Humoral Immune Correlates for Prevention of Postnatal Cytomegalovirus Acquisition.

BACKGROUND: Development of a cytomegalovirus (CMV) vaccine is a high priority. However, the ability of antibodies to protect against CMV infection is not well characterized. Studies of maternal antibodies in infants offer the potential to identify humoral correlates of protection against postnatal acquisition. METHODS: This hypothesis-generating study analyzed 29 Ugandan mother-infant pairs that were followed weekly for CMV acquisition. Seventeen mothers and no infants were infected with human immunodeficiency virus (HIV). We evaluated the association between CMV-specific immunoglobulin G (Ig

Maternal adverse childhood experiences and antepartum risks: the moderating role of social support.

The aims of the current study were to examine the association between maternal adverse childhood experiences (ACEs) and antepartum health risks, and to investigate whether social support moderated this association. It was hypothesized that ACEs would be associated with antepartum health risks; however, social support in the prenatal period would buffer mothers from the deleterious consequences of ACEs. Data from 1994 women (mean age = 31 years) and their infants were collected from a longitudinal cohort recruited in health care offices in Alberta, Canada. Pregnant women completed questionnaire

The Contribution of Untreated and Treated Anxiety and Depression to Prenatal, Intrapartum, and Neonatal Outcomes.

Objective  To determine independent perinatal associations of anxiety and depression in women who were and were not treated with psychotropic drugs in comparison to unaffected pregnancies. Study Design  From 2013 to 2014, 978 (6.3%) cases of anxiety/depression, of which 35% used psychotropic drugs, were compared with 14,514 (93.7%) unaffected pregnancies using logistic regression. Results  Subjects were more likely to be Non-Hispanic Whites, use tobacco and illegal substances, be unmarried, use public insurance, and have medical complications of pregnancy. For independent maternal outcomes, un

Loss of neurosteroid-mediated protection following stress during fetal life.

Elevated levels of neurosteroids during late gestation protect the fetal brain from hypoxia/ischaemia and promote neurodevelopment. Suppression of allopregnanolone production during pregnancy leads to the onset of seizure-like activity and potentiates hypoxia-induced brain injury. Markers of myelination are reduced and astrocyte activation is increased. The placenta has a key role in maintaining allopregnanolone concentrations in the fetal circulation and brain during gestation and levels decline markedly after both normal and preterm birth. This leads to the preterm neonate developing in a ne

Contrasting Patterns of Serologic and Functional Antibody Dynamics to Plasmodium falciparum Antigens in a Kenyan Birth Cohort.

IgG antibodies to Plasmodium falciparum are transferred from the maternal to fetal circulation during pregnancy, wane after birth, and are subsequently acquired in response to natural infection. We examined the dynamics of malaria antibody responses of 84 Kenyan infants from birth to 36 months of age by (i) serology, (ii) variant surface antigen (VSA) assay, (iii) growth inhibitory activity (GIA), and (iv) invasion inhibition assays (IIA) specific for merozoite surface protein 1 (MSP1) and sialic acid-dependent invasion pathway. Maternal antibodies in each of these four categories were detecte

Maternal anaemia and preterm birth: a prospective cohort study.

BACKGROUND: The role of maternal anaemia in preterm birth remains poorly defined, and the association between anaemia and preterm birth clinical subtypes remain unclear. We examined if maternal anaemia exposure both within and across trimesters during gestation is associated with preterm birth. METHODS: This was a secondary analysis of data from a population-based prospective cohort study in 13 counties of East China (1993-96). All singleton live births delivered at 20-44 weeks to women with at least one haemoglobin measure during pregnancy were included (n = 160 700). Risk of preterm birth (

The effect of physical and emotional partner violence exerted during pregnancy on prenatal attachment and depression levels in pregnant women.

Exposure of pregnant women to physical and emotional violence during pregnancy adversely affects the health of the mother and the fetus. This study aimed to assess the effects of emotional and physical partner violence on prenatal attachment and depression levels in pregnant women. Five-hundred and ten pregnant women in the second and third trimesters were included in the study. A personal information form, the Epidemiological Research Center Depression Scale, and the prenatal attachment inventory (PAI) were used for data collection. Although 7.5% of the pregnant women were exposed to physical

[Consequences of congenital COVID-19 on the cognitive development of affected children].

Congenital COVID-19 refers to infection with the SARS-CoV-2 virus acquired in utero as a result of maternal infection during pregnancy. Although vertical transmission of the virus from mother to fetus appears to be relatively infrequent, growing clinical and epidemiological evidence indicates that prenatal exposure to SARSCoV-2, as well as to the maternal immune response it elicits, may have significant implications for early neurological development. Therefore, understanding the potential cognitive consequences of congenital COVID-19 is essential for the long-term monitoring of child health i

Imaging of pregnant and lactating patients with suspected adrenal disorders.

A high level of clinical suspicion is essential in the diagnosis and management of a suspected adrenal mass during pregnancy and the peripartum period. Timely recognition is important in order to improve fetal and maternal outcomes. Imaging is often performed to confirm a suspected adrenal lesion; however, increasing usage of diagnostic imaging during pregnancy and lactation has also increased awareness, concerns and confusion regarding the safety risks regarding fetal and maternal exposure to radiation and imaging intravenous contrast agents. This may lead to anxiety and avoidance of imaging

Malaria in pregnancy shapes the development of foetal and infant immunity.

Malaria, particularly Plasmodium falciparum, continues to disproportionately affect pregnant women. In addition to the profoundly deleterious impact of maternal malaria on the health of the mother and foetus, malaria infection in pregnancy has been shown to affect the development of the foetal and infant immune system and may alter the risk of malaria and nonmalarial outcomes during infancy. This review summarizes our current understanding of how malaria infection in pregnancy shapes the protective components of the maternal immune system transferred to the foetus and how foetal exposure to pa

Urgent global opportunities to prevent birth defects.

Birth defects are an urgent global health priority. They affect millions of births worldwide. But their prevalence and impact are largely under-ascertained, particularly in middle- and low-income countries. Fortunately, a large proportion of birth defects can be prevented. This review examines the global prevalence and primary prevention methods for major preventable birth defects: congenital rubella syndrome, folic acid-preventable spina bifida and anencephaly, fetal alcohol syndrome, Down syndrome, rhesus hemolytic disease of the fetus and the newborn; and those associated with maternal diab

Chondrodysplasia punctata and maternal autoimmune disease: a new case and review of the literature.

Classic rhizomelic chondrodysplasia punctata is a rare, autosomal, recessively inherited disorder that is characterized by proximal shortening of the limbs, punctuate calcifications of the epiphyses, cataracts, developmental delay, and early lethality. A distinctive biochemical profile is characteristic for each of the several defects of peroxisomal metabolism. Recently, cases have been described that were not associated with peroxisomal dysfunction. These cases were found to be secondary to teratogen exposure or maternal conditions. Since 1993, there have been 9 reported cases of neonates wit

Maternal depression effects on infants and early interventions.

Our recent research suggests that: (1) maternal depression negatively affects infants as early as the neonatal period, implicating prenatal effects of maternal depression; (2) as early as birth the infants show a profile of "dysregulation" in their behavior, physiology, and biochemistry which probably derives from prenatal exposure to a biochemical imbalance in their mothers; (3) these effects are compounded by the disorganizing influence of the mother's interaction behavior; (4) depressed mothers have two predominant interaction styles, withdrawn or intrusive, which seem to have differential,

Childhood brain tumors and exposure to tobacco smoke.

Brain tumors are the second most common cancer in children after leukemia, yet the etiology of childhood brain tumors remains unknown. Tobacco smoke contains several dozen compounds that are known to be carcinogens. Among these are N-nitroso compound precursors, principally tobacco-specific nitrosamines. Although smoking has not been identified as a significant risk factor for the development of brain tumors in adults, fetuses and infants have incompletely formed blood-brain barriers that may allow the passage of carcinogenic tobacco metabolites into the central nervous system and initiate the

Substance abuse and HIV pregnancy.

Substance abuse is a significant risk factor for HIV in pregnancy. Most substances can have deleterious effects on perinatal outcome, and in pregnancies complicated by maternal HIV infection, substance abuse may contribute to the rate of vertical transmission and the progression of maternal disease. Therefore, pregnant substance-abusing women should be identified and targeted for HIV counseling and testing. In addition, drug rehabilitation should be strongly advocated. In populations with high prevalences of substance abuse and HIV infection, exceptional measures such as voluntary routine drug

Exposure to mercury during the first six months via human milk and vaccines: modifying risk factors.

Breastfeeding is the best natural protection infants have against morbidity and mortality, and the development of safe and effective vaccines has made it possible to immunize children against infectious disease. Both of these mechanisms for ensuring good health in children may be compromised by contact with mercury (Hg). Maternal exposure to environmental Hg during pregnancy can predispose nursing children to neurodevelopmental disorders. Despite the World Health Organization assurance that thimerosal-preserved vaccines are safe to use in infants, the United States, the European Union, and doz
