Distribution of a putative endothelial barrier antigen in the ocular and orbital tissues of the rat.

BACKGROUND: A rat endothelial barrier antigen (EBA) recognised by a monoclonal antibody has been shown to be expressed strongly by endothelial cells of brain capillaries possessing a blood-brain barrier and only weakly expressed by fenestrated brain vessels. METHODS: In this study immunocytochemical methods for light and electron microscopy were used to study EBA distribution in the eye and orbital tissues of the rat. RESULTS: Blood-ocular barrier vessels in the optic nerve, retina, iris, and some vessels in th choroid and ciliary body were immunopositive for EBA. By pre-embedding immunocyto…

Anomalous intracranial venous drainage mimicking orbital or cavernous arteriovenous fistula.

A 9-year-old child presented with right eye swelling and bluish discoloration over the periorbital region. MR and CT displayed prominent vessels within the orbit and in the facial region, suggesting a carotid-cavernous fistula. Doppler ultrasound indicated the vascular engorgement was not secondary to a proximal arteriovenous shunt. Arteriography with its inherent time sequence image capability prompted the proper diagnosis of bilateral atresia of the sigmoid sinuses.

Orbital decompression for non-Graves' orbitopathy: a consideration of extended indications for decompression.

Orbital decompression is typically indicated for Graves' orbitopathy. Other causes of proptosis can also be safely and effectively addressed surgically with orbital decompression. Patients with prominent globes can have significant discomfort related to exposure keratopathy, lagophthalmos, and inefficient function of the globe-eyelid interface. We present six cases of non-Graves' proptosis that were addressed with orbital decompression. Indications for surgery included hypoplastic malar eminence with scleral show, enlarged globes, and congenital shallow orbits. Successful reduction of proptosi…

Reconstruction of traumatic orbital floor defects using irradiated cartilage homografts.

The important role of orbital shape and volume reconstruction has been studied by many investigators. There is, however, no consensus on the material that should be used in the reconstruction of the orbit. Both biologic and alloplastic materials have been used, each with its advantages and disadvantages. Here we report our experience with irradiated costal cartilage homograft in the reconstruction of the orbital floor. Irradiated cartilage grafts were used in 31 patients with significant traumatic defects in the orbital floor. Long-term follow-up in 21 patients up to 48 months revealed no inci…

Orbital cellulitis--an unusual presentation and late complication of severe facial trauma.

We report an unusual case of orbital cellulitis in a patient with only one seeing eye and severe facial asymmetry secondary to a road traffic accident twenty two years previously. Facial trauma sustained in the road traffic accident created continuity between the right orbit and the adjacent ethmoidal cells. The abnormal communication between the ethmoidal labyrinth and the right orbit rendered this patient highly susceptible to an orbital cellulitis from an adjacent paranasal sinus infection. However, this case is unusual with regard to the length of time that elapsed before such an infection…

[Color Doppler imaging in the study of normal orbital vessels and its hemodynamics].

Color Doppler Imaging (CDI) is a recent development in ultrasonography, which can display simultaneously two dimensional images of structure and Doppler evaluation of blood flow. Using the technique, we examined 74 normal eyes. Orbital vessels including ophthalmic artery (OA), posterior ciliary arteries (PCA), center retinal artery (CRA) and central retinal vein (CRV) could be located to make Doppler spectrum analysis and quantification of hemodynamics. Blood velocity (Unit cm/s) in these vessels was obtained as following: the average peak systolic flow velocity for OA was 31.9 +/- 5.01; for P…

Results of transantral orbital decompression in 428 patients with severe Graves' ophthalmopathy.

We reviewed records from 428 consecutive patients with severe Graves' ophthalmopathy to determine early and late results after transantral orbital decompression. Optic neuropathy was present in 217 (50.7%) patients. Post-operatively, 402 (89%) of 453 eyes with preoperative visual acuity worse than 20/20 improved or remained the same. Visual field scotomas improved or resolved in 245 (91%) of 269 eyes tested pre- and postoperatively. Preoperative papilledema resolved or improved in 99 (94%) of 105 eyes, and preoperative exposure keratitis improved or resolved in 178 (92%) of 195 eyes. Average p…

Pneumomediastinum following blowout fracture of the medial orbital wall: a case report.

An unusual case of cervical emphysema and pneumomediastinum following a blowout fracture of the medial orbital wall is presented. Severe pulmonary distress did not occur, and the trapped air rapidly absorbed. The source of the emphysema and its progression is discussed considering the anatomic relations of the facial and cervical subcutaneous space and the mediastinum. To our knowledge, this complication is rare and has been documented only once before in an isolated blowout fracture of the orbit.

Surgical management of Graves' ophthalmopathy: stage I, inferomedial orbital decompression.

Inferomedial orbital decompression was done on 26 eyes with dysthyroid optic neuropathy (DON), seven eyes with corneal exposure which had not responded to topical lubricants, and six with disfiguring exophthalmos. Twenty-one of the 26 DON eyes (80.7%) had visual acuity improvement equal to or greater than two lines by the Snellen chart, and there was only one in all 39 eyes that dropped more than two lines. The average retinal sensitivity improvement was of borderline significance (4.25 +/- 5.73 dB), but for those 13 severely affected eyes (preoperative sensitivity loss > or = 10 dB), 11 (84.6…

[Bilateral colobomatous microphthalmus with orbital giant cysts in incomplete tetrasomy 22].

A case of bilateral microphthalmos with orbital cysts is presented with clinical, echographic, CT and genetical findings. An incomplete trisomy 22 is shown as the likewise cause of the malformation. No further symptoms known to be part of the cat-eye-syndrome could be detected.

Three wall orbital decompression for Graves' ophthalmopathy via a coronal approach.

Ten patients with dysthyroid eye disease who underwent a three wall orbital decompression procedure performed via a coronal approach were reviewed. The indications, results, complications and surgical techniques involved in this surgery are discussed. We believe that this approach offers a number of advantages over other techniques and has a role in the management of carefully selected patients with dysthyroid eye disease who have severe unilateral or bilateral proptosis, with or without dysthyroid optic neuropathy.

Orbital and eyelid involvement with Erdheim-Chester disease. A report of two cases.

Erdheim-Chester disease is an idiopathic condition characterized by infiltration of the heart, lungs, retroperitoneum, bones, and other tissues by a fibrosing xanthogranulomatous process composed of xanthomatous histiocytes and Touton giant cells. This condition is often fatal, with death due to cardiomyopathy, severe lung disease, or chronic renal failure. Ocular findings with this potentially fatal disease are rare. We report the clinical and histopathologic findings in two cases of bilateral xanthelasmas and bilateral orbital infiltrates in association with Erdheim-Chester disease. The firs…

MR imaging of orbital mass lesions with contrast agent: a preliminary report.

To investigate the advantage and limit of contrast enhancement in the examination of orbital mass lesions, precontrast T1- and T2-weighted, and postcontrast T1-weighted spin-echo images were retrospectively compared by two experienced observers. Using contrast material, intraocular tumors were well detected and characterized, and several orbital tumors were characterized as to whether cystic or necrotic and intracranial involvement was better evaluated. On the other hand, tumor-fat interface lost conspicuity when tumors showed enhancement. We conclude that gadopentetate-dimeglumine-administere…
