One-year mortality risk in older individuals with femoral intertrochanteric fracture: a tertiary center in China.

BACKGROUND: The accelerated growth of older individuals worldwide has increased the number of patients presenting with fragility hip fractures. Having a hip fracture can cause excess mortality, and patients with hip fracture have a higher risk of death than those without hip fracture. Most studies have treated hip fracture as a single, homogeneous condition, but hip fracture includes two major anatomic types: intertrochanteric fracture and femoral neck fracture. Few studies have specifically evaluated 1-year mortality risk in older individuals with femoral intertrochanteric fracture. The aim o

Circadian clock disruption impairs immune oscillation in chronic endogenous hypercortisolism: a multi-level analysis from a multicentre clinical trial.

BACKGROUND: Glucocorticoids (GC) are potent entrainers of the circadian clock. However, their effects on biological rhythms in chronic human exposure have yet to be studied. Endogenous hypercortisolism (Cushing's Syndrome, CS) is a rare condition in which circadian disruption is sustained by a tumorous source of GC excess, offering the unique opportunity to investigate GC's chronic effects in vivo. METHODS: In a 12-month prospective case-control multicentre trial, the daily fluctuations in the number of circulating peripheral blood mononuclear cells (PBMCs) and the time-specific expression of

Post-sepsis chronic muscle weakness can be prevented by pharmacological protection of mitochondria.

BACKGROUND: Sepsis, mainly caused by bacterial infections, is the leading cause of in-patient hospitalizations. After discharge, most sepsis survivors suffer from long-term medical complications, particularly chronic skeletal muscle weakness. To investigate this medical condition in detail, we previously developed a murine severe sepsis-survival model that exhibits long-term post-sepsis skeletal muscle weakness. While mitochondrial abnormalities were present in the skeletal muscle of the sepsis surviving mice, the relationship between abnormal mitochondria and muscle weakness remained unclear.

Identification and Characterization of Novel Mutations in Chronic Kidney Disease (CKD) and Autosomal Dominant Polycystic Kidney Disease (ADPKD) in Saudi Subjects by Whole-Exome Sequencing.

Background: Autosomal dominant polycystic kidney disease (ADPKD) is a condition usually caused by a single gene mutation and manifested by both renal and extrarenal features, eventually leading to end-stage renal disease (ESRD) by the median age of 60 years worldwide. Approximately 89% of ADPKD patients had either PKD1 or PKD2 gene mutations. The majority (85%) of the mutations are in the PKD1 gene, especially in the context of family history. Objectives: This study investigated the genetic basis and the undiscovered genes that are involved in ADPKD development among the Saudi population. Mate

Use of En Face Swept-Source Optical Coherence Tomography Angiography in Identifying Choroidal Flow Voids in 3 Patients With Birdshot Chorioretinopathy.

IMPORTANCE: Patients with birdshot chorioretinopathy (BSCR) can experience a delay in diagnosis owing to the challenges of identifying the condition prior to evolution of characteristic choroidal scars. An objective, noninvasive method for detecting early lesions in BSCR might have an effect on preventing vision loss in these patients. OBJECTIVE: To test the feasibility of swept-source optical coherence tomography angiography (SS-OCTA) in the detection of BSCR choroidal lesions and to use en face image analysis of choroidal layers to localize lesion depth. DESIGN, SETTING, AND PARTICIPANTS:

Chronic Liver Disease as a Risk Factor For Post-ERCP Complications: A Nationwide Retrospective Analysis.

INTRODUCTION: Endoscopic retrograde cholangiopancreatography (ERCP) is indicated for multiple pancreatic and biliary pathologies and carries a heightened risk profile compared with other endoscopic procedures. Considerable research has been directed towards discerning risk factors associated with complications such as post-ERCP pancreatitis and post-ERCP bleeding. Despite this, data on chronic liver disease (CLD) as a risk factor for complications is limited. We aimed to assess the outcomes of patients with CLD who underwent therapeutic or diagnostic ERCP to determine whether these patients ha

Characterization of immunoreactive forms of human osteocalcin generated in vivo and in vitro.

Three monoclonal antibodies recognizing the 5-13, 25-37, and 43-49 sequence of the human osteocalcin were used in competitive and two-site radioimmunoassays (RIA) to characterize specifically various immunoreactive forms of circulating human osteocalcin. The intact molecule accounts for 36% of total in normals (2.6 nM), 46% in patients with osteoporosis (3.1 nM), and 26% in chronic renal failure (6.9 nM). Four fragment were detected in addition to the intact molecule in the serum of healthy adults and patients with metabolic bone disease. N-terminal, mid, and mid C-terminal fragments were pres

Exploring the diagnostic and prognostic utility of resting state functional MRI connectivity in acute/sub-acute TBI.

Traumatic brain injury (TBI) is a leading cause of disability worldwide. Yet, our understanding of the mechanisms of this condition is limited, especially in the acute setting. Here, we investigated the relationship between functional connectivity and common clinical assessments, like the Glasgow Coma Scale (GCS) at admission and modified Rankin scale at 3-months (mRS) to determine if functional connectivity can provide a broader representation of the brain's networks than these standard tests. We performed a retrospective analysis of resting state functional MRI and clinical data in 58 patien

Gene polymorphisms associated with heterogeneity and senescence characteristics of sarcopenia in chronic obstructive pulmonary disease.

BACKGROUND: Sarcopenia, or loss of skeletal muscle mass and decreased contractile strength, contributes to morbidity and mortality in patients with chronic obstructive pulmonary disease (COPD). The severity of sarcopenia in COPD is variable, and there are limited data to explain phenotype heterogeneity. Others have shown that COPD patients with sarcopenia have several hallmarks of cellular senescence, a potential mechanism of primary (age-related) sarcopenia. We tested if genetic contributors explain the variability in sarcopenic phenotype and accelerated senescence in COPD. METHODS: To ident

A modelling method for large-scale open spaces orientated toward coordinated control of multiple air-terminal units.

Skin ulcers are a commonly encountered problem at departments of tropical dermatology in the Western world. Furthermore, the general dermatologist is likely to be consulted more often for imported chronic skin ulcers because of the ever-increasing travel to and from tropical countries. The most common cause of chronic ulceration throughout the world is probably pyoderma. However, in some parts of the world, cutaneous leishmaniasis is one of the most prevalent causes. Mycobacterium ulcerans is an important cause of chronic ulcers in West Africa. Bacterial infections include pyoderma, mycobacter

The use of a multiplanar, multi-axis external fixator to achieve knee arthrodesis in a worst case scenario: a case series.

BACKGROUND: One of the most catastrophic outcomes following total knee arthroplasty (TKA) is a chronic periprosthetic infection with concomitant failure of the knee extensor mechanism. This study retrospectively reviewed the clinical records of 7 patients who were treated with a 6 axis circular external fixation frame (Taylor Spatial Frame (TSF)) for this condition. Fusion was achieved in 5 of 7 patients (71%) at an average of 8.4 months after surgery. Complications occurred in the treatment of 5 of 7 patients (71%). Infection was controlled in all cases. The TSF presents another valuable tool

Motor cortex stimulation for the treatment of refractory peripheral neuropathic pain.

Epidural motor cortex stimulation (MCS) has been proposed as a treatment for chronic, drug-resistant neuropathic pain of various origins. Regarding pain syndromes due to peripheral nerve lesion, only case series have previously been reported. We present the results of the first randomized controlled trial using chronic MCS in this indication. Sixteen patients were included with pain origin as follows: trigeminal neuralgia (n = 4), brachial plexus lesion (n = 4), neurofibromatosis type-1 (n = 3), upper limb amputation (n = 2), herpes zoster ophthalmicus (n = 1), atypical orofacial pain secondar

Zinc Hexacyanoferrate Nanoenzymes Attenuate Neuroinflammation After Spinal Cord Injury by Modulating Mitochondrial Function and Microglial Polarization.

BACKGROUND: Secondary pathology after acute spinal cord injury (SCI) is driven by pathological iron accumulation, reactive oxygen species (ROS) overproduction, and mitochondrial failure, which together sustain chronic neuroinflammation. Methylprednisolone (MP), the most widely used ROS-scavenging agent for acute SCI, provides limited clinical benefit and carries considerable side effects. To address this, we engineered zinc hexacyanoferrate nanoenzymes (ZnHPB) that chelate excess ferrous ions while displaying intrinsic SOD- and CAT-like activities, attenuating oxidative injury at its source ra

Carrageenan-induced immunothrombotic models in rodents (Review).

Carrageenan (CGN)-induced immunothrombotic models are used in rodent thrombosis research. They enable both mechanistic dissection of inflammation-coagulation crosstalk and high-throughput screening of antithrombotic agents. The present narrative review summarizes current knowledge on modeling approaches, thrombotic mechanisms, quality determinants and translational relevance of the CGN model. CGN is a sulfated linear galactan that can be administered by intraperitoneal injection, tail vein injection or subcutaneous injection. Amongst these, intraperitoneal injection of κ-CGN (20-100 mg/kg in m

The roles of diabetes-specific self-compassion, affect balance, and self-care self-efficacy in psychological distress among Hong Kong patients with Type 2 diabetes.

OBJECTIVE: Patients with Type 2 diabetes (T2D) often blame themselves for their conditions and not reaching their self-management goals. Guided by the self-regulation resource model, self-compassion facilitates adjustments to chronic diseases through increasing affective and cognitive resources like positive affect and self-care self-efficacy. However, the contributions of positive and negative self-compassion (PSC and NSC) in psychological adjustment among patients with T2D have not been examined. Studies also suggest that patients with T2D experience positive changes from their illness journ
