Unipedicular vs. Bipedicular Balloon Kyphoplasty in the Treatment of Osteoporotic Vertebral Compression Fractures: Single-Institute 3-Year Follow-Up Results.

Background and Objectives: Balloon kyphoplasty is one of the most commonly performed minimally invasive surgical procedures for the treatment of osteoporotic vertebral fractures, with the bipedicular technique being the conventional approach. However, the use of both pedicles may present certain disadvantages, including higher costs, longer operative times, increased radiation exposure, and a greater risk of bone cement leakage. This study aims to report the 3-year follow-up outcomes of double-pedicle and single-pedicle kyphoplasty performed at our institution. Materials and Methods: Between J…

Recovery after neurologic injury in operative acetabular and pelvic fractures: Defining the natural history of foot drop.

INTRODUCTION: Dorsiflexion weakness, or footdrop, is a well-described sequela of high-energy acetabular and pelvic trauma, but little data exists describing the factors related to neurologic recovery and the timeline therein. An improved understanding of these factors would facilitate prognostication, patient education, and treatment decision-making. The aim of this study was to compare neurologic recovery between acetabular and pelvic fractures, delineate factors associated with recovery, and determine the expected timeline of recovery. METHODS: A retrospective chart review was conducted at

The analysis of percutaneous pedicle screw technique with guide wire-less in lateral decubitus position following extreme lateral interbody fusion.

BACKGROUND: Lateral lumbar interbody fusion (LLIF) and bilateral percutaneous pedicle fixation are valuable, minimally invasive lateral approaches used to treat symptomatic degenerative disc disease. In the current procedure, the patient's position on the operating table is changed after LLIF surgery from the lateral decubitus to the prone position. The ability to perform both approaches with the patient in the same position should reduce operation time. Use of a guide wire is problematic during percutaneous pedicle screw (PPS) insertion using fluoroscopy with the patient in the lateral decubi

Impact of artificial intelligence arrhythmia mapping on time to first ablation, procedure duration, and fluoroscopy use.

INTRODUCTION: Artificial intelligence (AI) ECG arrhythmia mapping provides arrhythmia source localization using 12-lead ECG data; whether this information impacts procedural efficiency is unknown. We performed a retrospective, case-control study to evaluate the hypothesis that AI ECG mapping may reduce time to ablation, procedural duration, and fluoroscopy. MATERIALS AND METHODS: Cases in which system output was used were retrospectively enrolled according to IRB-approved protocols at each site. Matched control cases were enrolled in reverse chronological order beginning on the last day for w

Lack of Pharmacokinetic Basis of Weight-Based Dosing and Intra-Operative Re-Dosing with Cefazolin Surgical Prophylaxis in Obese Patients: Implications for Antibiotic Stewardship.

Traditionally, there have been uniform antibiotic dosing guidelines for prophylaxis for clean-clean-contaminated surgery in both non-obese and obese adults. All other factors predisposing to surgical site infections (SSIs) being equal, over time, the preferred drug is cefazolin. The usual dose, given immediately pre-procedure, has been 1 g intravenously (IV) in non-penicillin-allergic patients, which has been highly effective, Recently, it has become common practice to use high-dose cefazolin; i.e., 3 g IV, in obese patients. This article reviews the literature on high-dose cefazolin prophylac

Single-visit versus multiple-visit root canal therapy: Post-endodontic pain outcomes from an umbrella review.

OBJECTIVE: This umbrella review synthesized and critically appraised evidence from systematic reviews comparing post-operative pain after single-visit and multiple-visit root canal treatment in permanent teeth. DATA: Twelve systematic reviews met the inclusion criteria. Post-operative pain was most commonly assessed as pain intensity using visual analogue or numeric rating scales at prespecified intervals, and less often as pain incidence. Flare-up outcomes were reported inconsistently and were defined variably across reviews. Overall, the higher-confidence reviews did not show a consistent o

Distributed functional cross-network connectivity of the epileptogenic zone predicts late seizure relapse in temporal lobe epilepsy.

OBJECTIVE: Although many patients with temporal lobe epilepsy achieve seizure freedom in the first post-operative year, the likelihood of long-term seizure freedom declines substantially over time. Late seizure relapse may reflect ongoing epileptogenicity, potentially driven by persistent epileptic activity arising from the epileptogenic zone. We hypothesized that its more distributed connectivity profile across brain networks would distinguish patients experiencing late relapse. METHODS: We prospectively enrolled 209 patients with drug-resistant temporal lobe epilepsy across six centers who

Day-to-Day Risk and Resilience Factors in the Context of Pediatric Post-Surgical Recovery - A Network Analysis of Intensive Longitudinal Data From Adolescents Undergoing Spinal Fusion Surgery and Their Parents.

OBJECTIVE: Ineffective pediatric post-operative pain management increases the risk of Chronic Post-Surgical Pain (CPSP), affecting around 20% of children undergoing major surgery. Psychological predictors of recovery, in both children and their parents, have been identified. However, how these variables change throughout the recovery process remains unclear. The aim of the present study was to investigate the associations of adolescent and parental risk and resilience variables in everyday life, during post-operative recovery, for adolescents undergoing spinal fusion surgery. METHODS: Partici
