Morbidity and cost-effectiveness analysis of outpatient analgesia versus general anaesthesia for testicular sperm extraction in men with azoospermia due to defects in spermatogenesis.

The outcome and costs of testicular sperm extraction under outpatient local analgesia or general anaesthesia were compared in men with non-obstructive azoospermia. Nineteen consecutive patients were allocated to receive general anaesthesia, while the subsequent 21 consecutive patients received outpatient analgesia in the form of i.v. midazolam sedation, lignocaine spray, scrotal infiltration with local anaesthetic and spermatic cord block. Blood pressure, pulse rate and respiratory rate were determined. Sedation and testicular pain were assessed by subjective scoring. Both groups showed haemod

Concomitant administration of recombinant human interleukin-2 and recombinant interferon alfa-2A: an active outpatient regimen in metastatic renal cell carcinoma.

PURPOSE: A phase II trial of interleukin-2 (IL-2) and interferon alfa (IFN-alpha) in metastatic renal cell carcinoma (RCCa) was conducted. A lower dosage of IL-2 was given via continuous intravenous (IV) infusion, a route with documented tumor activity associated with less toxicity, with the purpose of improving the therapeutic index of this treatment in an outpatient setting. PATIENTS AND METHODS: Thirty patients with metastatic RCCa were treated with the combination of IL-2 and IFN-alpha-2A. IL-2 was administered on days 1 through 4 of each treatment week, as a continuous IV infusion at a d

Oral Colchicine and Low-Dose Aspirin Combination Therapy for Non-elderly, Non-severe, Early Time From Onset, Adult Outpatients with Coronavirus Disease 2019 (COVID-19) during "The Fifth Pandemic Wave" in Japan.

BACKGROUND: Treatment with antiviral drugs for non-severe, early time from onset, adult outpatients with Coronavirus Disease 2019 (COVID-19) had not been established in 2021. However, some new variants of SARS-CoV-2 had caused rapid exacerbation and hospitalization among non-elderly outpatients with COVID-19, contributing to widespread crises within healthcare systems. METHODS: From July to October 2021, we urgently assessed a therapeutic program using oral colchicine (1.0 mg loading dose, followed approximately half a day later by 0.5 mg twice daily for 5 days, and then 0.5 mg once daily for

Drug survival superiority of tumor necrosis factor inhibitors and interleukin-17 inhibitors over Janus kinase inhibitors and interleukin-12/23 inhibitors in German psoriatic arthritis outpatients: retrospective analysis of the RHADAR database.

OBJECTIVE: Treatment options with disease-modifying antirheumatic drugs (DMARDs) for psoriatic arthritis (PsA) have evolved over recent years. In addition to Janus kinase inhibitors (JAKi), four classes of biologic DMARDs (bDMARDs; interleukin [IL]-23 inhibitors [IL-23i], IL-12/23 inhibitors [IL-12/23i], tumor necrosis factor inhibitors [TNFi], and IL-17 inhibitors [IL-17i]) are currently approved for moderate to severe PsA treatment. There is minimal evidence of the persistence of these drugs among PsA outpatients in a real-world scenario during the period following the approval of JAKi. Ther

Nutritional Needs in Mental Healthcare: Study Protocol of a Prospective Analytic Observational Study Assessing Nutritional Status, Eating Behavior and Barriers to Healthy Eating in Psychiatric Inpatients and Outpatients Compared to Healthy Adults.

BACKGROUND: Mental disorders are often associated with unhealthy eating behaviors and metabolic comorbidities. This can result in reduced life expectancy and poorer quality of life in people with mental disorders. This study protocol describes an observational study that examines the nutritional status and eating behaviors of people with severe mental illness (SMI) and the need for psychiatric treatment who were between 18 and 65 years old. In addition, the study focuses on possible barriers for healthy eating that have not yet been examined in this population. METHODS: A total of 192 study p

Left bundle-branch block is associated with increased 1-year sudden and total mortality rate in 5517 outpatients with congestive heart failure: a report from the Italian network on congestive heart failure.

BACKGROUND: A deleterious effect of complete left bundle-branch block (LBBB) on left ventricular function has been established. Nevertheless, the independent effect of a widened QRS on mortality rate in congestive heart failure (CHF) is still controversial. Therefore, we carried out this analysis to determine whether LBBB is an independent predictor of mortality in CHF. METHODS AND RESULTS: We analyzed the large Italian Network on CHF Registry of unselected outpatients with CHF of different causes. The registry was established by the Italian Association of Hospital Cardiologists in 1995. Comp

Updated UK good practice recommendations for outpatient parenteral antimicrobial therapy (OPAT): ensuring quality of care and antimicrobial stewardship in an evolving healthcare landscape.

Outpatient parenteral antimicrobial therapy (OPAT) offers safe, effective and patient-centred care for patients with infections requiring IV or complex oral antimicrobial therapy. The UK OPAT Good Practice Recommendations (GPRs) have recently been updated through a systematic literature review, expert consensus and consultation. In this article we outline the key changes in the updated GPRs. There are five new recommendations on (i) self-administration, (ii) OPAT in hard-to-reach groups, (iii) patient education, (iv) developing monitoring protocols for patients on complex oral antimicrobials a

Early outpatient follow-up, hospital-free survival, and health-related quality of life in older adults after hospitalization for acute myocardial infarction.

Current evidence regarding the benefits of early outpatient follow-up for older adults after acute myocardial infarction (AMI) is conflicting, and prior studies may have been impacted by confounding by indication by not accounting for geriatric vulnerabilities. We used data from a unique nationwide cohort of older adults hospitalized with AMI, linked to Medicare data, that captured rich data on geriatric vulnerabilities and other potentially influential covariates. We used inverse-probability of treatment-weighted analyses to evaluate associations of early outpatient follow-up with hospital-fr

High rates of viral co-detection in outpatient children with acute respiratory infections after the easing of preventive measures against Covid-19 in Germany.

BACKGROUND: Following the easing of preventive, non-pharmaceutical COVID-19 interventions (NPIs) during the late phase of the pandemic, acute respiratory infections (ARI) in children reemerged. We investigated the viral etiology, seasonality and clinical characteristics of children with ARI treated in primary care practices during this period. METHODS: We conducted a prospective, observational study on children ≤ 14 years of age, presenting with acute upper (URTI) or lower (LRTI) ARI in five pediatric primary care practices in Wuerzburg, Germany, from 10/2021 to 05/2022. Impact of an antimicrobial stewardship intervention on clinical and economic outcomes in outpatient parenteral antimicrobial therapy (OPAT) for urinary tract infections: A quasi-experimental study.

INTRODUCTION: Urinary tract infections (UTIs) are a common indication for Outpatient Parenteral Antimicrobial Therapy (OPAT) and often involved prolonged intravenous (IV) treatments. A theory-informed Antimicrobial Stewardship (AMS) intervention was implemented to optimize antibiotic duration and promote IV-to-oral switching in OPAT patients with UTIs. METHODS: This pre-post quasi-experimental single-centre study included adults with UTIs managed through OPAT. The intervention was designed using behaviour change techniques, and structured as a bundle of quality improvement strategies to optim

Early Outpatient bFGF-Based Wound Care for Foot Lesions in Haemodialysis Patients: A Real-World Stepwise Limb-Salvage Strategy From Japan.

Haemodialysis patients face a markedly elevated risk of foot lesions because of coexisting diabetes mellitus (DM), peripheral arterial disease (PAD) and uremia-related impairment of wound healing. Structured outpatient strategies and the optimal place of basic fibroblast growth factor (bFGF) within a stepwise algorithm remain poorly defined. To report complete healing and its durability in haemodialysis patients treated with bFGF for foot lesions, and to propose a stepwise limb-salvage algorithm with prespecified escalation criteria in which bFGF precedes dehydrated human amnion/chorion membra

[Impact of deficient communication in chronic progressive diseases in outpatient (i.e. pre- and posthospital) and hospital care - Illustrated by COPD and idiopathic pulmonary fibrosis as examples].

In the outpatient (i.e., pre- and post-hospital) and inpatient care of patients with chronic progressive diseases, such as chronic obstructive pulmonary disease (COPD) or idiopathic pulmonary fibrosis (IPF), there are serious communication gaps across sectors, particularly regarding advanced care planning and palliative care treatment. Strategies to avoid over-treatment in intensive care are needed.

Patient-centered care and associated factors among outpatient attendees in primary hospitals in South Gondar Zone, Northwest Ethiopia: a mixed methods study.

BACKGROUND: According to the Picker Institute model, patient-centred care (PCC) includes respect for patients’ values, preferences, and expressed needs; information, communication, and education; access to care; emotional support; involvement of family and friends; continuity and transition; physical comfort; and coordination and integration of care. It is one of the six aims for quality improvement guiding today’s health care systems. However, PCC remains underutilized by both patients and professionals, and significant challenges persist in its implementation worldwide. Additionally, few stu

Intimate Partner Violence and Mental Well-Being Among Married Women Attending Psychiatric Outpatient Clinics: The Roles of Childhood Trauma and Self-Compassion.

Background/Objectives: Intimate partner violence (IPV) is a public health problem associated with adverse mental health outcomes among women. Although prior research has focused on psychological distress (PD), mental well-being (MWB) is a distinct dimension of mental health that has received less attention in IPV research, particularly in clinical samples. Childhood trauma (CT) and self-compassion (SC) may contribute to variability in IPV-related outcomes. This study examined associations between IPV and MWB, considering SC, PD, and CT within an integrated framework. Methods: In this cross-sec

Development of and satisfaction with a short-term outpatient preventive medical service for working informal caregivers: a study protocol of a non-randomized prospective pilot study.

BACKGROUND: About 80% of care receivers (CRs) in Germany are cared for at home by family members or friends. These informal caregivers (CGs) often report a high burden from caregiving, which negatively affects their mental and physical health and often results in reduced work hours. CGs with statutory health insurance can apply for preventive medical services. A change in the law has allowed the duration of service to be split. Recent research postulated specialized digital and analog multi-component interventions to protect CGs' health, which the pAKur project wants to develop and implement i
