ligament insertion, no systematic osteological investigation has evaluated the prevalence, morphology, and variability of this structure. Therefore, this study aimed to determine whether an alar ligament tubercle (ALT) is a consistent anatomical feature or a morphological variant. Two hundred fifty adult skulls were examined. The presence, morphology, and laterality of the ALTs were documented. Tubercle dimensions were measured using microcalipers, and a classification
