Outpatient palliative care for chronic heart failure: a case series.

BACKGROUND/OBJECTIVES: Although the palliative care needs of outpatients with chronic heart failure (HF) are numerous, there is limited published experience in providing outpatient HF palliative care. This article describes the patients seen and the issues addressed in an outpatient palliative care program for patients with HF. METHODS: Case series involving a retrospective medical record review using descriptive quantitative and qualitative analysis. RESULTS: Over a 3 ½ year time period, 50 patients were seen, resulting in 228 total visits. Fifty percent of patients were seen only once. Fif

Risk levels for suffering a traffic injury in primary health care. The LESIONAT project.

BACKGROUND: Literature shows that not only are traffic injuries due to accidents, but that there is also a correlation between different chronic conditions, the consumption of certain types of drugs, the intake of psychoactive substances and the self perception of risk (Health Belief Model) and the impact/incidence of traffic accidents. There are few studies on these aspects in primary health care. THE OBJECTIVES of our study are: Main aim: To outline the distribution of risk factors associated with Road Traffic Injuries (RTI) in a driving population assigned to a group of primary health care

Barriers to implementing infection prevention and control guidelines during crises: experiences of health care professionals.

BACKGROUND: Communicable disease crises can endanger the health care system and often require special guidelines. Understanding reasons for nonadherence to crisis guidelines is needed to improve crisis management. We identified and measured barriers and conditions for optimal adherence as perceived by 4 categories of health care professionals. METHODS: In-depth interviews were performed (n = 26) to develop a questionnaire for a cross-sectional survey of microbiologists (100% response), infection preventionists (74% response), public health physicians (96% response), and public health nurses (

Anastomotic leaks after intestinal anastomosis: it's later than you think.

PURPOSE: Anastomotic leaks are among the most dreaded complications after colorectal surgery. However, problems with definitions and the retrospective nature of previous analyses have been major limitations. We sought to use a prospective database to define the true incidence and presentation of anastomotic leakage after intestinal anastomosis. METHODS: A prospective database of two colorectal surgeons was reviewed over a 10-year period (1995-2004). The incidence of leak by surgical site, timing of diagnosis, method of detection, and treatment was noted. Complications were entered prospective

Cost effectiveness of a smoking cessation program in patients admitted for coronary heart disease.

BACKGROUND: Smoking cessation is probably the most important action to reduce mortality after a coronary event. Smoking cessation programs are not widely implemented in patients with coronary heart disease, however, possibly because they are thought not to be worth their costs. Our objectives were to estimate the cost effectiveness of a smoking cessation program, and to compare it with other treatment modalities in cardiovascular medicine. METHODS: A cost-effectiveness analysis was performed on the basis of a recently conducted randomized smoking cessation intervention trial in patients admit

[Characteristics and analysis of survival of people served in the program of home care from the basic health area of Raval Nord, Barcelona, 1993-2002].

BACKGROUND: Longer life expectancy has led to a progressive aging of the population and to growing sociosanitary homecare. This study is aimed at describing the population of patients suffering from chronic diseases having been provided with homecare throughout the first ten years that homecare was available by a primary care team at an urban Healthcare Center, to study how often this type of care was used and to analyze the survival of those individuals having been provided with this care. METHODS: A descriptive study. The subjects under study were 1,357 chronically-ill patients included fro

Giant shagreen patch associated with spina bifida occulta in tuberous sclerosis.

BACKGROUND: Patients with systemic sclerosis (SSc) frequently experience symptoms such as pain, fatigue, and functional impairments, leading to a significant decline in their quality of life (QoL). Accurately evaluating the QoL of SSc patients is crucial for developing personalized treatment plans and enhancing prognosis. The Systemic Sclerosis Quality of Life Questionnaire (SScQoL), an internationally recognized SSc-specific tool, is commonly used for QoL assessment. However, this questionnaire has not undergone cross-cultural adaptation in China, and its reliability and validity require furt

Cognitive impairment after stroke: frequency, patterns, and relationship to functional abilities.

Cognitive function was examined in 227 patients three months after admission to hospital for ischaemic stroke, and in 240 stroke-free controls, using 17 scored items that assessed memory, orientation, verbal skills, visuospatial ability, abstract reasoning, and attentional skills. After adjusting for demographic factors with standardised residual scores in all subjects, the fifth percentile was used for controls as the criterion for failure on each item. The mean (SD) number of failed items was 3.4 (3.6) for patients with stroke and 0.8 (1.3) for controls (p < 0.001). Cognitive impairment, def

A Cardiovascular Care Pathway Supported by Remote Patient Management in Dutch Primary Care: Cost-Effectiveness, Budget Impact, and Workload Analysis.

BACKGROUND: Remote patient management (RPM) that supports patient self-monitoring of vital parameters and lifestyle factors may improve cardiovascular risk management (CVRM) in primary care. However, large-scale implementation remains limited, partly due to insufficient evidence on long-term value for money, budget impact, and implications for health care professionals' workload. OBJECTIVE: This study aimed to estimate the long-term cost-effectiveness, 5-year health care budget impact, and expected changes in general practitioner (GP) and practice nurse (PN) workload associated with the CVRM-

Psilocybin-assisted group psychotherapy and mindfulness-based stress reduction for frontline healthcare provider COVID-19-related depression and burnout: A randomized controlled trial.

BACKGROUND: Depression and burnout, which are common among healthcare workers, were exacerbated by the COVID-19 pandemic. Mindfulness-Based Stress Reduction (MBSR) and psilocybin have been reported to reduce depressive symptoms, but the efficacy of the combination requires comparison to an active treatment control. We sought to evaluate the safety and preliminary efficacy of psilocybin and MBSR versus MBSR alone for frontline healthcare providers with symptoms of depression and burnout related to the COVID-19 pandemic. We hypothesized that psilocybin would augment the antidepressant effects of
