Giant Cervicothoracic Madelung Disease Presenting with Airway Compromise: A Case Report from Ethiopia.

INTRODUCTION: To describe a rare (airway-threatening) cervicothoracic presentation of Madelung disease (multiple symmetric lipomatosis) with upper mediastinal extension and to introduce a reproducible peri-operative strategy in a resource-limited setting. CASE PRESENTATION: We report a case of a 40-year-old Oromo Ethiopian male who presented with progressive cervicothoracic adipose masses resulting in dyspnea and venous congestion. Awake fiberoptic intubation, staged resection, early extubation, and close airway management were the hallmarks of peri-operative management. Contrast-enhanced CT

When gas travels: a case report of partial Horner's syndrome from subcutaneous emphysema post-laparoscopic hiatal hernia repair.

INTRODUCTION: Postoperative partial Horner's syndrome associated with subcutaneous emphysema after laparoscopic hiatal hernia repair has not been previously reported. In this particular case, this rare complication is due to compression of the cervical sympathetic plexus surrounding the carotid artery by subcutaneous emphysema. CASE PRESENTATION: A 59 year-old female awoke postoperatively from laparoscopic hiatal hernia repair and complained of blurred vision in the left eye and difficulty opening it. This prompted the general surgery team to consult neurology. The patient was examined and un

Correlation of preoperative radiological lymph node assessment with pathological nodal and tumor findings in tongue and oral floor squamous cell carcinoma.

PURPOSE: Preoperative imaging is central to cervical lymph node staging in oral squamous cell carcinoma (OSCC). However, the pathological and biological determinants underlying radiological lymph node suspicion remain poorly defined. METHODS: This retrospective cohort study included 438 patients with OSCC of the tongue or floor of mouth treated with curative surgery and neck dissection between 2011 and 2021. Radiological lymph node assessment was classified using a three-level scale (unremarkable, suspicious, metastatic) and a dichotomous scale (unremarkable vs. suspicious/metastatic). Associ

Transoral robotic surgery using CO2 laser in oropharyngeal squamous cell carcinoma: a clinical case series.

PURPOSE: Transoral robotic surgery (TORS) is an established surgical approach for oropharyngeal squamous cell carcinoma (OPSCC). Carbon dioxide (CO₂) laser offers high cutting precision and minimal collateral thermal injury, yet its use in TORS remains underreported. The purpose of this study is therefore to describe the first UK case series detailing the use of CO₂ laser via hollow waveguide in TORS. METHODS: Eight patients (six male, two female; mean age 60 years) presenting with either left tonsillar (seven cases) or right tongue base (one case) OPSCC underwent primary TORS resection with

Transnasal-Transoral Endoscopic Surgery Followed by Radiotherapy for Nasopharyngeal Spindle Cell Carcinoma.

Spindle cell carcinoma (SpCC) is a rare and aggressive variant of squamous cell carcinoma characterized by the coexistence of epithelial and mesenchymal components, accounting for <1% of all head and neck malignancies. Nasopharyngeal SpCC is extremely rare, and its optimal management remains unclear, particularly in patients who cannot tolerate extensive surgery. An 80-year-old man presenting with right-sided ear fullness was diagnosed with nasopharyngeal SpCC (T1N0M0). Imaging revealed a submucosal tumor on the posterior wall of the nasopharynx that was in contact with the right Eustachian tu

Prognostic value of worst pattern of invasion and tumor budding as histological predictive factors in oral cancer.

OBJECTIVE: In patients with oral cancer, unfavourable clinical and histological features may require postoperative radiotherapy or chemoradiation. Our study investigated the impact of the invasive front of the tumor on oncological outcomes. In our cohort, we investigated the impact of worst pattern of infiltration (WPOI) and tumor budding (TB) on oncological outcomes. MATERIALS AND METHODS: We included 142 patients with primary oral cancer in our prospective cohort study. We excluded 19 patients with other malignancy than oral squamosus cell carcinoma. Two groups were created of 123 cases bas

Clinicopathological factors and prognostic analysis of adenoid cystic carcinomas of sublingual gland: a single-centre retrospective study of 41 patients.

A retrospective review was performed to evaluate the clinicopathological factors and prognosis of patients with sublingual gland adenoid cystic carcinomas (ACC) from January 2007 to December 2023. Forty-one patients were included, 26 women and 15 men (median age 57 years; range 25-76). Surgical excision was performed in all patients, and the occult node metastasis rate was 12.5%. The median overall survival time was 117 months (range 13-147). Twenty-two patients developed recurrent disease after initial surgery. Thirteen patients died due to the tumour during follow-up: distant metastasis was

Clinical course of Merkel cell carcinoma: A DeCOG multicenter study of 1049 patients.

BACKGROUND: Merkel cell carcinoma (MCC) is a highly aggressive skin cancer with neuroendocrine differentiation characterized by frequent recurrences. Large epidemiological databases (e.g., SEER, IARC) lack granularity in analyzing associations between tumor, patient characteristics, locoregional interventions and recurrence patterns. PATIENTS AND METHODS: Within the pre-immunotherapy era (1998-2017) the DeCOG MCC registry included 1049 patients with histopathologically confirmed MCC. Patient/tumor characteristics, treatment details, and outcomes were analyzed. Primary endpoints were progressi

Gasless versus gas-inflated transaxillary endoscopic thyroidectomy for papillary thyroid carcinoma: a cohort study on surgical outcomes and learning curves.

BACKGROUND: Conventional open thyroidectomy (COT) results in visible neck scarring. Transaxillary endoscopic thyroidectomy (TET) comprises gasless (suspension-assisted) and gas-inflated approaches, both of which offer superior scar concealment. This study aimed to compare the efficacy and safety of these two endoscopic techniques for treating papillary thyroid carcinoma (PTC). METHODS: A total of 471 patients were stratified into three groups: gasless transaxillary endoscopic thyroidectomy (GTET), gas-inflated transaxillary endoscopic thyroidectomy (GITET), and COT. Comparative analyses inclu

Benchmark Results for Physician-Modified Endografts.

BACKGROUND: The term, "Physician-modified endografts (PMEGs)" was first coined by Dr Benjamin Starnes et al at the University of Washington in 2009 to extend endovascular repair to complex aortic anatomy. METHODS: A literature review was done and included studies written in the last 10 years about PMEG including current guideline statements. RESULTS: PMEGs are used to extend endovascular repair to short-neck infrarenal, juxtarenal, pararenal, and thoracoabdominal aneurysms, including symptomatic or ruptured presentations as well as postdissection pathologies. Overall mortality ranges from 2-

Preserving Cervical Mobility: A Novel Robot-Assisted Approach for Atlas Fracture Fixation.

BACKGROUND The management of unstable atlas fractures remains a subject of ongoing debate and controversy. The conservative surgical treatment commonly involves fusion, resulting in severe loss of cervical spine mobility, and a large incisions and extensive tissue dissection are required. We aim to introduce a novel concept and surgical approach for treating atlas fracture, one that involves minimizing trauma while maintaining mobility of the upper cervical spine without resorting to fusion. CASE REPORT We present the case of a 48-year-old woman with an atlas fracture who underwent robot-assis

Characterization of biomechanical risk factors during endoscopic submucosal dissection: Ergonomic pilot study.

BACKGROUND AND STUDY AIMS: Musculoskeletal disorders (MSDs) are prevalent among endoscopists. The aim of this study was to evaluate biomechanical risk factors for MSDs in gastroenterologists performing ESD. METHODS: An observational study was performed among interventional endoscopists performing ESD in three French centers. Physical constraints were assessed using an analog scale of perceived physical effort intensity and physiological sensors to measure joint angulation kinematics and muscle activity levels (flexor and extensor carpi radialis muscles) during diagnostic colonoscopy and ESD.

Congenital Pouch Colon in Males: Exploring the Risk of Epididymo-orchitis.

AIM: Patients with anorectal malformations (ARMs) are known to have a higher association with epididymo-orchitis (EO). Congenital pouch colon (CPC), a rare subtype of ARM, presents with several predisposing factors that are presumed to be linked with EO. This study aims to investigate the occurrence of EO in this specific subgroup of ARM patients to enhance the understanding of its etiology. MATERIALS AND METHODS: An ambispective observational study was conducted by analyzing medical records of male patients with CPC. Out of 28 patients managed primarily at our center, 25 fulfilled the inclus

HALP (Hemoglobin, Albumin, Lymphocyte, and Platelet) Score and Its Companions in Papillary Thyroid Cancer: Can They Predict Central Lymph Node Metastasis?

Background Papillary thyroid cancer (PTC) is one of the most common cancers with a favorable prognosis. The prognosis is most commonly associated with lymph node metastasis. We investigated whether there is a relationship between the hemoglobin, albumin, lymphocyte, and platelet (HALP) score and PTC central region lymph node metastasis in the preoperative period. NLR (neutrophil/lymphocyte ratio), PLR (platelet/lymphocyte ratio), MLR (monocyte/lymphocyte ratio), PIV (pan-immune-inflammation value), PNI (prognostic nutritional index), SII (systemic immune-inflammation index) were analyzed as ot

Extracranial-Intracranial Bypass with Reconstruction Clip Surgery Following Failed Flow Diverter Therapy for a Giant Internal Carotid Aneurysm: A Case Report.

Flow diverter (FD) is often the first-line treatment for giant internal carotid artery aneurysms, with a high rate of aneurysm occlusion. However, up to 10% of giant cerebral aneurysms increase in size after FD treatment. Surgery is usually considered if the giant internal carotid artery aneurysm continues to enlarge and cause compression. We report a case of a giant internal carotid artery aneurysm that continued to increase in size after FD treatment and was subsequently treated surgically. We also review the literature on the management of giant cerebral aneurysms that increased in size aft

Cervicofacial Surgical Emphysema Following Tonsillectomy: A Rare Complication Linked to Mucosal Disruption and Air Tracking.

Tonsillectomy is one of the most frequently performed procedures in ear, nose and throat (ENT) surgery, commonly indicated for recurrent tonsillitis, recurrent peritonsillar abscess, obstructive sleep apnoea or suspected malignancy. While generally safe, it carries recognised risks including haemorrhage, infection, pain and orodental trauma. Surgical emphysema is a rare but clinically significant complication, with fewer than 30 cases reported and an estimated incidence below 0.02%. It is hypothesised to result from mucosal or muscular disruption of the pharyngeal wall during dissection, parti
